Book excerpt: Introduction : Les fractures de l'angle mandibulaire sont très fréquentes parmi les fractures mandibulaires et sont associées à un taux important de complications. L'objectif de cette étude est d'évaluer l'influence des dents de sagesse incluses avec les complications des fractures de l'angle mandibulaire. Matériel et méthodes : Tous les patients présentant une fracture de l'angle mandibulaire suivis pendant au moins 2 mois ont été inclus rétrospectivement. Les complications suivantes ont été recueillies : malocclusion post-traumatique, limitation de l'ouverture buccale, hypoesthésie du nerf alvéolaire inférieur, infection, retard de consolidation osseuse, mobilisation ou fracture du matériel d'ostéosynthèse. Résultats : 68 patients ont été inclus : 36 avec dent de sagesse incluses et 32 avec dent de sagesse absente ou sur arcade. 40 complications ont été rapportées chez 27 patients à 2 mois. Aucune différence statistiquement significative n'a pu être mise en évidence entre les deux groupes concernant la malocclusion, la limitation de l'ouverture buccale et l'hypoesthésie du nerf alvéolaire inférieur. Néanmoins, un taux plus faible d'hypoesthésie persistante du nerf alvéolaire inférieur (p=0,0557) a été observé chez les patients ayant une dent de sagesse incluse (19,4 %) par rapport aux patients sans dent de sagesse incluse (40,6 %). Aucun retard de consolidation osseuse ou problème de matériel n'a été enregistré dans le groupe des dents de sagesse incluses, alors que 5 retards de consolidation osseuse et 4 problèmes de matériel ont été enregistrés dans le groupe des dents de sagesse absente/sur arcade. Enfin, le taux de patients présentant au moins une des 6 complications est significativement plus élevé dans le groupe des dents de sagesse absente/sur arcade (53,1%) que dans le groupe des dents de sagesse incluses (27,8%), p=0,033. Discussion : La présence d'une dent de sagesse incluse diminue le risque de complication globale des fractures de l'angle mandibulaire. Les résultats de cette étude confortent l'idée que la présence d'une dent de sagesse incluse participe à la réduction et la stabilité de la fracture.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.

Bion’s theories of alpha function, transformations, dreaming and his clinical emphasis on the present moment are foundational to this book. Brown’s writing is clear and aims to describe the various theoretical ideas as plainly as possible. Detailed clinical material is given in most chapters to illustrate the theoretical perspectives. Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. Transformational Processes in Clinical Psychoanalysis will be of great interest to all psychoanalysts and psychoanalytic psychotherapists.

Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
