Book excerpt: Compassion fatigue is a term used to describe the unique stressors affecting people in care-giving professions (Joinson in 1992). For nurses, the impact of compassion fatigue may result in stress-related symptoms, job dissatisfaction, decreased productivity, patient satisfaction, safety issues, and job turnover. The purposes of this non-experimental descriptive survey using electronic distribution to collect data were to identify the prevalence of compassion fatigue composed of burnout and secondary traumatic stress and compassion satisfaction among nurses and nursing staff who deliver direct patient care at the hospital and the relationship between demographic variables (age, work category, level of education, work experience) and the presence of compassion fatigue, burnout, secondary traumatic stress, and compassion satisfaction on selected nursing care units at a Midwest pediatric hospital. Swanson's (1991) Caring Theory was used to guide this study. Compassion fatigue was normally distributed throughout the sample. Results demonstrated that nurses who work on the cardiology unit had higher compassion satisfaction scores than nurses who worked in the pediatric intensive care unit, cardiothoracic care unit and the emergency unit. Nurses working in the pediatric intensive care unit had higher secondary traumatic stress scores than nurses who work in the emergency unit and cardiology unit. Additionally, nurses who worked in the pediatric intensive care unit reflected lower compassion satisfaction scores as well as higher burn out and secondary traumatic stress scores than nurses in all of the other nursing care units studied. Compassion satisfaction, burn-out and secondary traumatic stress scores did not statistically differ by age, work category, level of education, or work experience. The results of this study demonstrate the existence of compassion fatigue at a Midwest pediatric hospital. This study provided data necessary to develop a program addressing compassion fatigue at this hospital. Results also suggest that nurses who work in the pediatric intensive care unit must be a priority group targeted for immediate intervention.

Repeated exposure to children's pain and suffering can lead to compassion fatigue. Scant literature describes either (a) the prevalence of compassion fatigue among pediatric nurses or (b) the personal, experiential, or work factors contributing to compassion fatigue. The purposes of this study were three-fold: first, to examine the prevalence of compassion fatigue among pediatric nurses in free-standing pediatric hospitals; second, to explore personal, experiential, and work characteristics as potential antecedents to compassion fatigue; and third, to determine whether or not these antecedents can help identify nurses with compassion fatigue or proclivity for compassion fatigue. A descriptive correlational research design was utilized. Established self-report survey instruments were used to gather information from nurses working at least fifty percent of their time in clinical care. The Professional Quality of Life Inventory was utilized for the variables of compassion fatigue, secondary traumatic stress, burnout, and compassion satisfaction. The Differentiation of Self and Role Inventory was utilized to assess self/role differentiation. The response rate was 16% (n=463). Compassion fatigue was present in 11% of the total sample. Book excerpt: Due to the nature of their work, healthcare providers working in pediatric settings may be especially vulnerable to experiencing negative consequences of compassion fatigue and burnout. Pediatric healthcare providers often work long hours in the hospital to manage children with acute and chronic medical concerns. Within this setting, unpredictable outcomes and potential for death when caring for this fragile population may contribute to short-and long-term effects on not only caregivers but also healthcare providers. Healthcare providers typically work on an interdisciplinary team within the pediatric hospital setting which can include medical doctors, nurses, social workers, psychologists, psychiatrists, and child life specialists. Providers come from a variety of academic and training backgrounds which may have different approaches with regard to managing challenges that arise in response to providing care for such a medically fragile population. The present study sought to examine healthcare providers' experiences of compassion fatigue and burnout as factors related to working in pediatric settings. Additionally, this study sought to understand health care providers' self-care practices as well as the barriers they encounter in attending to their mental health. Interviews with nine healthcare providers yielded six emerging themes. The themes included: (1) attitudes and awareness, (2) barriers to self-care, (3) impact on work/personal life, (4) coping strategies, (5) lack of systemic measures and resources, and (6) promotion of self-care in the workplace. The emerging themes expressed by the participants provide insight into their perceptions and experiences of compassion fatigue and burnout. Findings have clinical implications with regard to enhancing support of pediatric healthcare providers and provide justification for increased research with this population.

