After 30 years of use in the clinic, cisplatin remains a central element of many treatment regimens. Cisplatin is still an irreplaceable component of a regimen that produces high cure rates in even advanced nonseminomatous germ-cell cancers, and is widely used in the treatment of ovarian cancers and other gynecologic cancers, head and neck, and numerous other tumor types. The development of carboplatin has reduced some of the adverse events associated with cisplatin treatment, and the introduction of the DACH platinum compound oxaliplatin has broadened the spectrum of activity of the platinums to include gastro-intestinal cancers, especially colorectal cancer. The clinical importance of this family of drugs continues to drive investigation into how these drugs work and how to improve their efficacy and reduce their toxicity. The papers in this volume were presented in Verona, Italy, during the tenth International Symposium on Platinum Coordination Compounds in Cancer Chemotherapy. The symposium was jointly organized by the Department of Oncology of the Mater Salutis Hospital – Azienda Sanitaria Locale 21 of the Veneto Region – and by the Department of Medicine and Public Health, Section of Pharmacology, the University of Verona. In recent years we have frequently heard the expression "war on cancer," but compared to the carnage inflicted by cancer, our scientific and medical efforts, to date, would seem more like a minor skirmish. Some comprehension of the cancer problem can be obtained from a look at the current and projected casualty list for the United States. In this country, about 700,000 new cases of cancer will be diagnosed in 1976 and over 1 million known cases will continue to be treated. Over 400,000 of these patients will die from cancer in our bicentennial year. With the incidence of cancer in the United States increasing to 5.2% in 1975, compared to the 1.1 % yearly rate experienced for decades, Dr. F. J. Rauscher, Jr.,338 Director of the National Cancer Institute, has estimated that more than 10 million people will be under treatment for cancer and nearly 4 million will expire from cancer in this decade. At that rate, cancer will appear in nearly two of three families and the necessary medical care will cost some $15-20 billion per year. Thus unless methods for the treatment and control of cancer are markedly improved, about 53 million Americans now alive will eventually be cancer patients. Unfor tunately the major types of human cancer are still beyond curative care by surgical and radiological techniques and because of the paucity of currently available cancer chemotherapeutic drugs with curative potential.

These include childhood leukemia, adult leukemias and lymphomas, as well as testicular and trophoblastic tumors. The addition of high-dose chemotherapy with growth factor and hemopoietic stem cell support has increased clinical remission rates even further. For the majority of patients with some of the more common malignancies, however, palliation (rather than cure) is still the most realistic goal of chemotherapy for metastatic disease. The failure of chemotherapy to cure metastatic cancer is commonly referred to among clinicians as "drug resistance". This phenomenon can, however, often be viewed as the survival of malignant cells that resulted from a failure to deliver an effective drug dose to the (cellular) target because of anyone of or combination of a multitude of individual factors. Clinically, this treatment failure is often viewed as the rapid occurrence of resistance at the single cell level. However, in experimental systems, stable drug resistance is usually relatively slow to emerge.

The introduction to Section A brings the summary of such biosynthetic products to literature available November 1, 1977. Section B incorporates a summary of data of essentially all previously isolated and characterized marine animal constituents irrespective of biological activity.

Despite the serendipitous discoveries and inspired insights which tantalized us with striking remissions, or the rare tumors which proved to be fully susceptible to a given agent, in the main, there has been either total failure or a painfully slow acquisition of an armamentarium against a limited number of malignancies. To expect more, however, was the result of ignorance of the malignant cell, for, as has been described in the previous volumes of this series, the exploitable differences between malignant and normal cells are few or undiscovered. "Differences" is the "numerator" in this formula, but "exploitable" is the operational term, for, although a great number of differences bet\\\een normal and malignant cells have been described, rarely are these differences observed in a vital metabolic pathway or a crucial macromolecu lar structure. Essentially, the basic metabolic pathways and nutritional require ments for :lOrmal and malignant cells are the same, resulting in the fact that no chemotherapeutic agent can successfully inhibit a function in the majority of malignant cells without adversely affecting a similar function in the normal cell. It was, therefore, naive to expect a "magic bullet" which would select the malignant cell and destroy it.
