Book excerpt: The rapid developments in molecular genetics have clarified many of the muta tions in monogenic thyroid diseases over the last two decades; now the target of molecular thyroid genetics has become the oligogenic thyroid diseases. These include the autoimmune thyroid diseases and familial thyroid cancers, both of which are much commoner than the monogenic diseases. However, the method ological approach to the genetics of these more complex diseases is still far from being well established. Although the discovery of susceptibility genes has been partially accomplished in complex diseases such as asthma, Crohn's dis ease, and types I and II diabetes mellitus, the elucidation of susceptibility genes in complex diseases remains a major challenge. This volume contains papers presented at the International Symposium on the Genetics of Complex Thyroid Diseases. This meeting was held in association with the International Thyroid Congress in Kyoto in October 2000 and sup ported in part by the Japan Intractable Diseases Research Foundation and Knoll Pharmaceuticals Inc. The symposium was the first international symposium con cerning the genetics of complex thyroid diseases and was restricted to the study of the autoimmune thyroid diseases and familial thyroid cancer. Twenty distin guished researchers from the United States, the United Kingdom, France, Ger many, Italy, and Japan were invited. Each presentation precipitated intense dis cussion and there was much consensus during the meeting. Nevertheless, this volume will leave the reader with a clear understanding of how little we still know.

Abstract: Abstract of thesis entitled Pathology and molecular biology of malignant thyroid tumours submitted by Alfred King Yin Lam for the degree of Doctor of Medicine at the University of Hong Kong in August 2004 Thyroid cancer is the most common endocrine malignancy in Hong Kong. The thesis aims to delineate the clinicopathological features and molecular biology of malignant thyroid tumours in Hong Kong Chinese. Over a 30-years study period, 872 primary cancers and 140 secondary cancers of the thyroid gland were obtained in the pathology files of Queen Mary Hospital. The different types of malignant thyroid tumours may be difficult to differentiate from each other. Carcinomas comprised 97.3% of the primary thyroid cancers. The thyroid carcinomas included 652 papillary carcinomas (74.8%), 129 follicular carcinomas (14.8%), 22 poorly-differentiated carcinomas (2.5%), 38 undifferentiated carcinomas (4.4%), 4 squamous cell carcinomas (0.5%) and 2 carcinomas showing thymus-like differentiation (0.2%). The other malignant thyroid tumours included 23 lymphomas (2.6%) and two sarcomas (0.1%). The different malignant thyroid tumours had diverse pathological features and clinical behaviour. Cytokeratins expression profile was different in different types of thyroid carcinomas. Strong cytokeratin 19 expression was noted in papillary carcinoma, squamous cell carcinoma and carcinoma showing thymus-like differentiation of thyroid. Cytokeratin 10/13 was expressed in some undifferentiated carcinomas of thyroid and not in other thyroid carcinomas. The negative expression of cytokeratin 10/13 in primary squamous cell carcinoma of thyroid was a useful finding to differentiate from the positive expression in metastatic squamous cell carcinoma from upper aerodigestive tract. Cytokeratin 18 was positive in carcinomas of thymus-like differentiation of thyroid and was negative in squamous cell carcinomas of thyroid. p53 expression was seen in undifferentiated carcinomas and squamous cell carcinomas but not in other thyroid carcinomas. Also, the p53 expression was associated with tumour differentiation in primary squamous cell carcinomas of thyroid. RET oncogene activation was often detected in papillary thyroid carcinoma. The oncogene activation was seen more often in conventional papillary carcinoma. Telomerase activity was infrequently reactivated in thyroid cancers. Epstein Barr virus was detected in thyroid lymphomas but not in thyroid carcinomas showing thymus-like differentiation. The Epstein Barr virus genes expression in thyroid lymphomas showed type II latency pattern. In summary, identification of the different histological types of malignant thyroid tumours is mandatory for in depth medical research. The study of pathology and molecular biology of malignant thyroid tumours allows better understanding and management of the diseases. Preparations for the conference began 18 months earlier with the designation of a steering committee and the establishment of a dedicated, p- manent web site. The meeting took place on October 22 and 23, 2007 in Bethesda, Maryland and was co-moderated by Susan J. Mandel and Edmund S. Cibas. The discussions and conclusions regarding terminology and morphologic criteria 1, 2 from the meeting were summarized in publications by Baloch et al. and form the framework for this atlas. The atlas is organized by the general categories of “Nondiagnostic,” “Benign,” “Follicular Neoplasm/Suspicious for a Follicular Neoplasm”, “Suspicious for Malignancy,” and “Malignant,” and it includes the defi- tions and morphologic criteria of these categories as set forth by Baloch et al. The majority of the conference participants also agreed on a category of “undetermined significance,” which is incorporated in this atlas (Chap. 4). It is critical that the cytopathologist communicate thyroid FNA interpretations to the referring physician in terms that are succinct, unambiguous, and helpful clinically. We recognize that the terminology used here is a flexible framework that can be modified by individual laboratories to meet the needs of their providers and the patients they serve.
