Book excerpt: The value of ultrasound contrast agents (USCA) in everyday clinical practice depends on the pharmacokinetics, the signal processing, and the contrast-specific imaging modalities. Second-generation USCA, are blood pool agents that do not leak into the organ tissue to be examined but remain in the intravascular compartment increasing the Doppler signal amplitude during their dynamic vascular phase. Taking advantage of the stability of their microbubbles, they can withstand the acoustic pressure of insonation much better than first-generation contrast media, which results in an increased half-life of the agent and, consequently, in a prolonged diagnostic window. Concomitant with the improvement of contrast agents, different contrast-specific imaging modalities have been developed which, used in combination with USCA and a low mechanical index, allow continuous real-time grey-scale imaging. These recent technical improvements have opened new possibilities in the use of USCA in a variety of indications. UTIs are the most prevalent infectious illness, and account for hundreds of thousands of emergency and hospital admissions yearly. Furthermore, UTIs are the most common (almost 40%) type of hospital-acquired infections, with bladder catheterisation being the key risk factor. According to the European Association of Urology guidelines, complicated urinary tract infections (C-UTIs) are those associated with structural or functional genitourinary abnormalities or with conditions that impair the host’s defence mechanisms, leading to an increased risk of acquiring infection or therapy failure. Besides offering current perspectives from urologists, nephrologists, and specialists in infectious diseases, the book presents the techniques and highlights the role of ultrasound and contrast-enhanced ultrasound, nuclear medicine, multidetector computed tomography (CT) and magnetic resonance imaging (MRI) in providing comprehensive investigations of upper and lower tract UTIs, and of systemic infections from unknown sources. Book excerpt: This research examines the ability of volumetric contrast-enhanced ultrasound (VCEUS) imaging to quantify kidney blood flow, with the potential to monitor changes in renal perfusion following acute kidney injury (AKI). Ultrasound (US) imaging of microbubble contrast agents allows for increased imaging isolation of the vasculature, creating a non-toxic approach to visualizing blood flow. VCEUS utilizes a series of planar ultrasound acquisitions that can be collected in real time to survey the vascularity of a volumetric region-of-interest. Through post-processing methods it is possible to quantify perfusion parameters such as peak intensity (IPK), time-to-peak intensity (TPK), wash-in rate (WIR), and area-under-curve (AUC). We hypothesize that VCEUS will be a valuable addition to current techniques to track changes in kidney perfusion for both healthy and injured kidney. AKI involves the development of ischemia and decreased glomerular filtration rate in kidneys following some inducing injury, and is in need of a non-toxic, noninvasive imaging approach to monitor patient response to treatment. Current methods of detecting and determining the extent of AKI involve the detection of certain biomarkers in the blood serum and urine, such as creatinine. However, these methods lack the ideal specificity and sensitivity for AKI detection and monitoring. For this thesis project, custom US image processing software was developed with the purpose of deriving the described perfusion parameters, which will give us the necessary information to assess the kidney activity. Studies were initially performed investigating perfusion parameters in a vascular flow phantom, confirming the ability of the software to quantify VCEUS. Investigation of healthy rat kidneys further optimized data acquisition and image processing techniques. Investigation of rat kidneys that underwent ischemia-reperfusion injury allowed collection of pilot information to validate the ability of VCEUS imaging to monitor reperfusion response after AKI. Comparison with controls and current methods, such as creatinine, allow for confirmation of imaging analysis. These preclinical explorations give a complete study for VCEUS imaging of kidney blood flow, which gives the basis for further investigation into the field. The proposed methods in conjunction with currently available techniques have the potential for providing a more accurate representation of the disease state.

A new imagistic method is contrast-enhanced ultrasound (CEUS) that accurately depicts the circulatory pattern of tumors. In kidney pathology, any mass or even capsular deformation depicted by B mode ultrasound represents an indication for CEUS.¬†The kidney is completely and uniformly vascularized. In cystic tumors, there is a lack of contrast loading. In ,Äúimpure,Äù cystic masses (Bosnian 3 and 4), CEUS reveals vascularized septa and walls. In malignant tumors, accelerated and heterogeneous loading of contrast agent is observed in arterial phase followed by early, inhomogeneous washout of the contrast agent compared to normal renal parenchyma. In the abscesses, the appearance is the early loading of the walls associated with moderate hyperemia of the normal circulatory bed. This chapter details the CEUS in kidney tumor pathology, emphasizing the accurate information for the circulatory pattern of renal masses. It requires correlations with clinical data and information provided by other imaging explorations to make a final diagnosis.
