Book excerpt: La República Dominicana se destaca por ser una economía en rápido crecimiento que no le ha sido posible generar una reducción proporcional en la pobreza. Tres razones han sido planteadas anteriormente para explicar esta paradoja: (i) un mercado laboral que no traduce el aumento de la productividad en aumentos salariales; (ii) una economía interna con débiles encadenamientos intersectoriales; (iii) y un sector público que ni gasta lo suficiente, ni particularmente bien, para reducir la pobreza. Además, el país permanece mayormente expuesto a desastres naturales y choques exógenos que, si no se mitigan adecuadamente, pueden afectar la sostenibilidad del crecimiento a largo y mediano plazos. Esta obra conjuga varios análisis empíricos que exploran tres hipótesis complementarias que podrían ayudar a entender porqué la República Dominicana, aún hoy, sigue experimentando altas tasas de crecimiento económico con reducción limitada de la pobreza. La primera hipótesis trata de probar si el patrón de rápido crecimiento económico con la pobreza persistente que se observa en la RD lo impulsa en parte una metodología de la pobreza que no toma en cuenta la variación de precios que claramente afecta los patrones de consumo de los hogares de ingresos bajo y aquéllos de mejor posición económica. Si se sostiene esa hipótesis, la RD puede enfrentar una situación en la cual se subestiman los ingresos del hogar con respecto a los hogares en el extremo inferior de la distribución. La segunda hipótesis intenta validar si el patrón de especialización en la RD podría ser tal que no favorece la mano de obra no calificada. Si se sostiene esa hipótesis, entonces el rendimiento del capital probablemente es mucho mayor que el rendimiento de la mano de obra lo que podría ser un indicio de que la RD ha tenido una ventaja comparativa en productos que son de uso intensivo de capital. La tercera hipótesis investiga si la pobreza y la desigualdad salarial en la RD se ven afectadas no solo por la inmigración, sino también por la emigración. El aporte del volumen, por tanto, descansa precisamente en ofrecer una exploración más cuidadosa de los asuntos específicos que giran en torno a explicaciones comunes para las deficiencias de la RD en reducir la pobreza de manera más rápida.

Book excerpt: The Cipriano Castro administration, which ruled Venezuela from 1899 to 1908, was characterized by a series of internal and external political crises which seemed capable of toppling it at any moment. In 1901, a number of foreign countries provided financial backing to Castro's former allies, united under the leadership of Manuel Antonio Matos, who almost brought the government down. In the midst of this civil war, Germany, the United Kingdom and later Italy instituted what came to be known as the peaceful blockade of Venezuela to force the government to honor its foreign debts. The claims and counter-claims stemming from the conflict would eventually force the three foreign countries to sever diplomatic relations in the ensuing years. Far from its portrayal as a nationalist champion, the Castro administration was, in McBeth's findings, more focused on the accumulation of personal wealth than on defense of Venezuelan interests. Castro would pay dearly for his misdeeds, losing power in a 1908 coup to Juan Vicente Gómez and remaining in exile until his death in 1924. The conflict would prove to be a watershed in relations with Latin America, as the United States modified its own foreign policy in response and the European powers became more aware of the limit of their political influence in the region.

Además, considero justo decir, que la mayoría de los estudiantes de medicina y de otras ciencias de la salud, consideran a la Medicina del Trabajo y a la Epidemiología, como unidades de aprendizaje aburridas e irrelevantes, las que se estudian únicamente porque se les obliga a ello. Otro punto de vista común, en lo referente a la Epidemiología, es que se le considera altamente matemática y demasiado compleja para entenderla. Con este pensamiento, he intentado escribir un libro de texto conciso, para médicos del trabajo, estudiantes de medicina y otros profesionales de la salud, que pueda explicar los conceptos básicos de la epidemiología de manera clara y sencilla. He tratado de suprimir la brecha existente en la comunicación entre el Médico del Trabajo como clínico y el epidemiólogo, describiendo algunos ejemplos clínicos a través del libro, explicando al Médico del Trabajo, por que es necesario el énfasis epidemiológico sobre el estudio de grupos de trabajadores, mas que de individuos.
