Estos sistemas, al igual que la monitorización fetal y la ecografia tienen como soporte físico los ultrasonidos, los cuales han revolucionado la obstetricia en las últimas décadas: *Detección de la frecuencia cardiaca fetal, lo cual asociado al registro de las con-tracciones uterinas lleva al desarrollo de la monitorización fetal.*La ecografía, que permite conocer la morfología y el crecimiento fetal intraútero.*Finalmente, el principio Doppler que ha permitido la determinación de flujos sanguíneos en diversos vasos, tanto maternos como fetales.El Doppler se está mostrando como un arma muy importante en el control de embarazos de alto riesgo, superior en muchos aspectos a otros métodos más clásicos de control fetal. Son fundamentalmente los estados hipertensivos del embarazo y el crecimiento intrauterino restringido, las situaciones donde el estudio Doppler muestra su mayor utilidad.Las posibilidades y los límites de esta técnica serán más comprensibles si se conocen los principios físicos en que se fundamenta. Por ello creemos que es absolutamente imprescindible antes de su aplicación clínica el conocimiento de éstos por parte del explorador y seguir una correcta metodología. De esta forma podremos evitar errores y sacar partido a estos sistemas, que son una de las más importantes novedades en la clínica ginecológica y obstétrica de los últimos años, además junto a los sistemas de ecografia convencional y monitorización fetal, nos ayuda a un mejor conocimiento del estado biofísico fetal.

The fea sibility of the medical application of Doppler sonography was demon strated more than three decades ago, and the first report of Doppler velocimetry of the fetal circulation was published two decades ago. Over the years, impressive technological innovation and advances in clinical research have promoted the use of this method in medical practice. There also has been phenomenal expansion in research on the obstetrical and gynecological applications of Doppler sonography. In obstetrics, Doppler sonography has allowed unprecedented insight into the pathophysiology of human fetal response to in utero stress. This ushered in the possibility of using the method for antepartum fetal surveillance, so that timely recognition of fetal compromise and appropriate intervention could improve perinatal outcome. Numer ous clinical studies mostly have confirmed, but occasionally have re futed, the diagnostic efficacy of Doppler velocimetry in various preg nancy disorders. More significantly, the benefits of umbilical arterial Doppler investigation in reducing perinatal mortality in high-risk pregnancies have been affirmed by randomized clinical trials. In addi tion, the introduction of fetal Doppler echocardiography has not only tremendously increased our understanding of human fetal central circulation in health and disease, but also has enhanced our ability to diagnose congenital heart disease in utero. In addition, significant strides have been made in the gynecological applications of Doppler sonography, including general gynecology, infertility, and oncology.
