Book excerpt: Overweight among children and adolescents is a significant public health problem in the United States. Epidemiologic studies must scale up efforts to assess risk factors of overweight, by improving measurement and methodological analyses in order to frame and refine overweight prevention policies and programs. The objectives of this study were to determine the relative contribution of physical activity, dietary factors, television/video viewing, and computer use to weight status among adolescents, as well as to determine if the association between physical activity and weight status differs by gender or race. Secondary analyses of the nationally representative NHANES (2003-2006) cross-sectional surveys were conducted among 2, 136 U.S. adolescents. The dependent variable was weight status, categorized as: normal, at-risk for overweight, and overweight; the main independent variables were moderate-tovigorous physical activity, dietary intake (total energy intake, total fat intake, fiber intake), daily hours of television/video viewing, and daily hours of computer use. Multivariate statistical analyses were conducted using multinomial logistic regression models to obtain odds ratios and 95% confidence intervals. Gender and race were also analyzed as potential effect modifiers on the relationship between physical activity and weight status, by modeling multiplicative interaction terms, testing for departure from homogeneity of the point estimates between categories of the effect modifiers. After controlling for confounders, television/video viewing was significantly associated with both at-risk for overweight (OR = 1.57, 95% C.I = 0.94, 2.63) and overweight (OR = 1.84, 95% C.I. = 1.24, 2.69), meanwhile television/video viewing (OR = 1.84, 95% C.I. = 1.24, 2.69), physical activity (OR = 0.75, 95% C.I = 0.59, 0.95), and fiber intake (OR = 0.96; 95% C.I = 0.92, 1.00), were significantly associated only with overweight. Television/video viewing was found to be an important factor associated with overweight, in addition to physical activity, and fiber intake. The results support previous findings showing television viewing as a determinant of overweight among adolescents. Increased television/video viewing may reduce time spent in physical activity, exposure to high energy dense food advertising, and increased food intake. The findings do not support a multiplicative interaction effect of gender or race, suggesting universal overweight prevention policies and programs should be delivered. -- Abstract.

The purpose of this study is to examine whether participation in a creative, problem-solving program is related to overweight/obesity, other sedentary pursuits, physical activity, and diet. Methods: This study describes select health behaviors that are believed to be related to body weight status for 1,679 adolescents in grades 6-12 participating in the 2008 Destination Imagination (DI) Global Finals. Dietary, physical activity (PA), and sedentary behavior data was collected via self-reported survey based on the 2007 Youth Risk Behavior Survey. Measured height and weight data were collected to calculate BMI percentile. Using multinomial regression, models were constructed to predict the likelihood of being overweight/obesity compared to normal weight. Results: The prevalence of overweight in the study population was 15.6% and obese was 11.4%. Fruit/vegetable intake, milk consumption, moderate/vigorous PA, television time, and total screen time (ST) were all independently associated with weight status. After controlling for all covariates, male gender, minority racial/ethnic status, and high ST while not meeting the PA recommendation significantly increased the likelihood of overweight/obesity. DI exposure does not appear to be independently associated with, nor influence the relationship between, the above variables and the risk of overweight/obesity. Conclusion: The participants in the current study are leaner and engage in healthier behaviors compared to national data for adolescents. Even among these low risk adolescents, the interaction between sedentary and physical activity behavior, presented as an interactive measure, was strongly associated with weight status.

Physical activity and diet are both important factors associated with weight management, and current recommendations are to consume a diet low in saturated fat and high in fiber, fruit and vegetables and to participate in frequent and regular physical activity. Adherence to recommendations is low, a factor that is strongly correlated with development of obesity and associated chronic diseases such as hypertension and cardiovascular disease. While associations between diet and physical activity are well established, investigation of changes in their association during growth is lacking. This thesis uses five years of diet, physical activity, and anthropometric data from 2379 adolescent girls in the National Heart Lung and Blood Institutes, Growth and Health Study to explore associations among diet, physical activity, and obesity cross-sectionally and with age. Variables representing physical activity, diet quality, and obesity, as well as income, maturation stage, and other potential confounders, were evaluated pair-wise for correlation, and bivariate statistics were examined for longitudinal trends. For further evaluation of relationships between groups of variables we used a canonical correlation analysis. First, physical activity variables were grouped with confounders and examined in relationship to diet quality variables; next, we grouped physical activity, diet quality, and confounders and examined the relationship to obesity measures. We found a moderately increasing correlation between physical activity and diet with age and an age-related decrease in correlation of all health behaviors and confounding variables with obesity measures, indicating that obesity measures become less sensitive to behaviors and socioeconomic factors with age at the same time as health behaviors become more tightly linked. These results suggest that while health behaviors continue to interact during growth, and in fact become more intertwined, measures of obesity become more static and may be less responsive to potential interventions with increasing age. These findings should motivate intervention work to aim for youth as potential impact would be greater before health behaviors and obesity measures become "locked in" to the more static frame of adulthood.

During the early 1970s, 3 to 6 percent of American children and adolescents were obese. By 2004, this number had increased five fold to 16 to 18 percent of all US 6 to 19 year olds. This increase is especially troubling as obese children and adolescents have a greater risk for adult obesity, with its attendant health risks, and may experience obesity-related health conditions before adulthood, including type 2 diabetes mellitus, fatty liver disease, and elevated cardiovascular risk factors. Severe obesity in children and adolescents can be associated with severe health consequences and dire impacts on quality of life. The true toll of childhood obesity must be calculated across the lifespan since it often continues into adulthood. Thus, an important step to preventing adult obesity and its related health consequences is effectively treating childhood obesity. To this end, we conducted this systematic review to determine which treatments could effectively address child and adolescent obesity and overweight, including behavioral, pharmacological, and surgical treatment options. In conjunction with a Technical Expert Panel, we developed a set of five key research questions to evaluate the effectiveness and safety of behavioral, pharmacological, and surgical treatments for obese and overweight children and adolescents who were 2 to 18 years old. These research questions addressed various measures of the health impact of treatments to reduce or stabilize weight, including: short-term impacts on weight control (6 to12 months after enrolling in treatment); maintenance of weight changes in the medium-term (between 1 to 5 years after enrollment) or longer-term (5 or more years after enrollment); adverse effects of treatment (immediate and over time); beneficial effects of treatment, aside from weight control or weight loss; and treatment components or other factors that influence the effectiveness of treatments.
