Le traitement de la déglutition dysfonctionnelle est complexe et sa prise en charge est pluridisciplinaire. Le rôle de l'orthophoniste est central. Suite à un bilan, il met en place des séances de rééducation permettant d'acquérir et automatiser une déglutition fonctionnelle à condition qu'il n'y ait pas d'obstacle anatomique ou psychologiques. Une fiche diagnostic est élaborée dans cette thèse afin de sensibiliser les omnipraticiens au diagnostic et l'orientation des jeunes enfants vers un orthophoniste.

De plus, en présence d'une béance au niveau du secteur incisif, le guidage antérieur est absent. Nous pouvons alors nous demander quels sont les grands principes occlusaux à respecter lors d'une réhabilitation fonctionnelle et esthétique. Aussi, s'il est nécessaire d'avoir recours à d'autres spécialistes tels que les orthodontistes ou les orthophonistes . Afin de répondre à ces questions, nous allons tout d'abord faire quelques rappels d'anatomie, de physiologie et d'occlusion. Puis, nous verrons les étapes indispensables dans le plan de traitement de ces cas complexes. Enfin, nous illustrerons nos propos par des cas cliniques.

Book excerpt: Dans la pratique orthophonique, il arrive parfois de rencontrer des patients présentant à la fois une déglutition atypique et une dysphonie dysfonctionnelle. Existe-t-il des liens entre ces deux troubles ? De quelles nature seraient-ils ? Enfin la qualité vocale en cas de dysphonie fonctionnelle pourrait-elle être améliorée par la rééducation de la déglutition atypique? Pour tenter de répondre à ces questions nous nous sommes appuyées sur les données théoriques de la littérature, et avons mené parallèlement plusieurs études : un questionnaire rétrospectif a été distribué aux orthophonistes des régions Midi-Pyrénées et Limousin, et une enquête rétrospective puis prospective a été proposée à un échantilon de praticiens ciblés. Enfin, deux patients ont été suivis. ces recherches ont mis en avant le probable rôle de la fonction linguale dans la position laryngée, la posture et la ventilation, et défendent ainsi l'idée d'un caractère favorisant de la déglution atypique dans la genèse des troubles vocaux. Les observations faites par les orthophonistes semblent pour la plupart étayer ces hypothèses. Toutefois, l'association de la déglutition atypique et de la voix ne se retrouve que de façon occasionnelle. Quant à l'éventuel impact de la rééducation neuro-fonctionnelle sur la qualité vocale, si nos études de cas semblent aller dans ce sens, la pratique des orthophonistes, qui répond à la demande du patient et se base sur les particularités de ce dernier, ne permet toujours de le vérifier. Ainsi, l'absence de fondement théorique, l'hétérogénéité des expériences et des opinions de chacun, des rééducations, mais aussi le faible taux de réponses à nos différentes études rendent impossible toute conclusion définitive....
