Book Dental and Skeletal Outcomes for Class II Surgical orthodontic Treatment

Download or read book Dental and Skeletal Outcomes for Class II Surgical orthodontic Treatment written by Brittany Leigh Weaver Potts and published by . This book was released on 2009 with total page 53 pages. Available in PDF, EPUB and Kindle. Book excerpt: Abstract: Introduction: The information that details dental changes accompanying pre- and postsurgical orthodontics during orthognathic surgery treatment is disappointing and results in less than ideal surgical change, but is largely derived from university clinic samples with patients treated by residents (clinical novices). This study examined similar treatment performed by experienced practitioners and compared it to the novices' results. Methods: A sample of seventy-two class II subjects treated by experienced practitioners (EP) with a mean of 26.7 years of experience was selected. Inclusion criteria were: consecutively treated surgical-orthodontic patients with mandibular advancement, rigid fixation and good quality lateral cephalograms. Pretreatment skeletal and dental variables were compared to a sample treated by novices. This was followed by presurgery and final analyses using an ANCOVA with pretreatment values as the covariate. An efficacy analyses for treatment phase and study comparisons was the final component. Results: The novices treated significantly more severe class II skeletal patients. For both studies, there were significant positive changes in the position of the mandible. The ANCOVA analysis showed that the EP managed the bodily position of the upper incisors more effectively. The efficacy analysis found no statistically significant differences between novices and EP. For both novices and EP, according to the ANB changes, non-ideal incisor decompensation led to less than ideal final mandibular position. Conclusions: The dental and skeletal mean changes and efficacy analysis for both novices and experienced practitioners showed that presurgical orthodontic treatment often does not fully decompensate the incisors, which then limits the surgical outcome.

Book A Detailed Description of Dental and Skeletal Changes Associated with Class II Surgical orthodontic Treatment

Download or read book A Detailed Description of Dental and Skeletal Changes Associated with Class II Surgical orthodontic Treatment written by Brittany Leigh Weaver Potts and published by . This book was released on 2007 with total page 132 pages. Available in PDF, EPUB and Kindle. Book excerpt: Abstract: Introduction: There is little information that details the dental changes that accompany pre-surgical and post-surgical orthodontics during orthognathic surgery treatment. Methods: Thirty-four Class II subjects who underwent surgical- orthodontic treatment with mandibular advancement and rigid fixation were selected and the lateral cephalograms digitized. Mean skeletal and dental values were calculated and plotted for pre-treatment, pre-surgical and final treatment time points. Treatment efficacy as a percentage of an ideal goal achieved also was calculated. Results: All data showed there were significant positive changes in the position of the mandible. The mean changes demonstrated: the upper incisors were over retracted pre-surgically and then returned to normal position post-surgically while the lower incisors were proclined and protruded pre-surgically and remained so at posttreatment. The dynamic changes showed that the incisors were not ideally decompensated in many of the cases. This was further substantiated by the efficacy data. Conclusions: The dynamic changes and efficacy data show that pre-surgical orthodontic treatment often does not fully decompensate the incisors, which then limits the surgical outcome.

Book The Surgery First Orthognathic Approach

Download or read book The Surgery First Orthognathic Approach written by Jong-Woo Choi and published by Springer Nature. This book was released on 2021-01-20 with total page 370 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book is a comprehensive guide to the surgery-first orthognathic approach for patients with malocclusion and skeletal disharmony, which has been successfully applied by the authors in their practices over the past 15 years. The approach breaks with the time-tested principles of traditional orthognathic surgery in that corrective bone surgery is performed first, without the removal of dental compensations, followed by orthodontic finishing. All aspects are covered with the aid of numerous illustrations, the aim being to provide surgeons with a systematic educational tool that will enable them to introduce the approach into their own practice. In addition, the book addresses one of the hot issues in orthodontics, occlusal plane-altering orthognathic surgery, in which surgical modification of the occlusal plane is employed to treat various types of dentofacial deformity and improve facial proportions. This promises to become a very powerful tool in modern orthognathic surgery.

