As a counterpart to German and Italian intervention on behalf of the insurgents, the Soviet Union instructed the Comintern to recruit and organize an international volunteer army to come to the aid of the Republic. The International Brigades quickly achieved mythical status as the century's most conspicuous example of dedicated idealism, serving the cause of democracy in peril. The early "spontaneous" fighters and, later, the British Battalion in the XVth International Brigade, which included some 2,000 volunteers, fought in every major campaign of the war; about 85 percent of the Battalion's members were killed or wounded. The author is the first scholar to make systematic use of the recently opened archive of the International Brigades in Moscow, enabling him to take the measure of the nobility and tragedy of the British sacrifice in Spain. His study confirms popular mythology about the International Brigades in certain respects and sharply disputes it in others. Above all, Into the Heart of the Fire establishes the fact that the British volunteers were not social or neurotic misfits. Rather, they reflected in a distinctive way the political concerns of many of their generation.

In Britain, in spite of the government's official policy of non-intervention, there was a campaign of fervent support for the legitimate Republican government. The first British Medical Unit in Spain had immense political significance for the Spanish Republic. Barely a month into the start of the civil war, this small group was the first visible sign of international support. It would later become part of the Republican Medical Service and, within that, of the Medical Service of the International Brigades. Not only did volunteers help to create and maintain an emergency medical service, some of the individuals were also responsible for important developments that were of relevance to later military-medical practice and also to the history of medicine in general. Medical personnel generally worked in dreadful conditions, for hours and even days without rest, and with a lack of equipment and provisions of all kinds. They were mostly young and inexperienced men and women who suddenly found themselves thrown together in desperate circumstances with the task of salvaging something of life amidst the inhumanity and mayhem. General Eoin O'Duffy organised 700 Irishmen in a modern Crusade; 500 Catholic Frenchmen fought in the "Jeanne D'Arc" unit; and thirty British volunteers, including aristocrats and working-class fascists, also took up arms. Romanian Iron Guard extremists died at Majadahonda and an Indian volunteer fought in the fascist militia. There were Russians, Americans, Finns, Belgians, Greeks, Cubans, and many more. Goose-stepping alongside the volunteers were fascist conscripts from Germany and Italy, in training for the next world war. Foreigners, whether unknown individuals like British pilot Cecil Bebb or infamous figures like the German dictator Adolf Hitler, were essential to Franco's victory. Without Bebb--who flew General Francisco Franco from the Canary Islands to Spanish Morocco in 1936, a journey which was to precipitate the onset of the Spanish Civil War--the war would never have started; without Hitler, Franco would never have won.
