The literature report a majority of persons who are lesbian, gay, bisexual, and transgender (LGBT) have experienced discrimination from a healthcare provider. This study encouraged nurses to assess their beliefs and behavior when providing health services to lesbian and gay individuals. Such self-assessment can positively impact patient outcomes. Nurses acknowledging sexual orientation and providing a safe environment for the patient and their family improves outcomes by providing patient-centered care.Aim. To assess their beliefs and behavior when providing healthcare services to gay and lesbian individuals using the Gay Affirmative Practice (GAP) scale.Methods. The study involved convenience sampling of registered nurses in a southwest urban hospital system utilizing an eNewsletter with a link to a Qualtrics survey that included a consent statement, demographic questions, and the Gay Affirmative Practice (GAP) Scale. The data was analyzed using descriptive statistics.Findings. Of 70 respondents, 89% were female, 91% Caucasian, 90% heterosexual, and 84% Christian. The average age was 49; 76% had a baccalaureate or master’s degree; 71% had a nursing certification; 60% had a friend or family member who is gay or lesbian; and 61% believed a cultural competence course would benefit them professionally. The mean GAP score was 110 (150 possible); the average belief score was 63 (75 possible); and the average behavior score was 49 (75 possible).Conclusions. Small sample size limited generalizability. Belief scores were higher than behavior scores, likely representing nurses’ positive beliefs and their requirement for education to increase behavior scores. Recommendations include future research with larger sample sizes throughout the nation; development of a tool that has language more reflective of nursing; and promote cultural competence courses with definitions of LGBT terminology and how to ask sexual orientation- and gender identity- questions in a safe and optimal manner.

Book excerpt: APRNs are increasingly providing more of the primary care services to diverse populations in the United States. The population includes persons of non-heterosexual identities who often encounter barriers when seeking health care. Given the minuscule focus in nursing education about these patient populations, a clearer understanding of APRNs’ beliefs, behaviors, and experiences caring for lesbian and gay patients was essential. The purpose of this mixed methods study was to explore and identify the beliefs, behaviors, and experiences of APRNs with lesbian and gay and patients. Social constructionism was the theoretical perspective that guided this study, and the philosophical perspective of pragmatism informed the methodology. The study followed a convergent parallel design informed by Creswell and Plano Clark (2011). A sample of 678 APRNs from a northeastern state completed an on-line survey comprised of the 30-item, Gay Affirmative Practice (GAP) (Crisp, 2002) Likert-type scale, 13 demographic items and an open-ended statement requesting a description of experiences of having cared for lesbian and gay patients. Quantitative data were analyzed using descriptive statistics and ANOVA. Qualitative data were analyzed using Krippendorff’s (2013) content analytic technique of clustering units to form themes. ANOVA indicated statistically significant differences in GAP scale scores based on personal identity, having a lesbian or gay family member, political party, practice place, practice focus, and the number of lesbian and gay patients. Eight thematic categories emerged from APRN experiences: affirming, more education needed, witnessed discrimination, limited experience with lesbian/gay patients, sexual orientation only asked if relevant, treat all the same, non-affirming, and sexual orientation not focus of practice. The thematic categories of affirming, have witnessed discrimination, and more education needed consistently had the highest GAP scale scores compared to the other themes.

Book excerpt: Literature suggests there is a relationship between cultural competency and health outcomes as well as a perceived fear of discrimination LGBT individuals face when attempting to receive healthcare services. The aim of this study is to assess the level of LGBT CC (cultural competency) among prospective professionals who are expected to provide health care services and to explore factors that affect those attributes. Multiple linear regressions were conducted to test the effect of the following factors of LGBT CC: postsecondary experience, relations with LGBT individuals, and religion using a sample of 57 different healthcare students in a faith-based university during the spring of 2019. Although postsecondary experience did not have a significant effect, other findings within the disciplines raised awareness for this study. However, having a close relationship with a LGBT individual was found to be significant for both LGBT CC-belief and behavior, and religiosity was found to have a negative effect towards LGBT CC-behavior. The implication of the findings is not to disregard the postsecondary experience, but to point out the differences among disciplines. Knowing that the LGBT population is facing discrimination within healthcare settings makes it imperative for healthcare university programs and healthcare facilities to educate students and employees to be more culturally competent when working with LGBT clients.

Bachelor of Science in Nursing (BSN) students are at a high risk of making assumptions based on personal stigma, stereotypes, education, low exposure, prejudice, and religious beliefs toward the lesbian, gay, bisexual and transgender (LGBT) population, thus compromising the quality of health care provided. The purpose of this descriptive, correlational and exploratory research was to report the presence of homophobia in undergraduate nursing students. Methods. Data was collected from a convenience sampling of traditional and accelerated Bachelor of Science in Nursing students enrolled at a California State University in the Spring of 2016. Quantitative data was collected from a purposive sample of 256 nursing students using a demographic profile sheet, the Index of Attitudes Towards Homosexuals and Homophobic Behavior of Student Scale. Results. The cutting score for higher level of homophobia was set at 50 by the authors of the survey instruments. The result revealed lower level of homophobia among the BSN students. The mean IAH score (n= 256, M = 24.421 SD =15.91) and the mean HBSS score (n= 256, M = 12.37, SD = 15.29) were both lower than 50 suggesting lower-level of homophobia. There was a strong positive correlation between the IAH scores and HBSS scores of BSN students toward homosexuals, r = .682. These results suggested that the students’ behavior were consistent with their attitudes. Non-accepting religion to homosexuality accounted for strongest predictor for the homophobic scores. Conclusion: The results of this research revealed that nurses have lower level of homophobia. Future qualitative research should be built on this study to further investigate why nursing students continue to have homophobic attitudes and behaviors. The nursing faculty and students should continue to participate in cultural or diversity class to achieve neutrality regardless of their religious belief to truly advocate for patient’s health.
