MRI's contrast between soft tissues in the breast is many times greater than that obtained by plain-film mammography. As opposed to x-rays, which are known to cause damage to cellular DNA, the magnetic fields and radiowaves used with MRI are not known to have any long-term biologic effect. MRI of the breast requires intravenous injection of a contrast agent, which helps highlight breast abnormalities. The American Cancer Society has advised women at high risk for breast cancer to have an MRI. This book, a collaboration between an experienced breast imager and a breast surgeon, contains 100 cases and covers high risk screening, extent of disease evaluation, as well as the full range of benign and malignant tumors found in the breast: DCIS, invasive ductal cancers, and invasive lobule cancers. Rare lesions such as phyllodes, mucinous, liposarcomas, and myotiboblastomas are also covered. Pathologic correlations are included where appropriate. Book excerpt: OBJECTIVE:Patients deemed to be at high risk for developing breast cancer (25% lifetime risk or higher)qualify for high-risk screening with annual mammography and breast magnetic resonanceimaging (MRI). A proportion of these patients undergo short-interval follow-up breast MRI 3-6months following their baseline study. Our objective is to assess the utility of these short-intervalfollow-up breast MRI studies for patients at high risk for breast cancer.METHODS:A total of 330 baseline high-risk screening breast MRI studies performed at our institutionbetween 2013-2017 were retrospectively reviewed. Of these, 70 baseline studies (21%) had arecommendation for short-interval follow-up MRI at 3-6 months [Breast Imaging Reporting andData System u2013 BIRADS 3]. The findings at baseline MRI and the outcome of the short-intervalfollow-up, including any interventions, were recorded.RESULTS/DISCUSSION:Of the 70 studies, 2 were excluded due to lack of follow-up. 68 high-risk patients were included(age range 30-67, mean 42). Patients had short-interval follow-up due to moderate/markedbackground parenchymal enhancement, benign-appearing non-mass enhancement and/or benignappearing lesions. Of the 68 patients, 5 (7%) went on to have a biopsy, 4 of which had benignpathology. One patient had atypical lobular hyperplasia (ALH). The remaining 63 patients (93%)had no suspicious findings on follow-up imaging.CONCLUSION:Short-interval follow-up breast MRI in high-risk patients after a baseline screening study withlikely benign findings is unlikely to yield clinically significant findings. This needs to be furtherconfirmed with evaluation of a larger cohort of patients.

Preliminary results suggest that CEDM is comparable to breast MRI for evaluating the extent and size of lesions and detecting multifocal lesions, and thus has the potential to become a readily available, fast and cost-effective examination. With a focus on the basic imaging principles of CEDM, this book takes a practical approach to breast imaging. Drawing on the editors’ and authors’ practical experience, it guides the reader through the basics of CEDM, making it especially accessible for beginners.
