Book excerpt: Partially biodegradable composites were prepared from a biodegradable polyester, reinforced with carbon or ceramic fibers, and evaluated for their potential use in fixing mandibular fractures resulting from maxillofacial injuries. The composites, made using a lamination technique, were found to possess the strength and rigidity of bone. Their incubation in phosphate-buffered saline at 37°C resulted in the gradual reduction of both flexural strength and flexural modulus of the samples. Reductions in strength and rigidity were paralleled by commensurate reductions in the viscosity of the matrix resin. Evaluation of the model system demonstrated the potential for improved management of fracture wounds using biodegradable materials.

The specific goal for the current contract year has been the attainment of high-strength, high-modulus, biodegradable fibers for use in the reinforcement of biodegradable polymeric fracture-fixation plates. In studies conducted during the previous years of the project, composites fabricated form poly(DL-lactide) (DL-PLA) and commercially available, non-biodegradable ceramic or carbon fibers were shown to provide sufficient strength and rigidity for use in the repair of mandibular fractures in dogs. Our work during this period has been directed toward the examination of biodegradable ceramics and glasses for the production of biodegradable fibers for use in the construction of totally absorbable fixation-plate appliances.

Treatment of pediatric mandibular fractures aims to restore function by fracture reduction, proper fixation to ensure union of fracture and reestablishment of normal occlusion. Titanium plates are routinely used for fixation of maxillofacial fractures. However, titanium devices have several potential problems which may indicate removal including hypersensitivity, thermal conductivity, palpability, pain and irritation. In children, metal plates should be removed after bone healing to avoid disturbance of facial growth. This needs a second hospitalization time with increased burden on the health system. Biodegradable plates have become an established alternative to metal fixation in reconstructive and orthognathic craniomaxillofacial surgery. Currently there are different materials of biodegradable devices. The main advantage of biodegradable devices in children is the avoidance of a second surgery to remove the metal plates. We are reporting experience with one type of biodegradable fixation system (self-reinforced poly L/DL lactic acid) in children to evaluate its stability, efficiency and complications.

Book excerpt: Survivable, maxillofacial injuries, such as those arising from military conflicts, often require open or closed reductions that involve the use of metallic plates, screws, and wires. Because these are nonresorbable, internal-fracture-fixation plates made from poly(DL-lactide) and reinforced with calcium metaphosphate (CMP) fibers have been designed to provide initial rigidity and weaken gradually. This exercises the newly formed bone, thereby strengthening it. Also, the need for a second surgery is obviated. Because the biodegradable, composite plates are not readily comformable to closely match the contour of bone and must be cut, drilled, and beveled to the required size and shape, alternative approaches to fracture fixation are desirable. This investigation delved into the feasibility of preparing moldable, moisture- curable poly (DL-lactide)s as matrix for CMP whiskers. Various synthetic schemes were investigated to put isocyanate functionality onto alcohol-terminated prepolymers. At the time of surgery, the curable polymer may be cured by moisture or other curing agents as it is mixed with CMP whiskers. In curing with moisture, foaming can be expected to induce a porosity in the implant. It is concluded from this study that appropriate, biocompatible curing agents to be used with alcohol-terminated prepolymers represent a preferred route to resorbable, fracture fixation materials that can be quickly and simply shaped at the time of surgery.
