Its status as an opportunistic pathogen, especially in AIDS patients, and the lack of effective anti-cryptosporidial drugs have served to emphasise the public health importance of this organism. This has to some extent overshadowed the fact that Cryptosporidium is also an important pathogen of domestic animals and wildlife. In recent years, the application of molecular biology and culture techniques have had an enormous impact on our understanding of the aetiological agents of cryptosporidial infections and our ability to study the causative agents in the laboratory. Today, approximately one-third of the world's population lives in areas with scarce water resources. Nearly one billion people currently lack access to an adequate water supply, and more than twice as many lack access to basic sanitation services. It is projected that by 2025 water scarcity will affect nearly two-thirds of all people on the planet. Recognizing that water availability, water quality, and sanitation are fundamental issues underlying infectious disease emergence and spread, the Institute of Medicine held a two-day public workshop, summarized in this volume. Through invited presentations and discussions, participants explored global and local connections between water, sanitation, and health; the spectrum of water-related disease transmission processes as they inform intervention design; lessons learned from water-related disease outbreaks; vulnerabilities in water and sanitation infrastructure in both industrialized and developing countries; and opportunities to improve water and sanitation infrastructure so as to reduce the risk of water-related infectious disease.

Book excerpt: The enteric protozoans Cryptosporidium spp. and Giardia duodenalis are the most common diarrhoea-causing protozoan parasites worldwide. Cryptosporidium spp. is a leading cause of diarrhoea morbidity and mortality in children younger than 5 years of age in poor-resource settings in sub-Saharan Africa and South Asia. Although infection by G. duodenalis is rarely a fatal condition, giardiasis is commonly associated with childhood growth faltering and cognitive impairment. Because of their significant socioeconomic impact, particularly in low-income countries, giardiasis and cryptosporidiosis joined the "Neglected Disease Initiative" launched by the World Health Organization in 2004. Both Cryptosporidium spp. and G. duodenalis are ubiquitous in the environment and can infect a wide range of hosts with different specificities, meaning that humans may acquire the infection via waterborne, foodborne, or zoonotic transmission. Recent advances in detection and molecular epidemiology have indicated that certain species/genotypes of Cryptosporidium and G. duodenalis have an expanded range of suitable hosts, suggesting that their transmission pathways are more intricate than previously thought, challenging our current notion of host specificities. Therefore, there is a clear need for more studies aiming at investigatingthat aim to investigate the frequency and molecular diversity of these parasites in humans, production and companion animals, and wildlife species. This information would be extremely useful to elucidate the transmission dynamics of cryptosporidiosis and giardiasis, and to investigate the exact contribution of zoonotic events to human infections.

Future adaptation measures therefore need careful assessment and implementation for us to be able to minimise the potential risks from climate change and, at the same time, maximise the potential health benefits of a cleaner, greener world. This Special Issue comprises original research articles and detailed reviews on the likely impacts of climate change on health in a range of geographical settings, and the potential for adaptation measures to reduce some of these risks. Countries that are more technologically advanced have experienced a significant reduction in water borne morbidity within the last 100 years: This reduction has been achieved through the application of effective technologies for the treatment, disinfec tion, and distribution of potable water. However, morbidity resulting from the ingestion of contaminated water persists globally, and the available ep idemiological evidence (Waterborne Diseases in the United States, G. F. Craun, ed. , 1986, CRC Press) demonstrates a dramatic increase in the number of waterborne outbreaks and individual cases within the United States since the mid-1960s. In addition, it should also be noted that the incidence of water borne outbreaks of unknown etiology and those caused by "new" pathogens, such as Campylobaeter sp. , is also increasing in the United States. Although it might be debated whether these increases are real or an artifact resulting from more efficient reporting, it is clear that waterborne morbidity cannot be ignored in the industrialized world. More significantly, it represents one of the most important causes of illness within developing countries. Approxi mately one-half the world's population experiences diseases that are the direct consequence of drinking polluted water. Such illnesses are the primary cause of infant mortality in many Third World countries.
