According to studies, Ventilator Associated Pneumonia (VAP) is one of the leading causes of death in ICUs and it occurs in 9% to 28% of patients treated with mechanical ventilation, and the percentage of mortality rates of this problem are from 24% to 50%' (Berry and others, 2007). There are many studies done to find out about the onset of VAP in mechanically ventilated patients. According to one of the studies, 'Ventilator-associated pneumonia (VAP) occurs within 24 hours of intubation and mechanical ventilation' (Stonecypher, 2010). Due to the assistive-breathing devices and the medications used to keep the mechanically ventilated patients sedated, normal defense mechanisms change, and they become more vulnerable to respiratory infection such as VAP (Pear, 2007). By implementing evidence based oral care into clinical practice, staff members can reduce the cases of VAP and also can improve the quality of care in ICUs. The purpose of this proposal is to implement a comprehensive oral care protocol in critically ill patients such as mechanically ventilated patients in ICU to reduce or prevent the incidences of VAP. Through this proposal the author will also talk about the consequences of inadequate oral care in ventilated patients. The nursing theory, which supports the writer's proposal, is Florence Nightingales environmental theory. A healthy environment is essential for the faster and better healing and recovery according to the Nightingale theory. Evidence based practices and 15 articles containing information about the importance of comprehensive oral care will also supports the author's proposal of reducing or preventing the cases of VAP in ICUs.

Book excerpt: Ventilator-associated pneumonia is a serious hospital-acquired infection with reported incidence rate of 12.2% and mortality rate of 29.3%(Chao and others, 2009).Strict implementation of oral care protocol can prevent VAP to greater extent. With the help of 15 evidence-based articles on prevention of VAP, the writer implemented a comprehensive oral care protocol in critical care unit of white memorial medical center where 80% of the patients were on mechanical ventilator. Regardless of whether the experiences and skills of the hospital's staff are known or not, oral care practices within the hospital were practiced. As an emphasis on the relevance and significance of this study, the implementation of oral care to counter VAP was aimed to fight back stress to both the patient and his/her family, reduction of the duration a patient takes under ventilation, and elimination of cumulative costs associated with the prolonged admissions. The author was able to complete the project on VAP with the support from the resource personnel, such as the infection control, research nurses, and pharmacists, physicians, and staff members of the MICU. Various tools like power point presentations were used to educate the nursing staff about the significance of oral care. To determine the efficiency of the proposed solution implemented, the members used evaluation tools in the form of patient chart auditing and feedback from the nurses. The findings of the study are assessed by the nursing research committee and quality control department and necessary changes being made in the existing policy. A new protocol for the oral hygiene was also implemented in ICU.

Book excerpt: Ventilator-associated pneumonia (VAP) is a common complication and linked to high mortality and morbidity among mechanically ventilated patients in the United States (Zurmehly, 2013). It is the second most common hospital-acquired infection that carries a 40% - 80% mortality rate (Fields, 2008). The Southwest Medical Center (SMC) is a 406-bed hospital located in Oklahoma City, Oklahoma. The VAP rate for the last six months during the year 2008 was reported as 4.3, 4.7, 0.0, 4.1, 3.9, and 0.0 respectively (Oklahoma Hospital Report, 2009). Each episode of VAP incidence can cost an average $40,000 more in healthcare expenses (Zurmehly, 2013). The proposed intervention for oral hygiene in intubated ventilated adult patients is tooth brushing three times a day and 0.12% chlorhexidine application twice a day. This mechanical and pharmacological intervention is to promote oral hygiene and thereby reduce the colonization of oropharynx and dental plaque by bacteria (Feider, Mitchell, and Bridges, 2010). The variables measured here are ventilator-associated pneumonia (VAP) rates, ventilator days, length of stay of patients in intensive care unit (ICU), length of stay of patients in hospital, frequency of tooth brushing, frequency of chlorhexidine application, oral care evaluation score, acute physiology and chronic health evaluation score, mortality rates of ventilated patients, and patient demographics. The infection-control department will keep track of the data collected during intervention and pre-intervention period. Unit directors and infection-control department will work together to evaluate the implementation process. Different strategies were used to disseminate the evidence within the organization and to the outside healthcare community.

In a critically ill patient, the development of VAP significantly increases their risk of mortality, length of hospital stay, and decelerates the speed of recovery. Research has shown a substantial correlation between poor oral hygiene in mechanically ventilated (MV) patients and the development of VAP. The implementation of an oral hygiene protocol and/or order-set increases nurse adherence to performing these interventions, improves quality of oral care practices, and ultimately reduces the incidence of VAP. The successful implementation of an oral care protocol requires thorough preparation. Components of implantation will include gathering of evidence, literature review, stakeholder presentation, involvement and education of intended staff, actual implementation, evaluation plan, and dissemination of evidence. The effective implementation of an oral care protocol in intensive care units for MV patients will decrease the incidence of VAP, reduce hospital stays, and decrease costs.

Research shows that providing oral care on a frequent basis with the use of an antiseptic solution such as Chlorhexidine, will reduce the rate of ventilator associated pneumonia among vented patients as well as decrease mortality rates and decrease the length of a patient's hospital stay. Current policies for oral care already exist but this proposal will include an implementation plan for making sure staff is properly educated and are incorporating the appropriate oral care policies and techniques for mechanically ventilated patients. Ventilator associated pneumonia is impacting healthcare profoundly and will begin to affect hospital reimbursement rates. Prevention of this infection needs to be a team effort involving everyone who comes into contact with vented patients, however nurses will be on the from lines of prevention. Ventilator associated pneumonia in an infection that can be prevented.

