Nous souhaitons que cet ouvrage soit un point de référence actuel de l’échographie de contraste, point qui demandera certainement à évoluer dans les prochaines années. Il démontre avec force que les ultrasons peuvent jouer un rôle très important dans l’imagerie moderne tant diagnostique que thérapeutique en raison de leur performance diagnostique mais également de leur caractère non invasif. Nous espérons que vous prendrez autant de plaisir à y rechercher l’information utile que nous en avons eu à découvrir une imagerie fonctionnelle ultrasonore en temps réel et écrire ces différents chapitres.

Book excerpt: The aim of this work was to study the potential for contrast-enhanced functional ultrasound imaging (Cefui) to assess the abdominal microcirculation, according to three major axes of research: 1) The first axis consisted in proposing and validating a new method improving non invasive contrast-enhanced renal blood flow measurements, using the mean signal intensity in a feeding artery in order to normalize contrast-enhanced renal blood flow estimation. However, while these ex vivo results suggest that this technique has great potentials, other parameters, mainly ultrasound beam attenuation, must be taken into account to obtain reproducible measurements. 2) The second axis consisted in evaluating qualitative abdominal Cefui for tumoral detection. For the detection of liver metastases from carcinoid tumor, the first study reports sensitivity values for contrast-enhanced ultrasound similar to those obtained with enhanced computed tomography. The second study demonstrates that Cefui provides high specificity and predictive values for the early detection of residual tumor after radiofrequency ablation of renal tumors even though its sensitivity remains lower than that of contrast-enhanced CT. 3) The third axis deals with added diagnostic value in terms of characterization of renal tumors of quantitative assessment of tumor perfusion compared to that of qualitative analysis. Abdominal Cefui has unique advantages including low cost, availability, absence of renal toxicity and radiation, and excellent temporal resolution. However, it still requires improvements and standardization to be reproducible and be transferred to the clinical setting. It may somehow yield quantitative information with respect to abdominal organs microcirculation. Further studies correlating Cefui results with those of other functional imaging methods are needed

Book excerpt: Objectifs : Etudier la faisabilité de l'échographie de contraste (ECUS) en pédiatrie, en utilisant un produit de contraste ultrasonore hors AMM, évaluation de la tolérance clinique et de la pertinence diagnostique en comparaison aux données de la littérature chez l'adulte. Matériel et méthodes : Une étude rétrospective a été réalisée, après avis favorable du comité d'éthique, entre décembre 2003 et décembre 2011, incluant 563 examens d'ECUS soit 610 examens par organe cible, chez des enfants de la naissance à 18 ans. Ces examens ont exploré l'ensemble des organes accessibles par échographie. Les résultats ont été classés en deux cohortes (diagnostic et de suivi), et en fonction du type de pathologie et de l'organe cible. Résultats : Aucun effet secondaire grave n'a été recensé au cours de cette étude, seulement trois effets secondaires mineurs dont le caractère imputable reste difficile à évaluer. Dans 1 % des cas, l'ECUS n'a pas pu être classé en raison d'un échec d'injection ou d'une lésion non accessible. Dans seulement 1,7 % des cas, l'ECUS n'a apporté aucune information en comparaison à l'échographie en mode B et dans 69,9 % des cas, le diagnostic de certitude a pu être affirmé. Concernant la cohorte de suivi, l'ECUS pourrait trouver une place fondamentale, en permettant de multiplier les examens de suivi sans surexposition. Au décours de cette étude, 5 cas illustrés ont mis en évidence une sémiologie spécifique à l'ECUS nécessitant des investigations complémentaires à plus grande échelle. Conclusion : Malgré de nombreux biais que présente cette étude rétrospective, les premiers résultats ouvrent de nombreuses et de nouvelles perspectives dans le domaine de l'ECUS en pédiatrie. Cette analyse préliminaire à aussi grande échelle constitue une étude princeps pour de nombreuses études prospectives.

Second-generation USCA, are blood pool agents that do not leak into the organ tissue to be examined but remain in the intravascular compartment increasing the Doppler signal amplitude during their dynamic vascular phase. Taking advantage of the stability of their microbubbles, they can withstand the acoustic pressure of insonation much better than first-generation contrast media, which results in an increased half-life of the agent and, consequently, in a prolonged diagnostic window. Concomitant with the improvement of contrast agents, different contrast-specific imaging modalities have been developed which, used in combination with USCA and a low mechanical index, allow continuous real-time grey-scale imaging. These recent technical improvements have opened new possibilities in the use of USCA in a variety of indications. Written by internationally renowned experts, the contributions gathered in this book give an overview of current and possible future new applications of USCA in routine and clinical practice.
