Book excerpt: The accomplishment of developing a truly adhesive bond between a restorative material and the natural tooth structures is the goal of adhesive dentistry. Dentine adhesive systems come into close contact with dental and oral tissue, especially the pulp and gingival cells. Due to this close and long-term contact, adhesives should exhibit a high degree of biocompatibility. Biocompatibility is one of the most important properties of dental materials, and adhesives are no exception. It has been long demonstrated that different components of adhesives can be released. Numerous in vitro investigations have shown that released monomers and other components can cause damage to cultured cells. In addition, many in vivo studies have shown that uncured components which reach the pulpal space cause inflammatory response and tissue disorganization. Only a combination of various in vitro and in vivo tests can provide an overview of the interaction of biomaterials with the host. Therefore, it is necessary on a regular basis to carry out and re-verify the biological compatibility of the increasing number of new dental materials. Adhesives should be biofunctional, protective, and preventive, with health-promoting effects that contribute to a better prognosis for restorative treatments and its biocompatibility.

Book excerpt: The evolution of resin-containing materials has resulted in a major advance in restorative dentistry as mercury-containing silver amalgam are used less in the dental practice. However, there is still a debate on the in vitro and in vivo biological properties of dental resins now considered as potential amalgam substitutes. Differing answers between clinical data as opposed to what is known about the tolerability or toxicity of resin-containing materials, creates some benefit/risk interrogations for dental practitioners. In order to reduce the gap between basic science and the clinic, a dialogue has been established between researchers in biomaterials and in biology, and also between researchers and clinicians. A group of well-recognized experts was asked by the Regional European Organization of the Federation Dentaire Internationale to report on the intrinsic properties, the biocompatibility or cytotoxic effects of dental composites. Consequently, the physico-chemical properties of resin based composites and dental adhesives are reviewed in this text. The interaction of resin-containing materials with soft dental and calcified tissues is then studied and adverse effects are reported in a series of in vitro and in vivo studies. In order to reconcile differing evaluations, laboratory data are compared with human clinical data. In addition, a final chapter aims to provide some insights on emerging trends in dental material research. A series of investigations on a new restorative cement are given as examples of future developments. The gap between the bench and the chair side is still open, but answers are now better identified in this book.

Book excerpt: The purpose of this study was to investigate the biocompatibility and micromechanical properties of newly synthesized antibacterial monomer and cross-linker functional surfactants into a commercially available dental adhesive (Single Bond, 3M ESPE, Saint Paul, MN, USA) at three concentrations (0.05, 0.1, 0.25 mg/mg). All groups were analyzed by evaluation of micro-tensile bond strength, ultimate tensile strength, cell viability, antibacterial properties, and surface micro-hardness. Scanning electron microscopy (SEM) was used for interfacial characterization. Human extracted molars were used as a substrate for bonding adhesives for the micro-tensile bond strength (MTBS) and scanning electron microscopy (SEM) studies. Twenty resin-dentin beams (0.9 ± 0.1 mm2) per group were evaluated at 24 hrs and 6 mos for MTBS. Slabs of ~1 mm were analyzed in the SEM for surface characterization. For the ultimate tensile strength (UTS), ten hour-glass shaped specimens (10 x 2 x 1mm) per group were tested at 24 hrs, 1 wk, and 6 mos. To evaluate toxicity, four disc-shaped specimens (5 x 2 mm) per group were incubated with human gingival fibroblasts (HGF). Antibacterial properties were evaluated by incubating three disc-shaped specimens (8 x 1 mm) per group with a strain of caries-producing bacteria S. mutans. Within the limitations of this in vitro study, it was concluded that incorporation of antibacterial monomer and cross-linker additives may be a viable option to help increase the longevity of tooth-colored adhesive restorations. Single Bond adhesive modified with 0.1 mg/mg monomer appears to provide the optimal balance for biocompatibility and micromechanical properties.

However, the lack of a chemical spontaneous bond between dental alloys and PMMA often leads to mechanical failure at the interface causing catastrophic fracture and accumulation of debris in the finish lines. We have recently produced a new adhesive based on diazonium molecules that can be used in dentistry. This new type of adhesive can bind covalently to dental alloys (i.e. Ti, SS, Co-Cr) and enhance their adhesion to dental polymers.Objectives: The objective of this thesis was to assess the biocompatibility and endurance of this adhesive when used to bind PMMA and dental alloys.Methods: polished samples of titanium (Ti), stainless steel (SS) and cobalt chromium (Co-Cr) were coated with the diazonium-based adhesive, untreated samples served as controls. X-ray photoelectron spectroscopy (XPS) was performed to characterize the elemental compositions of the different surfaces. Biocompatibility of the coated alloys was assessed with human gingival fibroblasts (HGF). Inductively-coupled plasma (ICP) and total organic carbon (TOC) were used to quantify the chemicals released from the diazonium-coated alloys. Endurance of the adhesives was assessed by exposing the samples to autoclaving and thermocycling. The tensile strength of the PMMA-alloy bond was assessed using a universal testing machine.
