The whole of it, not withstanding, has been most carefully revised, and the volume extended by the addition of fifty pages, with twenty-one new cases and thirty-four new wood-cuts. I have endeavoured throughout, by reference to indisputable facts, to demonstrate the correctness_ of the principles which have guided my prac tice, and have, i therefore, authenticated every case with the name of its reporter in the hospital books. I have availed myself of numerous illustrations engraved on wood, having long been persuaded that mere description 'of morbid appearances, and especially of those that are made visible by means of the microscope, communicates only feeble or imper feet ideas to others. Of these illustrations, such as are borrowed have the names of their authors appended; such as have no name attached are original. Excerpt: ...over the apex or immediately below the left nipple, but When, as is often the case, the inflammation is limited to the semi-lunar valves of the aorta and adjacent parts, an equally distinct and rather rough bellows murmur may be traced from the base of the heart upward along the course of the aorta nearly or quite to the arch. The sound of course diminishes in proportion as we recede from the heart itself. In many cases of endocarditis the inflammation extends beyond the: avity of the heart along the lining of the aorta, causing more or less thickening and roughness of the membrane in this large artery, and in such cases the harsh rough sound, synchronous with the systole of the heart, may be heard as distinctly or even more distinctly than the ordinary bellows murmur over the heart itself. It is rare that these sounds are heard as distinctly over the right as over the left side of the heart. In cases which prove protracted, extending through two, three or four weeks, or as is often the case when associated with rheumatism, the alterations of the mitral valve, or the semilunar of the aorta, and sometimes of both in the same case, become so great that the valves fail to close their respective openings during the systolic action of the heart, the bellows murmur becomes altered in such a direction as to indicate regurgitant sounds. If the alterations include the in'tral valve only, the regiirgitant sound will accompany each impulse oftlie heart. If the semi-lunar valves of the aorta fail in their office the regurgitant sound will be heard more over the base of the heart and coniinencement of the aorta, and will immediately follow the impuise, being synchronous with the diastole.
