The private practice of a local endocrinologist lacks a formal diabetes education program. Diabetes self-management education training (DSME/T) is necessary in the early stages of the disease and as needed throughout the individual's life to ensure optimal glycemic control. The purpose of this evidence-based practice (EBP) change project was to develop and provide a comprehensive diabetes education program to patients with Type 1 or Type 2 diabetes. The intended outcome of the patient educational program was improved self-management strategies and increased knowledge regarding diabetes. A secondary purpose was to develop a consistent, patient-focused education program for individuals with diabetes in southern New Jersey. This was accomplished by implementing a comprehensive diabetes education program utilizing the 2012 National Standards for Diabetes Self-Management Education. A series of three educational classes were conducted over a three-week period. Statistical analysis of pre-intervention, conclusion, and four weeks after intervention scores on the Summary of Diabetes Self Care Activities questionnaire was utilized to validate the effectiveness of the evidence-based practice change project. Implementing a comprehensive diabetes education program has demonstrated to be an effective means to achieve national guidelines for the treatment of diabetes. Nursing implications are multiple but the most important is the development of a comprehensive diabetes education program. The EBP change project has contributed to increased self-management strategies, improved blood sugar control, and improved quality of patient care." -- Abstract.
