Book excerpt: Actuellement, la permanence des soins fait l'objet de nombreuses concertations entre les Agences Régionales de Santé, les médecins libéraux, l'état et l'assurance maladie. Dans ce contexte, une association libérale de permanence des soins, SOS Médecins Vannes, a entrepris, depuis avril 2010, une restructuration de ses activités autour d'un cabinet de consultation et de soins urgents, adossé à un plateau technique diversifié et à une structure d'hospitalisation afin d'optimiser sa réponse aux demandes de soins non programmés, aussi bien aux heures de PDS qu'en dehors, tout en préservant son activité traditionnelle de visites à domicile. Le cabinet de consultations et de soins urgents accueille ainsi une patientèle plutôt jeune, présentant majoritairement une pathologie traumatique, consultant principalement aux heures d'ouverture des cabinets médicaux. Il s'agit pour les 2/3 d'entre eux, de patients auto-référés, attirés par la présence d'un plateau technique et privilégiant la médecine libérale comme trajectoire de recours aux soins non programmés mais susceptible d'avoir recours aux urgences hospitalières lorsque l'offre de soins non programmée est insuffisante. L'activité de visites à domicile est quant à elle, préférentiellement dédiée à la permanence des soins ; nous constatons cependant, une part croissante de cette activité en journée. Ce nouveau modèle d'organisation des soins non programmés ambulatoire présente un tripe avantage. Il permet une alternative aux structures d'urgence hospitalières, pour les usagers ne présentant pas une urgence vitale. La prise en charge médicale s'avère moins onéreuse pour le système de santé que les autres modes d'action existant. Enfin, ce système par sa souplesse et son adaptabilité, permet d'intégrer en une seule et même structure, l'intégralité de l'aide médicale permanente à la personne, soulageant ainsi les médecins généralistes libéraux des activités de permanence des soins, en zone urbaine.
