It is now clear that the pathophysiology of many infectious, autoimmune, allergic, and malignant diseases can be largely explained by which cytokines are induced and subsequently regulate the cellular responses. In clinical medicine, cytokines are involved in a wide spectrum of diseases. This book describes in three parts the properties and roles of 15 key cytokines under physiological and pathological conditions. Part I presents nine cytokines associated with inflammatory disorders, pro-inflammatory cytokines, and the recently identified new helper T (Th) subset: Th17 cells. Part II gives details of three cytokines associated with allergic disorders, including Th2 responses and recently identified types of innate cells. Part III describes three cytokines that are associated with immunological tolerance and anti-inflammation, including regulatory T (Treg) cells, IL-10-producing Treg (Tr1) cells, and inducible IL-35-producing Treg (iTr35) cells. Cytokines are considered to be important as therapeutic targets for specific agonists or antagonists in numerous immune and inflammatory diseases. The ultimate goal of this book is to facilitate the development of therapeutic treatments for such diseases which has been limited by an insufficient understanding of the biology of cytokines and the complicated network that they create.

It is in this area that the current view of the immune system has arisen and developed. There is indeed little doubt that immune reactions are primarily determined by messages which are genera ted within the immune system and passed among different types of immunologie cells. This cell communication not only determines the type, intensity and duration of the response after perturbation of the immune system by exogenous antigens, but it is also essential for preventing autoimmune reactions and their clinical conse quences. In order to assure aperfect balance within the enormous com plexity of the immune system, it is not surprising that multiple self-regulatory mechanisms are organized at different levels, such as antibody feedback, idiotypic-anti-idiotypic responses, suppres sor and helper T cells, lymphokine signals and genetic require ments. A nu mb er of observations in recent years have, however, demonstrated that consistent contributions to the immunological homeostasis are given also by signals generated outside of the immune system, namely,in the central and autonomous nervous system as weIl as in the endocrine apparatus. Furthermore, the interactions between the immune system and the other body homestatic mechanisms seem to be bidirectional: if immunological cells may be targets of neuroendocrinological factors, immunological products seem in turn to contribute to the neuro endocrine homeostasis.

They play a key role in the regulation of the immune response and the coordination of the host response to infection. Based on these biological properties, nobody would have predicted that one decade later cytokines would burst upon neurosciences and permeate into several avenues of current research. In neurology, the connection between cytokines and inflammation, and the demonstration of a pivotal role of some of these molecules in cell death by apoptosis, prompted the investigation of their involvement in several neurological diseases involving an inflammatory component, including multiple sclerosis, brain trauma, stroke, and Alzheimer's disease. This movement started in the late eighties, and the corresponding field of research, known as neuroimmunology, is presently booming. In psychiatry, however, the relationship between cytokines and mental disorders was much less evident and took longer to materialize. The first indication that cytokines might be involved in psychopathology came from cancerology and internal medicine.

When one considers the updated signs of inflammation, they would be vasodilation, cell migration, and, in the case of chronic inflam- tion, cell proliferation, often with an underlying autoimmune basis. Gen- ally, inflammation may be divided into acute, chronic, and autoimmune, - though the editors believe that most, if not all, chronic states are often the result of an autoimmune response to an endogenous antigen. Thus, a proper understanding of the inflammatory basis may provide clues to new therap- tic targets not only in classical inflammatory diseases, but atherosclerosis, cancer, and ischemic heart disease as well. The lack of advances in classical inflammatory diseases, such as rh- matoid arthritis, may in part arise from a failure to classify the disease into different forms. That different forms exist is exemplified in patients with d- fering responses to existing antiinflammatory drugs, ranging from nonresponders to very positive responders for a particular nonsteroidal an- inflammatory drug (NSAID). Though researchers have progressively unr- eled the mechanisms, the story is far from complete. It should also be noted that the inflammatory response is part of the innate immune response, or to use John Hunter’s words in 1795, “inflammation is a salutary response.” That may be applied in particular to the defensive response to invading micro- ganisms.
