Book excerpt: "Trainers for force on force training exercises at the Army's maneuver combat training centers and at home station are often distracted from coaching and mentoring responsibilities by the need to perform exercise control and feedback (OAF) functions. The fielding of new weapons and reconnaissance, surveillance, and target acquisition (RSTA) systems as part of force modernization will overwhelm trainers with new requirements unless improved concepts for tactical engagement simulation (TES) and instrumentation systems (IS) are implemented. This study produced an online database that was used to assess the benefits of implementing various new TES and IS concepts, or combinations of concepts, in terms of the number of OAF functions automated, the extent to which each function disrupts trainer coaching and mentoring activities, the number of gaps in training feedback addressed, and the number of systems to which each function or feedback gap applies. The TES and IS concepts we evaluated were designed to address the additional goal of avoiding the stove-pipe nature of past systems. The online database can be used to extent%mine the benefits of additional TES and IS concepts. The study sponsor is using the results to define requirements for future TES systems and IS for live training at CTCs and home stations."--DTIC.

Targeting of immune checkpoints cytotoxic T-lymphocyte-associated protein 4, programmed cell death protein 1 (PD-1) and its ligand (PD-L1) has led to improving survival in a subset of patients. Despite their remarkable success, clinical benefit remains limited to only a subset of patients. A significant limitation behind these current treatment modalities is an irregularity in clinical response, which is especially pronounced among checkpoint inhibition. Currently, relevant predictors of cancer immunotherapy response include microsatellite instability-high/deficient mismatch repair (MSI-H/dMMR), expression of PD-L1, tumor mutation burden (TMB), immune genomic characteristics, and tumor infiltrating lymphocytes (TILs). However, none of them have sufficient evidence to be a stratification factor. Moreover, as the combined strategies for effective cancer immunotherapy had been developed in multiple tumors, such as Immunotherapy combined with chemotherapy, radiotherapy, targeted therapy and anti-angiogenesis therapy. Therefore, the development of novel biomarkers endowed with high sensitivity, specificity and accuracy able to identify which patients may truly benefit from the treatment with cancer immunotherapy would allow to refine the therapeutic selection and to better tailor the treatment strategy.

Both metabolic phenotype and metabolic dependence are changed during the development of cancer from precancerous tissue to local invasion to metastasis. Cancer cells autonomously alter their flux through various metabolic pathways in order to meet the increased bioenergetic and biosynthetic demand as well as mitigate oxidative stress required for cancer cell proliferation and survival. The change of the tumor microenvironment (TME) is a key factor driving tumor metabolic reprogramming. The tumor microenvironment denotes the non-cancerous cells and components presented in the tumor, including molecules produced and released by them. The constant interactions between tumor cells and the tumor microenvironment play decisive roles in tumor initiation, progression, metastasis, and response to therapies. More and more evidence has pointed out that the change in the tumor microenvironment is a key factor driving tumor metabolic reprogramming. However, the specific relationship between tumor metabolic reprogramming and tumor microenvironment remains unclear. Indeed, emerging evidence indicates that cancer cells are able to suppress the anti-tumor immune response by competing for and depleting essential nutrients or otherwise reducing the metabolic fitness of tumor-infiltrating immune cells. The actual process of how metabolic reprogramming and cancer immune response affect each other has not been understood.

Book excerpt: Epithelial ovarian cancer (EOC) is the most lethal gynecological disorder due to a lack of effective early detection strategies. Worldwide, approximately 230,000 women are diagnosed annually, whereas 150,000 die. It represents the seventh most commonly diagnosed cancer among women in the world with 5-year survival rate of 46%. More than one-fifth of EOC have been related to hereditary conditions. Considerable efforts have been made to implement screening of the general population to diagnose EOC early; nevertheless, this has been ineffective and there is no approved strategy. Nowadays, new approaches for early diagnosis and prevention based on molecular genomics are in development. Whole genome sequencing has established the potency of the somatic genome, characterised with diverse DNA repair deficiencies that can be used to stratify EOCs into distinct biological groups with predictive signatures of resistance or relapse. The incorporation of next-generation sequencing (NGS) into clinical practice remains challenging for two reasons. Firstly, the EOC risk is not clear for some of the included genes and secondly, the variant of uncertain significance rates increase as more genes are analyzed. Finally, beyond germline pathogenic variants, somatic mutations may also affect therapeutic choices, and as such upfront tumor sequencing may be equally important to NGS, particularly as we continue to challenge treatment paradigms in the first‐line management of EOC.
