Book excerpt: La réhabilitation prothétique implanto-portée pour les patients édentés est une modalité de traitement bien établie et hautement prévisible. Au cours des dernières décennies, l'incorporation de la technique d'imagerie tridimensionnelle par l'introduction de la tomographie à faisceau conique (CBCT) a permis une avancée importante en dentisterie. Grace à ces technologies, il est désormais possible de déterminer à l'avance la position tridimensionnelle précise de l'implant prévu avant l'insertion effective de celui-ci, et de transférer cette position sur le site chirurgical. Le concept de chirurgie mini-invasive consiste à tirer parti des progrès en chirurgie dentaire afin d'effectuer des interventions chirurgicales en causant le moins de dommages possibles aux patients. La mise en charge immédiate (MCI) des prothèses d'arcade complète sur implants représente aujourd'hui une solution idéale. L'objectif de cette revue de la littérature est d'examiner la survie des prothèses complètes immédiates et des implants chez les patients traités à l'aide d'une chirurgie implantaire sans lambeau assistée par ordinateur, d'analyser les complications observées en per et post opératoire de ce type d'intervention et d'évaluer les appréciations des patients vis-à-vis de ce traitement. Les résultats de cette revue, vont montrer que la chirurgie guidée par ordinateur apporte de nombreux avantages à la chirurgie implantaire, avec des taux de survie moyen implantaire et prothétique élevés de 98% et 97% respectivement. La littérature scientifique existante démontre que la chirurgie guidée sans lambeau peut avoir des avantages en diminuant la douleur et l'inconfort des patients dans la période postopératoire.

Book excerpt: Introduite il y a une dizaine d'années, l'implantologie assistée par ordinateur (IAO) change l'approche du chirurgien. Alors qu'avant les techniques conventionnelles comportaient une certaine marge d'erreur par rapport au projet implantaire, PIAO permet une mise en place des implants et des prothèses beaucoup plus précise. Cela nous amène à aborder le domaine de la mise en charge immédiate sans lambeau en prothèse fixée. En effet, de nombreux patients ne supportent plus une prothèse amovible le temps de l'ostéo-intégration implantaire. Or aujourd'hui, on peut traiter le patient par une prothèse fixe, provisoire ou d'usage, ce qui permet un certain confort du patient et le contrôle prothétique tout au long du processus d'ostéo-intégration. L'IAO avec technique sans lambeau a l'avantage de la préparation prothétique au préalable avec un minimum de séquelles post-opératoires et des ajustements prothétiques minimes. Malgré ces avantages, il existe des limites à cette application. Les patients doivent être soigneusement sélectionnés. Il faut une hauteur osseuse et des tissus mous adéquats pour placer l'implant en position idéale par rapport à la restauration prévue et pour stabiliser le guide chirurgical. Aussi, des complications biologiques, prothétiques et esthétiques peuvent apparaître à court, moyen et long terme.

La mise en œuvre de ces deux protocoles va dans le sens de l'évolution des principes qui régissent l'implantologie. En effet, la deuxième le partie de ce travail montre, d'une part, tout l'intérêt de réaliser une chirurgie sans lambeau moins invasive, et d'autre part, le changement de critères d'obtention de l'ostéointégration érigés par Branemark vers une simplification du protocole clinique et sa meilleure acceptation par les patients. La dernière partie décrit les principes et les protocoles de traitement par le système NobelGuideTM qui constitue une application fiable des concepts de planification assistée. Le transfert de la simulation implantaire en bouche, par l'intermédiaire d'un guide stéréolithographique à appui muqueux, est précis et reproductible.
