Book Diagnosis and Surgical Treatment of Abdominal Tumors Classic Reprint

Download or read book Diagnosis and Surgical Treatment of Abdominal Tumors Classic Reprint written by Spencer Wells and published by Forgotten Books. This book was released on 2017-09-12 with total page 256 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Diagnosis and Surgical Treatment of Abdominal Tumors But there is one fact which stands out with ominous significance in all these records. Whatever may be our diagnostic, accuracy and our Operative skill, our success in the treatment of these diseases is fatally restricted by the influence of septicaemia. We have already reached as great an amount of success in the results of ovariotomy as can reasonably be hoped for, and shall in like manner approach such success in the results of the extirpation of other organs. But until we find some more certain protection for our patients against the ravages of septiczemia than any antiseptic precautions hitherto taken secure, abdominal surgery, though not without just claim to the credit of having done good service to humanity, must still be looked upon as a branch of our science and art which, still imperfect, calls for continued search for truth, and for constant efi'orts to improve methods of practice, from every surgical student. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Diagnosis and Surgical Treatment of Abdominal Tumors The book which I have now written is in many respects a new work, though it may be called a fourth edition of the first published in 1865. Twenty years have entirely changed the boundaries of the subject. Before 1860, ovariotomy sometimes succeeded, as often failed, and was very generally discredited. My first book was a record of cases, showing how difficulties were overcome, how principles were gradually brought into view, how rules of practice were established, and what results might be obtained. It silenced objections, and encouraged others to follow in the same course. My second book, in 1872, was no longer a plea for ovariotomy. An experience of 500 cases, which then seemed large, made me feel that, though I was still a learner, there was something I could usefully teach, and had no right to withhold. According to the views then held, I went into the diagnosis of ovarian disease and into the details of ovariotomy. There is evidence enough that my example and teaching were not unfruitful. Ten years more brought into the field many co-labourers who tilled the ground I had partially cleared, and no one is more ready than I am to acknowledge the value of what they have done, and to rejoice over the relief they have given to a multitude of sufferers. My own experience had more than doubled, some of my views had changed, in some respects my practice was modified, and I had information acquired from others to add to my own observations. The subject had expanded. Uterine pathology and surgery had been grafted upon that of the ovary, and my third book, in 1882, included both. Since that time the domain of abdominal surgery has not only spread over every abdominal organ, but has been taken possession of with equal zeal by the profession throughout the civilised world. The proof of this, so far as I am concerned, is that in less than two years a new edition of my last book is called for. I have adopted a new form, and issue it at a price which puts the information it contains at the command of every student. Suppression and condensation have enabled me to bring this work into smaller compass, and have also enabled me to make considerable additions on various conditions which call for the performance of abdominal section. Details which were interesting when novelties have been omitted. My time has been too fully taken up as a practical surgeon for me to do much as a pathological explorer. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Surgical Diagnosis of Tumors Opinions concerning the pathology, and especially concerning the genesis, of tumors, must be various. There is in pathology as yet no finished chapter. We are everywhere in the midst, or rather, often at the commencement, of our labor. But all that has been accomplished in the way of pathological anatomy and clinical observation has already led to very tangible results in practical treatment; and in my opinion we have made most important advances in the certainty of diagnosis of tumors. Book excerpt: Excerpt from Diagnosis of the Malignant Tumors of the Abdominal Viscera In planning the book, I believe I have everywhere been mindful of the problem of the earliest possible diagnosis, especially in the case of gastric, intestinal and renal neoplasms. In the case histories, the symptoms appearing first have received sharp emphasis, and I considered it important to give detailed discussion to the diagnostic value of the subjective phenomena which so frequently are the forerunners of the Objective findings. Download or read book Perioperative Chemotherapy written by U. Metzger and published by Springer Science & Business Media. This book was released on 2012-12-06 with total page 170 pages. Available in PDF, EPUB and Kindle. Book excerpt: One reason for failure to cure solid tumors by surgery appears to be the impossibility of controlling metastases that are present but latent at the time of operation. This failure is a common clinical experience with aggressive neoplasms. but it is not always appreciated in tumors with longer survival times. e. g .• breast and colon cancer. In addition. recent evidence indicates that after resection of a primary tumor micrometas tases from it might be enhanced by suppression of immune and reticu loendothelial functions of the host. Other factors, such as increase of coagulability and stress in the perioperative period, can also promote tumor growth. The development of new metastases might be facilitated by cells forced into the circulation during operative manipulations. Such events could be important for the outcome of treatment and it is suggested that preventive measures should be directed to this systemic component of solid tumors. Radical surgery can reduce the number of tumor cells to a subclinical 3 6 stage (10 to 10 cells) in which chemotherapy might be more effective than in advanced stages. Chemotherapy, on the other hand, might aggravate the surgical morbidity by influencing the wound healing pro cess, by decreasing the immune response, and/or by toxicity to the bone marrow and to the gastrointestinal tract, for example.

Download or read book Management of Gastric Cancer written by Paul H. Sugarbaker and published by Springer Science & Business Media. This book was released on 2012-12-06 with total page 405 pages. Available in PDF, EPUB and Kindle. Book excerpt: Gastric cancer has been one of the great malignant scourges affecting man kind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.
