The dilatation of an artery which in the human subject would be considered aneurism, is normal in some of the Cetacea. The air-cells in the lungs of turtles have naturally a development, which in man would be emphysematous; and again, the normal thickness of the parietes of the heart in birds, and of the stomach in horses, represents what is known as hypertrophy in the human organism. In some of the Carnivora, and in birds of prey, the condition of the blood and the secretions, together with the action of the heart, and that of the respiratory organs, present a group of symptoms not very unlike what physicians designate inflam matory fever. Disease is often distinguished as structural or functional. Thus, hypertrophy of the heart is a structural disease, but mere palpitation is functional. Some have maintained that all diseases must be structural, because the deranged action of an organ implies a fault in the machinery. But, without insisting on such objec tions to this view as might be grounded on the fact, that the action of a machine may be disturbed by a disproportionate exertion of the impelling power, inde pendently of any fault in its works, we content ourselves with remarking, that disease of the structure itself must have been preceded by disordered action; that is, disordered nutrition of the part. The terms in question, however, are applied principally to the affections of special organs, rather than of their constituent tissues; and it must be allowed that there are very few instances in which the action of the organ is deranged, without alteration in some of its parts. The organ, strictly speaking, consists not only of the solid structure, but also of the fluid, which traverses it; and therefore any abnormal quantity or condition of this huid comes properly under the denomination of organic disease. When, however, they consider how widely and for how many ages doctors have differed as to the causes of the complaints herein discussed, they may be willing to excuse a man from the people for stepping in to settle the dispute. The intimate relation which these diseases bear to our mode of life and the sani tary condition of our dwellings has long been known. This relation, universally acknowledged, but imperfectly under stood, has caused the writer deep thought, the result of which is the present treatise. A physician's only end and aim is too often to effect a cure, not to ascertain first the exact cause of the illness. But people should first be instructed in the causes of dis ease; they will then know how to avoid them. The writer knows nothing of medicine, excepting the remedies that are Nature's own, and has, consequently, avoided all medi cal terms and technical phrases; and while no one could appreciate more highly than he the attainments and ser vices of the members of the medical profession, yet he ventures the opinion that Nature may reveal some of hersecrets of health and disease to an earnest student outside the halls of a medical college perhaps more readily than to one within. It will remain for physicians and scientists, who have the requisite knowledge and the appliances nec essary for further investigation to prove the truth of what is here advanced, and to devise cures for the diseases dis cussed, their causes having been plainly stated. Book excerpt: The past 25 years have seen a major paradigm shift in the field of violence prevention, from the assumption that violence is inevitable to the recognition that violence is preventable. Part of this shift has occurred in thinking about why violence occurs, and where intervention points might lie. In exploring the occurrence of violence, researchers have recognized the tendency for violent acts to cluster, to spread from place to place, and to mutate from one type to another. Furthermore, violent acts are often preceded or followed by other violent acts. In the field of public health, such a process has also been seen in the infectious disease model, in which an agent or vector initiates a specific biological pathway leading to symptoms of disease and infectivity. The agent transmits from individual to individual, and levels of the disease in the population above the baseline constitute an epidemic. Although violence does not have a readily observable biological agent as an initiator, it can follow similar epidemiological pathways. On April 30-May 1, 2012, the Institute of Medicine (IOM) Forum on Global Violence Prevention convened a workshop to explore the contagious nature of violence. Part of the Forum's mandate is to engage in multisectoral, multidirectional dialogue that explores crosscutting, evidence-based approaches to violence prevention, and the Forum has convened four workshops to this point exploring various elements of violence prevention. The workshops are designed to examine such approaches from multiple perspectives and at multiple levels of society. In particular, the workshop on the contagion of violence focused on exploring the epidemiology of the contagion, describing possible processes and mechanisms by which violence is transmitted, examining how contextual factors mitigate or exacerbate the issue. Contagion of Violence: Workshop Summary covers the major topics that arose during the 2-day workshop. It is organized by important elements of the infectious disease model so as to present the contagion of violence in a larger context and in a more compelling and comprehensive way.
