The role of the endothelium, especially of the coronary intravascular species is a relatively new and fascinating development in the diagnosis and treatment of cardiac disease. Three major aspects of endothelial function receive attention in this volume: its regulatory role in myocardial function, evidence for which is gradually forthcoming, with possible great implications for disease states such as volume overload and failure its role in the calcium metabolism of vascular smooth muscle cells and the effects of calcium antagonists and the endothelial dysfunction generated by coronary angioplasty and its possible restoration by cholesterol reduction. The topic of imaging atherosclerotic lesions in the coronary arteries is discussed with the emphasis on advanced methods: single photon emission computed tomography (SPECT), positron emission tomography (PET), and magnetic resonance imaging techniques. A wholly new development is the characterization of the atherosclerotic lesion by Raman spectroscopy using a laser beam to illuminate the vascular wall. Various aspects of inflammation related to the pathogenesis of acute coronary syndromes, in particular unstable angina, are illuminated, as well as the role of blood-borne substances as risk factors for atherosclerosis. Transgenic mice carrying the gene for human APOE*3 Leiden are being used successfully to study the environmental and genetic factors that influence the remnant lipoprotein metabolism. Possibilities for gene therapy to treat atherosclerosis in the clinic are being investigated in animal models. Finally, as the book is based on 50 years of Cardiology in Leiden, the history of its development up to the present day is the topic of a separate chapter. This book is intended for cardiologists, internal medicine specialists, nuclear medicine specialists, thoracic surgeons, cardiovascular radiologists and basic scientists in the field of the cardiovascular system.

Book excerpt: Many noninvasive examination methods of the heart have not held out against the invasive methods, which modern cardiac therapy, surgically or with catheterization, requires. They have disappeared completely or are only used by isolated groups of researchers. However, there is an obvious tendency to apply the invasive procedures as the last diagnostic possibility. In the attempt to select clinically relevant methods, the expert authors of this book demonstrate that echocardiography, expanded with contrast and Doppler, has been developed into one of the most important noninvasive methods. The results with tissue characterization show that the possibilities of this method have not yet been fully explored. Nuclear procedures are widely used, although they should only be applied in direct connection with clinical cardiology. The new lead methods of the ecg, such as ecg-mapping, show that standard electrocardiography of electrical functions is not yet fully exploited. The rapidly developing method of computer tomography is also being applied to cardiology. Since nuclear magnetic resonance requires extensive equipment con struction, its future is as yet unsure. Of course, a book like this does not intend to treat the subject of noninvasive cardiology in extensive detail. Established methods like standard electrocardio graphy, phonocardiography and sphygmography are not discussed. The aim of this book is rather to demonstrate the trend of present developments in the field. Book excerpt: To our knowledge, this is the first book dealing exclusively with myocardial contrast two-dimensional echocardiography (MC-2DE), a new and exciting diagnostic methodology for assessment of myocardial perfusion, which has seen rapid development and has now entered the clinical stage. The experi mental research and human applications have been described in technical papers published in a variety of journals, but our objective is to provide the reader with a comprehensive and concentrated overview of the field and of the current state of the art. To facilitate appreciation of the significant ad vances made and issues yet to be resolved, we are pleased to have several well known specialists contribute their own assessment of specific aspects of MC- 20E and illustrate the method's principles as well as applications. We were faced with inevitable overlaps and some repetitions in the discussion of quan titative potentials or limilations of the methodology. Rather than strictly 'streamlining' the text, we decided to accept some redundancy in the interest of presenting a diversity of points of views, reflecting the current evolutionary state of MC-20E. Following a brief reference to the established clinical contrast echocardiog raphy, recent developments and validations of the specialized MC-2DE tech nique are reviewed, bearing in mind that the field is in a flux and some of the ongoing activities have not as yet been formally reported. Mechanisms of the echo contrast and several new agents are decribed next, and an additional chapter illustrates current thoughts-on optimizing the echo contrast medium.
