Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Although animal and cell culture breast cancer models have proven to be of great use, there are now increasing opportunities to test the concepts developed in these models in actual clinical samples and cases. It is gratifying to see how well these concepts "translate" into the clinical setting. A very active area of research that is linking the laboratory to the clinic is the dissection of the biology and elucidation of the significance of proliferate breast disease and the identification of true, "high risk" or "preneoplastic" legions within the previously ill-defined spectrum of fibrocystic or benign breast disease. One anticipates that discoveries made here will also lead to earlier detection, intervention and prevention of life-threatening cancer.Even, however, as we look with optimism to the eventual eradication of breast cancer, we are once again forced to face the reality that we have not yet achieved our goal. Thus, we are saddened by the much too premature death of Dr. Helene Smith from breast cancer.
