Book excerpt: Biomedicalization is seen as the natural outgrowth of continued scientific progress--a movement towards improving the quality and quantity of life through scientific inquiries using biomedical perspectives and methods. This approach carries with it the assumption that with "proper" risk assessment, detection, and treatment, our lives can be lengthened, improved, and indeed more fulfilling. Yet critics question biomedicalization's ability to deliver. There is concern about how biomedicalization can change our traditional concepts of health as we discover more conditions for which we are at risk, and health maintenance is seen as the responsibility of the individual. The purpose of the book is to describe, assess, and critique biomedicalization and its influence as a larger social trend on the health field and specifically in the area of alcohol research, policy, and programs. Chapter 1 gives a broad overview of biomedicalization. Chapter 2 lays the groundwork for a historical understanding of how medicalization and biomeidcalization have developed and are expressed in diverse fields such as aging, psychiatry/mental health, and women's health. Chapter 3 focuses in-depth on alcoholism and assesses the development and assumptions underlying the two movements that have greatly influenced the substance abuse field: the medicalization of deviance and the growth of the disease model of alcoholism. Chapter 4 discusses the origins and development of the National Institute on Alcohol Abuse and Alcoholism (NIAAA) from its inception in 1970. Chapter 5 illustrates the growing biomedicalization that has occurred in the alcohol field prior to NIAAA's movement to the National Institute of Health (NIH). Chapter 6 assesses how Sweden has handled alcohol problems and currently funds alcohol research. Chapter 7 concludes with a rationale for an expanded discourse between social scientists and biomedical researchers working on social problems, particularly alcohol issues. This volume will stimulate discussion of the processes by which social problems, and specifically alcohol issues, are framed, managed, and studied. It will hold particular interest for researchers and students in the areas of alcohol studies, social science, and social welfare. Alcohol is the most widely used drug by adolescents. For some, this may be one or two isolated occasions of youthful experimentation; for others, the use becomes excessive, placing them in danger of immediate adverse consequences such as accidental injury and alcohol poisoning, or encouraging other high-risk behavior patterns including unprotected sex. Moreover, a pattern of heavy drinking established in adolescence and young adulthood may continue into an adult pattern of alcohol abuse. Concerned communities and institutions across the nation are tackling the problem of alcohol use and abuse by young people. Research-based knowledge is urgently needed to inform these efforts and to ensure that limited prevention resources are used as effectively as possible. The origins of youthful alcohol use and abuse are found within the complex interplay of individual characteristics, family and peer influences, the larger societal context for alcohol use, environmental conditions, and maturational processes that accompany adolescence. Book excerpt: From the President of the Research Society on Alcoholism In recent years the alcohol research field has matured and is attracting a substantial number of eager and technically sophisticated researchers. There is a feeling of excitement and premonitions of breakthroughs as more and more of alcohol's actions are being detailed. I, however, have at times been sobered by the perception that the lure of parsimonious explanations and the appeal of studying easily demonstrable effects obscure the critical issues re garding alcohol abuse and alcoholism. Central questions regarding (1) the reinforcing properties of ethanol and other factors responsible for inappro priate consumption of alcohol, (2) the heterogenous characteristics (both bi ologic and sociologic) of the alcoholic population, and (3) the differential predisposition of individuals to alcohol-induced medical problems needed to be vigorously pursued. Researchers who used animals as models for the human condition needed to become more intimately aware of the variety of factors that are of importance in the development of alcoholism in an indi vidual. On the other hand, researchers studying humans needed to attempt to more clearly define and categorize diagnostic criteria and characteristics of various alcoholic populations. Such categorization and continued character ization of alcoholism allows not only for a framework of concepts within which proper animal models for studies of biologic mechanisms can be de veloped but also allows for consideration of the most apt treatment approaches.
