Ces molécules sont symptomatiques et non curatives et génèrent d'importants effets indésirables, avec une efficacité parfois mise en doute. En effet, les problèmes de développement de dépendance et de tolérance sont une des causes de surconsommation de ces molécules. Au fur et à mesure du temps sont mis en évidence des liens entre les benzodiazépines et les risques de chutes chez les personnes âgées, les accidents de la route et l'apparition de troubles cognitifs évocateurs de démences incidentes. Il est donc nécessaire de trouver des outils pour prévenir ces risques étant donné le fort taux de population consommant des anxiolytiques/hypnotiques, de lutter contre ces problèmes de dépendance et d'apparition de syndrome de sevrage générateurs d'une consommation chronique de benzodiazépines. Des solutions peuvent être apportées pour limiter cette consommation de benzodiazépines comme par exemple l'action sur les primoprescriptions ou le développement de campagnes d'informations destinées aux patients. En effet, la formation initiale des pharmaciens ainsi que des médecins généralistes est insuffisante dans le domaine de la santé mentale alors qu'ils sont en première ligne en ce qui concerne la prise en charge des patients souffrant d'anxiété et d'insomnie. Il serait également intéressant de développer la formation médicale continue ainsi que la formation pharmaceutique continue dans le domaine de la psychiatrie pour se rendre compte de l'évolution des pratiques et des pathologies. Parmi les alternatives à ces traitements, il est nécessaire de souligner l'importance que peuvent prendre les thérapies, en particulier les thérapies comportementales et cognitives, dans les traitements des troubles anxieux et du sommeil avec des études d'efficacité significatives à l'appui.

Si leur efficacité à court terme est largement reconnue, leur utilisation sur le long terme est plus discutable. En effet, au fil des années leur consommation est devenue problématique de par le manque de pertinence clinique de certaines prescriptions et le phénomène de dépendance qu'elles peuvent impliquer. Cela a mené à une surconsommation de ces molécules dont les enjeux pour la santé publique sont considérables. Ce travail propose donc de faire un état des lieux de la consommation des benzodiazépines en France et de ses enjeux, puis de mettre en évidence les déterminants de leur prescription en médecine générale, afin de dégager des perspectives d'amélioration de cette prescription et de lutte contre la dépendance.

They frighten the patients who experience them; they frighten those who witness them; they also frighten many physicians who have to deal with them. Most individuals with seizures present to family physicians or to emergency room physicians. However, despite the fact that seizures are among the most common neurological conditions, most general practitioners, family practice specialists, and intemists do not see large numbers of patients with seizures. Given the apoplectic appearance of generalized tonic clonic convulsions, it is not difficult to understand why they arouse such emotional responses in those that experience them, those that witness them, and those whose care is sought for them. Seizures are symptoms of something wrong with the brain. Many different kinds of perturbations in brain anatomy, chemistry, or physiology can produce seizures. For many individuals, seizures occur in the context of an acute illness and will not recur once that illness is treated. These individuals do not have epilepsy. They have transient disturbances in brain function attributable to systemic medical conditions. It is important to recognize these issues, because, first, the seizure may be the initial, or even only, manifestation of the underlying medical problem and this needs to be recognized.
