In fact, psychiatric patients are no more likely than patients in other medical specialities to go against the advice of their doctor. That said, it is easy to find instances where psychotropic medication is refused by the supposed beneficiary. The value of neuroleptic treatment in schizophrenia is now widely accepted. Failure to take such treatment is associated with relapse and relapse may endanger the patient and other people. Despite this, people with schizophrenia frequently fail to take their treatment. This study shows that one third can be expected to be non-compliant within two years of leaving a general adult psychiatry ward. It also looks at the reasons for this: the influence of drug side-effects is examined, as well as the impact of each patient's attitude to treatment and whether or not they have stopped taking prescribed medication in the past.

Efficacy rates are further compromised by medication non-adherence, which has been reported to affect more than half of patients. In response to these issues, several non-pharmacological interventions have been developed for the treatment of schizophrenia, such as cognitive behavioral therapy, cognitive remediation, social cognition training and metacognitive approaches. Although these interventions have produced promising results, there is still much controversy regarding their usefulness and applicability in clinical practice. Book excerpt: Background and Aims:Studying the degree of adherence to therapy in patients with various clinical forms of schizophrenic spectrum disorders, assessing the effect of adherence to therapy on the hospitalization rate.Methods:The study included 100 patients undergoing inpatient treatment on the basis of the City Clinical Psychiatric Hospital of Tashkent, according to the diagnostic criteria for schizophrenic spectrum disorders according to ICD-10 (F21.0, F22.0, F23.0, F25.0). The age of patients from 18 years. Disease duration at the time of the survey was at least 1 year. The study used: The scale of assessment of compliance with the treatment regimen (MARS, Thompson et al., 1999).Results:At the first hospitalization, according to the patients themselves, only 37 people were aware of the need for maintenance therapy (37%), realized but did not follow the recommendations of the attending physician - 51 people (51%), did not realize and did not follow the doctor's recommendations - 22 people (22% ). Within a year after the first hospitalization, 78 people (78%) were re-hospitalized (78%), of whom, against the background of non-compliance with drug therapy or partial non-compliance, 62 people (62%).Conclusion:Patients with schizophrenic spectrum disorders during the first year after the manifestation of the disease in most cases either did not comply with the regimen of drug therapy, or completely stopped taking the drugs, which is combined with a low level of awareness of the need for patients to continue therapy.

It involves a combination of medication, psychotherapy, and coordinated specialty psychosocial services (National Alliance on Mental Health, n.d.). Selecting the appropriate combination of treatment support makes it possible to achieve a successful recovery process. Non-adherence to the medication management of schizophrenia has been found to increase the dangers of hospitalization, symptom relapse, and a reduction in the patient's quality of life (Velligan et al., 2017). An evidence-based practice project that utilized the implementation of educational sessions on the Medication Adherence Rating Scale (MARS) to improve clinicians' knowledge and the subsequent utilization of the MARS in the clinic was undertaken. The theory and evidence-based practice (EBP) model that guided project development and implementation efforts were Neuman's system theory and Rosswurn and Larrabee's model. The clinical practice setting of the EBP project was a mental health community clinic in Texas. Thirteen clinicians participated in this EBP project. Outcomes results measured included knowledge and use of the MARS tool by providers in patients experiencing schizophrenia. The mean knowledge score increased to 94% on the posttest, which represented an increase of 25.3% over the pretest. On average, for the six weeks of data collection, 87% of the patients with schizophrenia were administered the MARS tool. This outcome met the identified benchmark of 80% completion rate on the MARS. Findings showed that there was a significant improvement in clinicians' knowledge on medication adherence upon completion of the education sessions as well as increased and appropriate utility of the MARS tool. Treatment effectiveness is often vitiated by medication noncompliance. In the current study, clinical and sociodemographic correlates of treatment compliance were examined in outpatients with schizophrenia. Level of medication compliance, assessed independently by treatment staff at two outpatient settings, resulted in 40 compliant, 38 noncompliant, and 12 partially compliant patients. Key symptoms associated with medication noncompliance were anger, delusions, and hallucinations. As an initial investigation, a stepwise discriminant analysis was moderately successful at predicting medication noncompliance. The implications of these findings to mentally disordered offenders are explored.

Demographic information including sex, age, years of education, and current occupation was obtained. Information about current medications and the onset of illness was collected from patients. Insight was measured using the insight (Q8) and attitudes towards medication by the drug attitude inventory (DAI) and general knowledge about psychosis of patients was assessed with modified multiple-choice questionnaire, Knowledge of Schizophrenia Test (KAST).uf076tResults: A total of 45 participants participated in the study, with 28 (62.2%) males, mean age of 34.1 u00b1 11.8 years. The mean total Q8 scores were 4.83 u00b1 3.1. The KAST total score was correlated with the Q8 (p = 0.001). The nonadherence rate was 43.8%. The medication-adherent patients had Higher score of Q8 (p = 0.0001) and higher KAST subscore of cause (p = 0.006). These two variables explained 38.9% of the variance in adherence behavior, with Q8 being the mediator. uf076tConclusion: This study highlights the importance of patientsu2019 understanding of etiology of the illness and its relationship with insight and medication adherence.

Book excerpt: Several contributions in our first book about schizo affective disorders (Marneros and Tsuang, Schizoaffective Psychoses, Springer-Verlag, 1986) supported the assumption that schizoaffective disorders differ in relevant ways from schizophrenic disorders. The classification of schizo affective disorders as a subgroup of schizophrenia has also been criticized, and empirical research in clinical, genetic, therapeutic, and prognostic areas supports the idea that there are some strong similari ties between schizo affective and affective disorders. Of course, there are not only similarities between these two groups, but also differences just as there are between schizo affective and schizophrenic disorders. It is precisely the existence of similarities and differences between schizo affective disorders and the other two so-called typical mental disorders, i.e., schizophrenia and affective disorders, which makes them a challenge in psychiatric research, a challenge to the traditional dichotomy in the classification of disorders which originated with Kraepelin. This challenge is certainly proving fruitful in psychiatric research. These "cases in between" may well demonstrate that sep arating, dividing, and limiting is not always meaningful. Sometimes it can be more meaningful to unify; to unify in the sense of building bridges between typical groups. It is here that the assumption of a "psychotic continuum" can become relevant, and the investigation of schizoaffective disorders is of prime importance in research on a possible continuum of psychosis.
