Book Columbia Crew Survival Investigation Report

Download or read book Columbia Crew Survival Investigation Report written by Nasa and published by PDQ Press. This book was released on 2009 with total page 400 pages. Available in PDF, EPUB and Kindle. Book excerpt: NASA commissioned the Columbia Accident Investigation Board (CAIB) to conduct a thorough review of both the technical and the organizational causes of the loss of the Space Shuttle Columbia and her crew on February 1, 2003. The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. The damage was undetected during the mission. The Columbia accident was not survivable. After the Columbia Accident Investigation Board (CAIB) investigation regarding the cause of the accident was completed, further consideration produced the question of whether there were lessons to be learned about how to improve crew survival in the future. This investigation was performed with the belief that a comprehensive, respectful investigation could provide knowledge that can protect future crews in the worldwide community of human space flight. Additionally, in the course of the investigation, several areas of research were identified that could improve our understanding of both nominal space flight and future spacecraft accidents. This report is the first comprehensive, publicly available accident investigation report addressing crew survival for a human spacecraft mishap, and it provides key information for future crew survival investigations. The results of this investigation are intended to add meaning to the sacrifice of the crew's lives by making space flight safer for all future generations.

Book Working in Organisations

Download or read book Working in Organisations written by Andrew Kakabadse and published by Routledge. This book was released on 2018-12-20 with total page 326 pages. Available in PDF, EPUB and Kindle. Book excerpt: First published in 2004, this volume recognises that, as society changes, so must its organisations; as organisations change, so must their management competencies. The requirement for organisations to be flexible, innovative and adaptable in environments of increasing complexity and uncertainty is also a requirement of any organisation’s most essential resource: its people. They not only work for the organisation – they are the organisation. The second edition of this highly successful book provides a comprehensive and thought-provoking examination of the operational and strategic skills, demands and responsibilities of the modern workforce, and in particular its management. Reflected here are the manager’s new and changing priorities. Attention is given to the manager’s role, the motivation to work and succeed, and teamworking. Also emphasised is the new psychological contract, highlighting reliance on self whilst maintaining sensitivity to diversity concerns. Providing clear and decisive leadership requires the projection of a vision that captures the imagination of others, but inevitably managers face conflict and adversity. Working in Organisations therefore discusses how power and politics can be moulded to positive advantage. The challenges facing organisations go beyond cost and profitability, as political and environmental challenges have forcefully entered into managerial responsibility. The book asks 'What is the ultimate purpose and contribution of organisations?' and highlights profound governance and ethics concerns. The design of organisations is also explored, and how creating appropriate structures will in turn focus resources to achieve desired ends. This book provides a broad coverage of key issues, ranging from a close examination of the manager's job to a discussion of the corporate and social forces that determine our lives. Written in an easy-to-read style and bursting with case examples, Working in Organisations acts as mentor and guide to those whose quest is for ever greater sustainable accomplishment. Written by a distinguished team of authors, this book will continue to be welcomed as the definitive text on organisational culture and change for academics, researchers and managers around the world.

Book excerpt: Humanity's first reusable spacecraft and the most complex machine ever built, NASA's Space Shuttle debuted with great promise and as a dependable source of wonder and national pride. But with the Challenger catastrophe in 1986, the whole Space Shuttle program came into question, as did NASA itself, so long an institution that was seemingly above reproach. Wheels Stop tells the stirring story of how, after the Challenger disaster, the Space Shuttle not only recovered but went on to perform its greatest missions. From the Return to Flight mission of STS-26 in 1988 to the last shuttle mission ever on STS-135 in 2011, Wheels Stop takes readers behind the scenes as the shuttle's crews begin to mend Cold War tensions with the former Soviet Union, conduct vital research, deploy satellites, repair the Hubble Space Telescope, and assist in constructing the International Space Station. It also tells the heart-wrenching story of the Columbia tragedy and the loss of the magnificent STS-107 crew. As complex as the shuttle was, the people it carried into orbit were often more so--and this is their story, too. Close encounters with astronauts, flight controllers, and shuttle workers capture the human side of the Space Shuttle's amazing journey--and invite readers along for the ride. Book NASA 50th Anniversary Proceedings NASA s First 50 Years Historical Perspectives

Download or read book NASA 50th Anniversary Proceedings NASA s First 50 Years Historical Perspectives written by Steven J. Dick and published by U. S. National Aeronautics & Space Administration. This book was released on 2010-07-07 with total page 784 pages. Available in PDF, EPUB and Kindle. Book excerpt: On 29 July 1958, President Dwight D. Eisenhower signed the National Aeronautics and Space Act, creating the National Aeronautics and Space Administration (NASA), which became operational on 1 October of that year. Over the next 50 years, NASA achieved a set of spectacular feats, ranging from advancing the well-established field of aeronautics to pioneering the new fields of Earth and space science and human spaceflight. In the midst of the geopolitical context of the Cold War, 12 Americans walked on the Moon, arriving in peace “for all mankind.” Humans saw their home planet from a new perspective, with unforgettable Apollo images of Earthrise and the “Blue Marble,” as well as the “pale blue dot” from the edge of the solar system. A flotilla of spacecraft has studied Earth, while other spacecraft have probed the depths of the solar system and the universe beyond. In the 1980s, the evolution of aeronautics gave us the first winged human spacecraft, the Space Shuttle, and the International Space Station stands as a symbol of human cooperation in space as well as a possible way station to the stars. With the Apollo fire and two Space Shuttle accidents, NASA has also seen the depths of tragedy. In this volume, a wide array of scholars turn a critical eye toward NASA’s first 50 years, probing an institution widely seen as the premier agency for exploration in the world, carrying on a long tradition of exploration by the United States and the human species in general. Fifty years after its founding, NASA finds itself at a crossroads that historical perspectives can only help to illuminate.

Available in PDF, EPUB and Kindle. Book excerpt: Voted the Best Space Book of 2018 by the Space Hipsters The dramatic inside story of the epic search and recovery operation after the Columbia space shuttle disaster. On February 1, 2003, Columbia disintegrated on reentry before the nation’s eyes, and all seven astronauts aboard were lost. Author Mike Leinbach, Launch Director of the space shuttle program at NASA’s John F. Kennedy Space Center was a key leader in the search and recovery effort as NASA, FEMA, the FBI, the US Forest Service, and dozens more federal, state, and local agencies combed an area of rural east Texas the size of Rhode Island for every piece of the shuttle and her crew they could find. Assisted by hundreds of volunteers, it would become the largest ground search operation in US history. This comprehensive account is told in four parts: Parallel Confusion Courage, Compassion, and Commitment Picking Up the Pieces A Bittersweet Victory For the first time, here is the definitive inside story of the Columbia disaster and recovery and the inspiring message it ultimately holds. In the aftermath of tragedy, people and communities came together to help bring home the remains of the crew and nearly 40 percent of shuttle, an effort that was instrumental in piecing together what happened so the shuttle program could return to flight and complete the International Space Station. Bringing Columbia Home shares the deeply personal stories that emerged as NASA employees looked for lost colleagues and searchers overcame immense physical, logistical, and emotional challenges and worked together to accomplish the impossible. Featuring a foreword and epilogue by astronauts Robert Crippen and Eileen Collins, and dedicated to the astronauts and recovery search persons who lost their lives, this is an incredible, compelling narrative about the best of humanity in the darkest of times and about how a failure at the pinnacle of human achievement became a story of cooperation and hope.