Book Abstract Effect of Timing on Treatment Outcomes and Surgical Complications of Orthognathic Surgery in Class II Patients

Download or read book Abstract Effect of Timing on Treatment Outcomes and Surgical Complications of Orthognathic Surgery in Class II Patients written by Anna Morrow and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Class II malocclusions can be treated with a combination of orthodontics and orthognathic surgery. The severity of the malocclusion and growth potential play an important role in the treatment decision. Treatment involving orthognathic surgery is usually recommended after active growth is over to minimize relapse. On the other hand, improvement of a severe skeletal deformity and associated esthetic imbalance during adolescence can increase the patient’s quality of life and, in some cases, early treatment may decrease the financial burden due to changes in insurance coverage. Also, surgery and recovery from surgery may be easier and with less complications in younger individuals. The purpose of this study is to evaluate the effect of timing on treatment outcomes and the surgical complications of orthognathic surgery between younger and older Class II malocclusion patients. Methods Records of local orthodontists and oral surgeons were screened for inclusion in the study: Class II malocclusion treated with a combination of orthodontics and orthognathic surgery of the mandible (with or without maxillary surgery). Syndromic patients were excluded. Lateral cephalograms (initial, pre-surgical, post-surgical and final) as well as surgical notes were obtained. Patients were divided in 3 groups based on chronological age: 13-19, 20-25, 33-40. Dolphin imaging software was used and landmarks identified for cephalometric analysis. Cephalometric measurements were used to analyze the dental and skeletal changes associated with surgical and orthodontic treatment. Immediate and post-surgical complications were evaluated. A total of 112 patients were identified who fit the inclusion criteria. Only 35 patients had available cephalometric data (T1 pre-orthodontics; T2 pre-surgical; T3 postsurgical and T4 post treatment) which were used to compare anteroposterior (A-P) changes between the adolescent and adult patients. Less changes in cephalometric measurements were observed between T1-T2 and T3-T4 in the younger patients. Surgical data for 104 patients were included in the study, 8 patients were excluded due to missing data. For 1-jaw surgery, blood loss in the older group was greater than in the younger group. Limited TMJ opening (24-31 mm) at 6 weeks and more post- surgical complications after 1 jaw surgery were observed in the older group. More post-surgical complications after 2 jaw surgery (condylar resorption, infection, residual sensory loss and TMJ pain) were observed in the younger patient group. Younger patients who underwent orthognathic surgery to correct a Class II malocclusion had similar orthodontic treatment outcomes and similar immediate surgical complications compared to older patients. Possible continued mandibular growth contributed to improved skeletal relationships after the orthognathic surgery in the younger patients. Overall, there was no statistical or clinical difference in A-P changes of mandibular advancement surgery, nor differences in surgical complications between young and older patients.

Book Surgery First Orthodontic Management

Download or read book Surgery First Orthodontic Management written by Chai Kiat Chng and published by Springer. This book was released on 2019-06-29 with total page 174 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book is a comprehensive guide to the surgery-first orthognathic approach (SFOA), which overcomes the problem of the very prolonged treatment period associated with conventional orthognathic surgery by avoiding the presurgical phase. The shorter duration of the treatment is due to a phenomenon triggered by the surgery, termed the regional acceleratory phenomenon, whereby higher osteoclastic activity and metabolic changes at the dentoalveolar level accelerate postoperative orthodontic tooth movement. In this book, readers will find in-depth discussion of integration of the basic biological principles of SFOA with sound biomechanical doctrines. The authors’ own treatment protocol for surgical and orthodontic management is fully explained. Diagnosis and treatment plans are presented for various skeletal maxillofacial deformities, and key considerations in pre- and postsurgical patient care are highlighted. Detailed attention is given to complication management and to the impact of recent advances such as 3D image integration, virtual surgical planning, and rapid prototyping of surgical wafers. This richly illustrated book will be an ideal resource and quick reference guide for orthodontists at all levels of experience.
