En diminuant leur mortalité par cancer, il a démontré son efficacité. Mais doit-on arrêter de dépister à 74 ans un cancer dont l'incidence augmente avec l'âge, dans le contexte actuel du vieillissement de la population? Une revue de la littérature sur le sujet permet de constater que les femmes âgées sont sous-représentées dans les études. Néanmoins on retrouve des arguments en faveur de la poursuite du dépistage. Nous avons posé le problème de la poursuite du dépistage pour les femmes de plus de 70 ans aux médecins généralistes du Réseau de Santé Paris Nord, à l'aide d'une enquête d'opinion et de pratique effectuée par Internet, "Dépimamie". 88,6% des 35 médecins investigateurs y sont favorables, dont les 2/3 jusqu'à 80 ans ou sans limite d'âge. Dans l'hypothèse d'une recommandation future, 54,8% des praticiens proposent un dépistage personnalisé individuel (DPI), basé sur l'âge ou l'espérance de vie. Les autres préfèrent la poursuite du dépistage de masse actuel (avec ou sans limite d'âge). Mais en confrontant l'opinion et la pratique, on constate que la moitié des praticiens poursuivent déjà le dépistage au-delà de 74 ans. 283 patientes de plus de 70 ans ont été includes pour l'enquête de pratique. Leur dépistage augmente significativement avec certains facteurs: le suivi par un spécialiste (gynécologue), un antécédent familial de cancer du sein, de mastopathie bénigne, de Traitement Hormonal substitutif. Le risque de ne pas être dépisté augmente significativement avec l'âge, la fréquence des consultations, le nombre de pathologies chroniques. Le médecin généraliste est au cœur de cette réflexion, puisqu'il peut proposer ce dépistage aux patientes de plus de 74 ans, en fonction du bénéfice attendu.
