Book excerpt: At the Battle of the Wilderness, General Ulysses Grant was interrupted in conversation with an aide to request use of an ambulance for a civilian doctor to visit the field hospitals. Grant refused repeatedly until he was told that the doctor was William Thomas Green Morton, the dentist who first demonstrated the use of ether. Grant said, "You are right, Doctor, he has done more for the soldier than any one else, soldier or civilian, for he has taught you all to banish pain. Let him have the ambulance and anything else he wants." In the autumn of 1862, Morton joined the Army of the Potomac as a volunteer surgeon, and applied ether to more than two thousand wounded soldiers during the battles of Fredericksburg, Chancellorsville, and the Wilderness. The research examines the interaction of anesthetic agents appropriate for use in a combat environment, with hemorrhage. In doing so, the physiological processes that contribute to the differences among anesthetic agents for induction and maintenance of anesthesia during hemorrhage will be defined. Swine are used as the experimental model, examine the rationale and physiology of use of nitrous oxide, enflurane, isoflurane, halothane, thiopental and ketamine for induction of anesthesia during the hypovolemic condition. The products of this project will be important and meaningful data and recommendations to be provided USAMRDC, AHS, and ultimately the user--the anesthetist in a combat environment--regarding use (potential advantages and disadvantages) of anesthetic agents for acutely injured soldiers.

Out-patient cosmetic surgery clinics have sprouted up in droves all over the U.S., and the number of cosmetic procedures performed in 2005 increased by over 95% from the previous year. Although procedures like facelifts and abdominoplasties (the 'tummy-tuck') are considered minimally invasive, the anesthetic protocols and regimens here are often overly complex and unnecessarily toxic. Major complications involving anesthesia in this (and any other) surgical milieu can range from severe post-operative nausea and vomiting (PONV) to neuromuscular spasticity to mortality. The mortality spectrum of things may be rare, but there have been many cases in which perfectly healthy cosmetic surgery patients require emergency intervention due to a severe complication involving anesthesia. In recent years, many new anesthetic protocols have been developed to reduce the incidence of PONV and other complications, while ensuring that effective pain management and level of 'un-awareness' during surgery is always maintained.
