About 600 B.C., the Indians used other forms of natural polymers such as cotton, horse hair, and leather in repairing wounds. Wound closure procedures using silk sutures, based mostly on polypeptides, are likely to have been practiced during the second century. Surgical application of natural polymers continued to represent the major use of polymers until the twentieth century. Not too long after the development of several major synthetic polymers, their use in biomedical applications has attracted the attention of many re searchers and clinicians. Over the past few years, interest in the biomedical applications of polymers has grown considerably. This has been the result of the inevitable collaborative efforts of in novative materials scientists, engineers and clinicians. The es tablishment of the Society for Biomaterials, in our opinion, cata lyzed the growing interest in the use of polymers for biomedical application. In a major effort to bring team players even closer, a five-day symposium on "Polymers as Biomaterials" was held in Seattle, Washing ton, in March, 1983 as part of the national meeting of the American Chemical Society. The symposium was designed to provide a forum for communicating technical and clinical data to colleagues with a broad spectrum of interest in the biomedical applications of polymers.

Book excerpt: This book represents the first European effort to provide a collection of test descriptions used in evaluation of the compatibility of biomaterials in contact with tissues and blood. The urge to compile this book arose from the fact that it is the properties of the material which ultimatively seem to determine the functional outcome of a medical device, almost regardless of how ingenious the construction of the very device is. The longer the exposure is, the more important these basic properties become. Unfortunately only a small part of the interactive phenomena is fully elucidated and understood. This challenge reflects itself in an effort to cover numerous aspects of testing, beginning with fundamental analysis of the material, continuing with the mechanical properties, the resistance to degradation and the analysis of surface and chemical properties including adsorption patterns of proteins ending with test on cell cultures, ex vivo and in vivo. A number of the tests which are generally accepted as being important are already described as official requirements (primarily Pharmacopeas). These official requirements are not included in order to limit the size of the book. It is the aim of this book to present the tests like a recipe in a uniform way to ease the reader in finding his/her way through the material and to present it as a kind of "cook-book" in an order to provide an easy access to copy the procedures. This has unfortunately not been possible in all circumstances.

The biomaterials which are used in medical or dental prostheses must not only exhibit structural stability and provide the desired function, but they must also perform over extended periods of time in the environment of the body. The latter is a very stringent requirement. The oral and other physiological environments are designed by nature to break down many organic substances. Also of importance is the requirement that materials used in the prosthesis not have a deleterious effect on body tissues. Most foreign (to the body) substances are somewhat toxic to human tissues; in fact, few factors are more limiting in the medical prosthesis field than the biocompatibility problem. Some of these problems and the attempts to solve them are discussed in this volume.

Rehn for example in an early attempt, in 1868 performed a closure of a heart wound after a thoracic puncture. At that time noone thought that this attempt was the beginning of cardiac surgery. The dream was partially realized when the replacement of the heart function by the heart-lungmachine became possible. Immediately following the first operations, cardiac surgeons and cardiologists saw with great enthusiasm that this machine could be an important tool for correcting congenital and non congenital heart diseases. However, the heart-lungmachine was soon seen to not be an optimal instrument for controlling cardiac failure after cardiopulmonary bypass or myocardial infarction. then arose for the development of mechanical assistance to the heart. Concepts The rollerpump was first designed for assisted circulation and could be applied clinically in 1962. A new concept of mechanical assistance to the heart was pro vided by the intra aortic balloon pump and later by the bypass ventricles. Use of the intra aortic balloon pump can reduce cardiac work by counterpulsation in accord with the natural ECG. The bypass ventricle can functionally replace the heart in series or parallel to the natural heart. The intra aortic balloon pump, now an established method, has reduced the number of patients who used to die from cardiac failure.

A structural unit consisting of various groups of elements is called an "element-block." The design and synthesis of new element-blocks, polymerization of these blocks, and development of methods of forming higher-order structures and achieving hierarchical interface control in order to yield the desired functions are expected to result in manifold advantages. These benefits will encourage the creation of new polymeric materials that share, at a high level, electronic, optical, and magnetic properties not achievable with conventional organic polymeric materials as well as forming properties of molding processability and flexible designability that inorganic materials lack. By pioneering innovative synthetic processes that exploit the reactivity of elements and the preparation techniques employed for inorganic element-blocks, the aim is (1) to create a new series of innovative polymers based on the novel concept of element-block polymers, in which the characteristics of elements are extensively combined and utilized, and (2) to formulate theories related to these polymers. This book demonstrates especially the design strategies and the resulting successful examples offering highly functional materials that utilize element-block polymers as a key unit.
