Book excerpt: Thailand is the second-fastest aging country in the Association of Southeast Asian Nations (ASEAN) after Singapore, and it is growing old before it grows rich. Thailand has already made considerable progress in recognizing the ageing challenge and has initiated policy reforms and development programs to address it at both national and local levels. The 2012-16 National Economic and Social Development Plan and, more recently, the 2017-2036 National Strategy, underscore the welfare of older persons as a government priority. At the heart of Thailand's approach is universal health insurance, supported by a community-based primary health care system with a reliance on volunteers. Thailand is now working to expand access to long-term care (LTC) at the community level as part of this volunteer-supported primary health care system. This report was prepared based largely on secondary sources, given the limitations on travel during 2020. It makes use of the comprehensive results of the Survey of Older Persons in Thailand, as well as excellent studies produced by various government, non-government, and academic authors. Nevertheless, in preparing the report the authors noted considerable data gaps, especially on private sector services, costs and usage, labor force, and projections of future demand for care. The team fact-checked the report with various country and sector experts; however, it should be considered a preliminary overview of the current situation. Additional data and consultation are needed to elaborate on the recommendations provided in this report.

Demographically the most important continent on the world, Asia's population, currently estimated to be 4.2 billion, is expected to increase to about 5.9 billion by 2050. Rapid declines in fertility, together with rising life expectancy, are altering the age structure of the population so that in 2050, for the first time in history, there will be roughly as many people in Asia over the age of 65 as under the age of 15. It is against this backdrop that the Division of Behavioral and Social Research at the U.S. National Institute on Aging (NIA) asked the National Research Council (NRC), through the Committee on Population, to undertake a project on advancing behavioral and social research on aging in Asia. Aging in Asia: Findings from New and Emerging Data Initiatives is a peer-reviewed collection of papers from China, India, Indonesia, Japan, and Thailand that were presented at two conferences organized in conjunction with the Chinese Academy of Sciences, Indian National Science Academy, Indonesian Academy of Sciences, and Science Council of Japan; the first conference was hosted by the Chinese Academy of Social Sciences in Beijing, and the second conference was hosted by the Indian National Science Academy in New Delhi. The papers in the volume highlight the contributions from new and emerging data initiatives in the region and cover subject areas such as economic growth, labor markets, and consumption; family roles and responsibilities; and labor markets and consumption.

Book excerpt: Preparing for the Challenges of Population Aging in Asia discusses the challenges posed by a rapidly aging population and identifies needed research to help policymakers better respond to them. While the percentage of elderly people in nearly every nation is growing, this aging trend is particularly stark in parts of Asia. Projections indicate that the portion of the population age 65 and older will more than triple in China, India, and Indonesia and more than double in Japan between 2000 and 2050, based on data from the United Nations. Moreover, this demographic shift is coinciding with dramatic economic and social changes in Asia, including changing family structures and large-scale migrations from rural to urban areas. These trends raise critical questions about how nations can develop policies that best support health and economic well-being in large and growing populations at older ages. Governments in Asia still have time to determine the best ways to respond to the unfolding demographic transformation, but taking advantage of this window of opportunity will require new research to shed light on the status and needs of the aging population. Currently the research base on aging in this region is relatively underdeveloped. This book identifies several key topics for research to inform public policy, including changing roles in the family; labor force participation, income, and savings; and health and well-being of the public.

They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Beginning with a focus on demographic trends, the report puts the rapid decline in fertility and contrasting migration trends in the region in a historical perspective and looks forward to the varying paths that population change may follow in the region. Next, it examines the evidence on the likely impact of demographic change on growth and savings, the labor force, firm and economy-wide innovation, poverty and inequality, and intergenerational solidarity. A national survey found that 16 per cent of elderly households in the rural areas live in substandard conditions. The majority of the elderly (60%) rely on family support for their living while another 20 per cent work for a living and only 4 per cent have government pension. Thailand is now organising a National Saving Fund to promote savings for retirement. Another national survey found that 80 per cent of the population would like to save for retirement but only 48 per cent think that they can save regularly on a monthly basis. This is consistent with yet another survey which found that 50-60 per cent of the elderly actually prepared themselves physically and mentally for the ageing period. Though the survey found the elderly to be less happy than the young, they were quite healthy, that is, about 90 per cent of those in the 65-74 age group could take care of themselves. As it is quite usual in Thai culture for children to take care of their old parents, more than 80 per cent of the population expect that their children will take care of them physically, mentally, and financially when they become old. Daughters usually take care of their aged parents. About 45 per cent of the elderly who are older than 94 years are taken care of by a daughter or daughter-in-law, while another 38 per cent look after themselves. The United Nations (UN) projects Thailand to have an old-age dependency ratio of 38 in the next 30 years. Without income security and long term care schemes for the elderly, it would be very difficult for working children to take care of their parents in the future.