Adherence to ventilator bundle use has proven to decrease the incidence of VAP. However, the specifics for a ventilator bundle are non-existent with only vague recommendations from the Center for Disease Control and Prevention. The oral care component of a ventilator bundle was researched to attempt to determine best oral care practices for reduction of VAP. Research evidence supports oral care twice a day with 1% chlorhexidine for significant reduction in VAP, along with a reduction of ventilator days, hospital stay, and hospital costs. This proposal includes research based evidence for implementation of this oral care practice into the intensive care unit for mechanically ventilated individuals. It can be very challenging taking care of these type of patients due to the multifaceted health issues, particularly if their respiratory system is already compromised. Oral hygiene is considered to be an important intervention, in combination with other strategies, for the prevention of ventilator-associated pneumonia. Data suggest that the simple reduction of risk through better oral hygiene can lead to fewer incidence of Ventilator-Association Pneumonia. In addition to brushing, providing oral moisturizer to oral mucosa and lips every 2 to 4 hours is often a component of the oral care protocol. Implementation of an oral care protocol, ongoing nurse education, and evaluation were important in reducing the incidence of VAP. The use of teamwork tools and strategies to ensure this consistency can have a tremendous impact on closing the gap, improving patient outcomes, shorter length of stay in the intensive Care Unit and reduction of health care cost.

Book excerpt: Several evidence-based research studies have conclusively confirmed in recent years that there has been a considerable increase in the incidence of ventilator-associated pneumonia (VAP) cases of mechanically ventilated patients in critical care units (CCUs). The statistical evidence suggests that VAP occurrence rate ranges from 30% to 50% among mechanically ventilated patients and this is one of the primary causes for increased morbidity and mortality in seriously ill patients. Usually VAP builds up after 48 hours of intubation in a mechanically vented patient. In the wake of the development of VAP patients are likely to have extended length of stay on mechanical ventilators and in the intensive care units (ICUs). The purpose of this study is to determine the effectiveness of comprehensive and multifaceted bundle care interventions with full staff compliance with the necessary protocols for preventing VAP in the ICU at Memorial Regional Hospital (MRH), Florida. The proposed intervention plan anticipates that with full observance of all the components of VAP bundle care, which comprises of: elevating the head of the bed (HOB) to 30--45°, sedation cessation for assessing the readiness to extubate, peptic ulcer disease and deep vein thrombosis prophylaxis if not contraindicated, use of chlorhexidine for oral care, humidification of inspired gas appropriately, suction of respiratory secretions whenever required, and due diligence in hand washing and other related safety measures as stipulated in proposed patient care protocols will all facilitate a significant reduction in the rate of VAP in mechanically ventilated patients in the ICU. In order to effectively prevent the incidence of VAP, and decrease the patient's length of stay, and healthcare costs, it is of considerably significant importance that appropriate and effective implementation of a multifaceted intervention that consists of methodologically developed tools and strategies in the facility, as elaborated in the proposal and as annexed in the Appendices, with a goal to increase the compliance of the staff with the VAP bundle. This project has been initiated by the author after conducting thorough critical appraisal of fifteen evidence-based research articles published in academic journals to examine and review the existing evidence about the test studies that confirmed the effectiveness and applicability of the VAP bundle in preventing incidence of VAP in mechanically ventilated patients. Subsequently the project has been taken up for further initiatives pertaining to the implementation by the Unit Practice Council (UPC). The interdisciplinary team participating in this project includes physicians, nurses, respiratory therapists, and clinical leaders. This proposal discusses in details about the issues associated with of VAP and then proposes a modified plan for the nursing and clinical interventions in the ICU. The project plan has also developed tools to increase staff compliance and knowledge with the components of VAP bundle care in order to prevent VAP rates in mechanically ventilated patients in the ICU. The UPC team established provisions and strategies to ensure the staff compliance on protocols of ventilator bundle care that has been proposed. The staff was provided with the education tools such as checklists, educational posters and power point presentation, daily goal sheets, and consistent feedbacks in order to achieve 100 percent adherence to VAP bundle care and thereby reduce the VAP incidence to zero. The study proved that with the compliance of the VAP bundle care, VAP rate has been considerably decreased in the ICU of Memorial Regional Hospital. It has been established that with the observance of VAP bundle care protocol for all mechanically ventilated patients, in collaboration with the members of an interdisciplinary team, a significantly cost effective and improved patient care outcome is expected achieve in the MRH.

These protocols are acceptable for use among individuals in either acute care settings or long-term-care facilities that are at high risk for ventilator associated pneumonia (VAP). This paper will discuss the implementation of an oral care protocol on evidence based research and best practice guidelines. This protocol was implemented in the Surgical Intensive Care Unit (SICU) at the Veterans Administration (VA) of the Tennessee Valley Health Care System (TVHS). The aim of this intervention was to decrease the rate of Ventilator Associated Pneumonia to three of below and maintain compliance with the protocol. The protocol required the use of a tooth brush instead of an oral swab and suctioning above the endotracheal tube cuff. The result of the change in practice showed a decrease in VAP to below three and compliance rate of greater than 80%.