While nurses are an important part of healthcare, both for their compassion and care and for their skills at the bedside, they can become stressed and burned out. Compassion satisfaction is essential for nurses in today’s society. Compassion satisfaction has been defined as the pleasure an individual derives from being able to help others and do their work to the best of their ability. The lack of compassion satisfaction can lead to compassion fatigue. Compassion fatigue affects the care provided to patients. Compassion fatigue leads to burnout. Burnout has been defined as physical, mental, and emotional exhaustion and can lead to nurse disengagement. The purpose of this thesis was to assess the actual relationship between nurses’ burnout and the patient load during their workdays as well as the relationship between nurse compassion satisfaction and burnout. The theoretical framework was based on Helen Erickson’s Modeling and Role Modeling Theory. The participants of the study were registered nurses or licensed practical nurses working at the bedside. Study results showed a significant relationship between burnout and compassion satisfaction, while showing no association between burnout and patient load. The data analysis showed no significant correlation between patient load and the likelihood of leaving the job in the next year.

Often the healthcare workers will put their patients’ needs before their own. After an extended period of neglect nurses and healthcare workers often feel some burnout (BO) level, which leads to compassion fatigue. Healthcare workers cannot take care of their own physical and emotional needs, but they cannot be there for their patients like they should be able to provide optimum care. This can lead to medical errors, slower recovery time for patients and eventually nurses leaving the profession altogether due to BO. The purpose of this grant proposal is to examine if there is a relationship between different health promotion behaviors that California State University San Marcos (CSUSM) advanced practice nursing (APN) students may engage in and if there any long term effect on the level of compassion fatigue or BO that nurses experience as well as their varying levels of compassion satisfaction. The study’s dependent variable will be the CSUSM APN students’ responses on the self-reported CS and CF scores that will be measured by the Professional Quality of Life Survey version 5 (ProQOL). The relationship between the ProQOL scores and health promotion behaviors will then be evaluated to see if there are any patterns of significance. The data will be collected over a One month period of time. This study’s significance is to aid in the level of knowledge that we have about CF, BO, and its relationship to CS, improving patient care and retention of employees.

Book excerpt: "The continual use of self during stressful times to provide care and contact to the suffering, while being empathetic to another's circumstances, has the potential to develop into compassion fatigue (CF). CF is increasing within the nursing profession. The added demands placed upon nurses with the increase in patient care complexities take their toll on nursing staff emotionally. CF can lead to burnout, high vacancy rates and loss of experienced nurses within specialized areas of care. The focus of this quantitative, descriptive research study was to identify the prevalence of CF experienced by registered nurses working in the inpatient setting of the pediatric oncology unit. Data were collected from the ProQOL v.5 tool distributed to a convenience sample of registered pediatric oncology nurses. The participants were recruited on a voluntarily basis and informed consent was obtained prior to enrollment in the research study. All 39 registered nurses were asked to complete the survey, with 20 participating in the study. The oncology registered nurses surveyed were shown to have high levels of compassion satisfaction, low levels of burnout, and secondary stress associated with their work. Based on this triad of results, the sample group as a whole did not display the characteristics of CF. This finding, while unexpected, opens the door to further research within this population to determine how they manage to work within this field of nursing and not experience CF." --Abstract.

Book excerpt: The purpose of this study was to describe the prevalence of Compassion Fatigue (CF) in home care nurses who visit patients/clients in their home through home hospice, home health and a nurse home visitation program. The setting was a hospital affiliated home care agency in a southern town. Thirty five registered nurses volunteered to participate in this descriptive study. The study investigated the prevalence of compassion fatigue through the use of the fifth revision of the 30-item Professional Quality of Life Scale (ProQUOL) Compassion Satisfaction and Compassion Fatigue Scale. A series of crosstab analysis examined the relationship between participant demographics and three ProQUOL version 5 subscales of compassion satisfaction, burnout and secondary traumatic stress. There is value in analyzing the prevalence of compassion fatigue in home care nurses in healthcare. Understanding the needs of this population can be beneficial in reducing stress related loss of patient care providers and assist management in developing programs to retain staff.
