Book excerpt: A tese tem como objetivos colaborar na elaboração de políticas públicas, discutindo as estratégias de implantação de Programas de Saúde do Trabalhador, especialmente na vigilância, investigação de causas e prevenção de acidentes do trabalho. Tem ainda como objetivos específicos reconstruir a estruturação do Programa de Saúde do Trabalhador (pS1) de Piracicaba, buscando através da sua história (1997-2002) identificar os elementos centrais que permitiram a construção de um espaço privilegiado interinstitucional para a efetiva atuação preventiva. Quanto ao método utiliza-se de abordagem qualitativa, por meio de pesquisa documental junto aos materiais produzidos pelo PST de Piracicaba, relativos ao período de 1997 a 2002; apresenta estudo de 04 casos de acidentes do trabalho fatais ocorridos nos anos de 1998, 1999 e 2000 e seus desdobramentos preventivos; apresenta o estudo comparativo de dois casos investigados tanto pelo PST como pelo Ministério de Trabalho e Emprego de Piracicaba e pelo Instituto de Criminalística (IC), verificando as diferenças de abordagens entre os laudos; por fim realiza pesquisa documental e estudo analítico descritivo junto aos 71 Laudos do IC de Piracicaba referentes aos anos de 1997, 1998, 1999 e 2000. São identificados quatro elementos centrais da construção interinstitucional de Ações Preventivas no Município de Piracicaba: necessidades, demandas, sistema de informações e priorização de ações. Na investigação de quatro casos de acidentes fatais dos setores da construção civil e da fabricação de papel e embalagens, utilizando-se o método da Árvore de Causas (ADC), constata-se a falta de medidas básicas de segurança em máquinas e equipamentos bem como em elevadores de carga em obras da construção civil. Após a aplicação de penalidades administrativas, os casos dão origem a processos de negociação tripartite setorial, para a prevenção de novos acidentes. É efetuada ainda a comparação dos relatórios e laudos oriundos da investigação dos acidentes efetuada tanto pelo Programa de Saúde do Trabalhador como pelo Instituto de Criminalística, visando avaliar os resultados e comparar as duas abordagens. A pesquisa revela a importância de ações interinstitucionais, a parceria entre o Programa de Saúde do Trabalhador da Secretaria Municipal de Saúde de Piracicaba, com membros do Ministério do Trabalho e Emprego, a integração do serviço com as universidades locais e com representantes da sociedade civil na prevenção e promoção da saúde. Propõe-se ao final um sistema de vigilância aos acidentes _aves e fatais, nos moldes de eventos sentinela, que inclui um sistema de informação com várias fontes, a busca ativa de casos (locais de atendimento, sistemas de resgate, delegacias de polícias) e o registro das ocorrências na rede de pronto atendimento desencadeando posteriormente ações preventivas ou negociações coletivas tripartites e setoriais para prevenção dos riscos mais relevantes. Revela ainda que o Instituto de Criminalística utiliza na investigação dos acidentes métodos e concepções tendenciosas para a apuração das causas, com base num modelo teórico ("ato inseguro" das vítimas) que serve para descarregar no trabalhador toda a responsabilidade por estes eventos, descompromissando os empregadores com a ocorrência destes eventos, em detrimento de uma política de prevenção e apuração mais aprofundada de suas causas e das responsabilidades decorrentes dos mesmos.

It proposes a cycle of expansive learning that evolves from recognition of the problem to the visualization, testing and consolidation of solutions. The Change Laboratory method was first developed by Finnish researchers in the 90s and has been improved since then by an international network of research centers in ten countries.
