Book excerpt: Problem The problem investigated in this study was the lack of empirical evidence available regarding the effectiveness of electronic interactive simulation (EIS) for developing critical thinking disposition and clinical judgment skills in the senior baccalaureate nursing student. Aim The aim of this study was to identify an effective method of experiential learning simulation that may be independently accessed by the learner with a goal of enhancing critical thinking disposition and clinical judgment skills of senior baccalaureate student nurses (BSN). Purpose The purpose of this experimental study was to compare the effects of EIS to traditional paper case studies on the critical thinking disposition and clinical judgment skills, measured by accuracy and efficiency of situational decision making, of senior nursing students enrolled in baccalaureate nursing programs in the United States. Methods One hundred and seventeen senior nursing students completed the randomized control study by using either the EIS or paper case study learning intervention. Repeated measures ANOVA and nonparametric tests were used to test the hypotheses that senior BSN who participate in EIS of real-life clinical scenarios over a period of two weeks will experience significant increases in clinical judgment and critical thinking disposition compared to students who receive traditional paper case study simulation. Findings Results showed that participants who used EIS over a two-week period increased their scores for critical thinking disposition overall and on three of the subscales. Results also indicated a positive trend, greater than the comparison group, on the remaining subscales. It is noted that many scores for the Case Study group actually decreased, suggesting that this method had a stifling effect on the development of critical thinking disposition. Retention and application of learned information was apparent for both groups, however, there was a trend for a greater change in the EIS group compared to the Case Study group. Additional research is needed to explore the effectiveness of this emerging pedagogy to add to what is known about the effects of experiential learning in the healthcare professions.

Book excerpt: "The growing complexity of health care requires nurses to know, watch, do, and manage more, with more interdisciplinary team members than ever before leaving newly graduated nurses lacking the ability to apply new technology safely and appropriately. Hence, the significance of effectively preparing senior nursing students (SNSs) through the development of clinical reasoning, clinical judgement and critical thinking skills in order to face the challenges of their new career. Therefore, the aim of this evidenced-based practice (EBP) change project was to build stronger critical thinking skills in SNSs; thereby, effectively preparing them for transition from college into the health care system. The population consisted of SNSs in a rigorous 24-month rigorous Associate Degree Nursing (ADN) program in an urban Pacific Northwest community. Nine SNSs participated in this EBP change project. This practice change implemented scenario based simulation centered learning experience (SCSBLE) followed with debriefing for meaningful learning (DML) to increase critical thinking skills in SNSs. The results of these learning experiences were very positive, with an increase from pre-testing to post-testing scores by 7.22% demonstrating an increase in critical thinking skills in addition to students verbally expressing better assessment skills post SCSBLE/DML. Concluding, through SCSBLE/DML experiences with SNSs critical thinking skills and confidence were built. Subsequent implementations must be administered with larger numbers of participants to solidify the evidence. These abilities are used continually, enabling nurses to provide safe nursing care to increasingly complex patients in a variety of healthcare settings. The challenge that faces baccalaureate nursing programs as they prepare nursing students to function competently upon graduation, is how best to facilitate the development of these skills in our students. The purpose of this capstone project was to test the effect of the middle range theory of the Developing Nurses' Thinking (DNT) Model on clinical judgment in nursing students. Tanner's Clinical Judgment Model formed the conceptual framework for this project. An experimental, pretest/posttest study was conducted using a convenience sample of 44 senior students at one southeastern baccalaureate nursing program. Two clinical groups received the intervention in post conferences, while three groups served as the control group. Student clinical judgment was measured in high fidelity patient simulation, using the Lasater Clinical Judgment Rubric. Paired t-tests evaluated the differences between clinical judgment scores and an independent t-test was utilized to evaluate the difference between groups. Both groups showed statistically significant improvements on the posttest, but there was no statistically significant difference between groups. Therefore, the findings of this study did not support the use of the DNT Model to facilitate the development of clinical judgment in nursing students. However, small sample size and inadequate exposure to the intervention were likely contributors to these outcomes.

Improvement of critical thinking skills is essential to ensure patient safety as well as provide for the success of each professional nurse. Patient outcomes have been the focus of numerous research studies and correlation between critical thinking and clinical judgment has been demonstrated. The purpose of this study was to determine if critical thinking skills of undergraduate nursing students in a clinical setting could be improved. This was accomplished by utilizing Rosalinda Alfara-LeFevre's methods of developing critical thinking skills outlined in "Critical Thinking and Clinical Judgment, A Practical Approach". The experimental group was encouraged, mentored and instructed on the 'how to' of critical thinking techniques. The control group was not given the additional tools. Comparison of the two groups at the beginning of the clinical rotation and again at the completion will allow data to be compiled for statistical analysis. The data will be collected by the use of self and peer evaluations of Critical Thinking Indicators including both knowledge and behavior aspects. This study will be qualitative and quantitative in nature. While the scope of this study is limited to senior level students in one program, it provides a framework within which further evaluation of critical thinking assessment can be accomplished.

Book excerpt: The purpose of this qualitative study was to investigate and describe nursing students' and clinical instructors' understandings of critical thinking and to discover which clinical experiences were significant in allowing students to develop critical thinking abilities. Interpretive qualitative methodology was used to explore students' and instructors' perceptions of critical thinking experiences that occurred during clinical education. Data were obtained from 11 student participants and 4 instructor participants using responses to prompts defining critical thinking experiences and follow-up interviews. There were three overarching findings from this study: (a) students and instructors described similar characteristics of clinical experiences that were significant in developing critical thinking including complex situations warranting independent identification, interpretation, and decision making by students; (b) students and instructors described critical thinking as the ability to process an unclear situation, understand the significance of the context, and know what to do next; and (c) students and instructors revealed a disconnect regarding instructor significance and role during clinical experiences. The findings of this study have demonstrated the importance of clinical experiences and preparing for them and debriefing after them for students and the development of their critical thinking. Implications for nursing education include the need to implement certain strategies that maximize critical thinking experiences in the clinical setting, the need for education and training for clinical instructors, and the need for improved discourse between students and instructors regarding clinical experiences.
