Arriving too late to make his fortune he decided to join the US Army.??Two and a half years later, after seeing action in the Boxer Rebellion and the Philippines Insurrection, Sydney returned to England where he met and married Elsa de Verde Verder, a lady from an affluent Vermont family. A year later Sydney joined the Kings Colonials Imperial Yeomanry, later renamed the King Edward's Horse. ??Still seeking excitement, in August 1914 he transferred to the Royal Flying Corps and in 1916 went to France with 23 Squadron to fly the FE2b. Life expectancy for pilots on the front lines was very short and he was badly wounded while gun-spotting over enemy lines. After several months recovering he was posted to Turnberry as Chief Instructor and on the 13th August 1917, he was posted to Marske (by-the-Sea), with the rank of Lieutenant Colonel, to form and command No.2 Fighting School. In 1919 he was awarded the Air Force Cross.??But war was not quite finished with Sydney. In 1936, fleeing imminent bankruptcy, he became involved with the International Brigades in the Spanish Civil War. Returning back to England he joined the RAFVR (Royal Air Force Volunteer Reserve) and when the Second World War broke out he was posted to Turnhouse as Section Controller. This really didn't suit him and, determined to see more action, at the age of 58, he arranged a transfer to France as Adjutant with No 1 Squadron where his duties included liaising with the French Air Force. He later transferred to 1 ATS near Perpignan and was one of the last to leave France with the German Blitzkrieg only a few hours away. The 2010 Patient Protection and Affordable Care Act required the Department of Health and Human Services (HHS) to enlist the Institute of Medicine (IOM) in examining pain as a public health problem. In this report, the IOM offers a blueprint for action in transforming prevention, care, education, and research, with the goal of providing relief for people with pain in America. To reach the vast multitude of people with various types of pain, the nation must adopt a population-level prevention and management strategy. The IOM recommends that HHS develop a comprehensive plan with specific goals, actions, and timeframes. Better data are needed to help shape efforts, especially on the groups of people currently underdiagnosed and undertreated, and the IOM encourages federal and state agencies and private organizations to accelerate the collection of data on pain incidence, prevalence, and treatments. Because pain varies from patient to patient, healthcare providers should increasingly aim at tailoring pain care to each person's experience, and self-management of pain should be promoted. In addition, because there are major gaps in knowledge about pain across health care and society alike, the IOM recommends that federal agencies and other stakeholders redesign education programs to bridge these gaps. Pain is a major driver for visits to physicians, a major reason for taking medications, a major cause of disability, and a key factor in quality of life and productivity. Given the burden of pain in human lives, dollars, and social consequences, relieving pain should be a national priority.

Military doctors during the conflict on the Western Front observed and personally experienced psychiatric states they had never witnessed before. This text reviews the published medical literature of that era which graphically detailed the clinical states of hysteria (conversion disorder) and neurasthenia (anxiety and PTSD). Medical officers at the front evolved pragmatic medicinal, cognitive and behavioural interventions, still practised today, though never scientifically proven to be effective. The doctors, like their patients, endured numerous horrors at the front, which were, for many, to influence their post-war personal and professional lives. Much of what they wrote was forgotten and deserves reconsideration. Neuropsychiatry was founded in the shell craters of Flanders.

Book excerpt: Unskilled urban workers made up the bulk of Irish volunteers who fought in the British army during the First World War, and Sir Roger Casement described them as being "not Irishmen but English soldiers". In this book, the case of an illiterate general laborer, born in 1876 in Waterford city, who enlisted in the 16th (Irish) Division is used to study the motivation of Catholics enlisting in the British army and to assess the credibility of Casement’s judgment which, the book argues, is too simplistic. The decision to enlist resulted from a complex range of external social, economic and political pressures to which men were subjected during the course of their lifetimes. These are examined in detail and arguments are supported with graphs, charts, tables and numerical calculations. The case of the men enlisting in the British army is considered from three perspectives: via a study of Waterford’s community as representative of the social, economic and political relationships of southern Ireland as a whole; through the presentation of ground-breaking evidence and analysis of more immediate reasons for enlistment; through an examination of why, having enlisted, Irishmen remained loyal to the British army and the 16th (Irish) Division in particular.
