It is a commerce whose impact upon humanity rivals that of any other business--millions of lives have been saved by blood and its various derivatives, and tens of thousands of lives have been lost. Douglas Starr tells how this came to be, in a sweeping history that ranges through the centuries. With the dawn of science, blood came to be seen as a component of human anatomy, capable of being isolated, studied, used. Starr describes the first documented transfusion: In the seventeenth century, one of Louis XIV's court physicians transfers the blood of a calf into a madman to "cure" him. At the turn of the twentieth century a young researcher in Vienna identifies the basic blood groups, taking the first step toward successful transfusion. Then a New York doctor finds a way to stop blood from clotting, thereby making all transfusion possible. In the 1930s, a Russian physician, in grisly improvisation, successfully uses cadaver blood to help living patients--and realizes that blood can be stored. The first blood bank is soon operating in Chicago. During World War II, researchers, driven by battlefield needs, break down blood into usable components that are more easily stored and transported. This "fractionation" process--accomplished by a Harvard team--produces a host of pharmaceuticals, setting the stage for the global marketplace to come. Plasma, precisely because it can be made into long-lasting drugs, is shipped and traded for profit; today it is a $5 billion business. The author recounts the tragic spread of AIDS through the distribution of contaminated blood products, and describes why and how related scandals have erupted around the world. Finally, he looks at the latest attempts to make artificial blood. Stroup challenges the notion that political activism has gone beyond borders and created a global or transnational civil society. Instead, at the most globally active, purportedly cosmopolitan groups in the world-international nongovernmental organizations (INGOs)-organizational practices are deeply tied to national environments, creating great diversity in the way these groups organize themselves, engage in advocacy, and deliver services. Stroup offers detailed profiles of these "varieties of activism" in the United States, Britain, and France. These three countries are the most popular bases for INGOs, but each provides a very different environment for charitable organizations due to differences in legal regulations, political opportunities, resources, and patterns of social networks. Stroup's comparisons of leading American, British, and French INGOs-Care, Oxfam, Médicins sans Frontières, Human Rights Watch, Amnesty International, and FIDH-reveal strong national patterns in INGO practices, including advocacy, fund-raising, and professionalization. These differences are quite pronounced among INGOs in the humanitarian relief sector, and are observable, though less marked, among human rights INGOs. Stroup finds that national origin helps account for variation in the "transnational advocacy networks" that have received so much attention in international relations. For practitioners, national origin offers an alternative explanation for the frequently lamented failures of INGOs in the field: INGOs are not inherently dysfunctional, but instead remain disconnected because of their strong roots in very different national environments.

Book excerpt: Since its birth with the creation of the international Red Cross in 1863, international humanitarian assistance has developed considerably since World War II. In accordance with the Red Cross principle of humanity, it aims at preventing and alleviating human suffering wherever it may be found, protecting life and health and ensuring respect for the human being. International humanitarian assistance involves a complex network of government agencies, intergovernmental and non-governmental organizations, and individual volunteers: it has been labelled a `non-system'. While governments and intergovernmental organizations play a dominant and structured role in this field, the non-governmental organizations and their volunteers have proved to be their necessary operational partners, providing material, medical and moral relief and care wherever it may be needed, beyond borders, at the grassroots level. Following a brief review of recent humanitarian activities of intergovernmental organizations, and an analysis of current trends of voluntarism, this book focuses on the role, status and attitudes of the major humanitarian non-governmental organizations, including the Red Cross organizations, the British charities, Church-related agencies, medical volunteers (such as the `French Doctors') and U.N. volunteers. Should humanitarian non-governmental organizations provide relief assistance with the Red Cross concern for discretion, neutrality and impartiality? Or should they bear witness and denounce publicly human rights violations, at the risk of being expelled from recipient countries and having to stop their assistance? The controversial claim of a `right' to receive and a `duty' to provide humanitarian assistance beyond borders is also addressed, as well as the possible need for a status to be accorded to international volunteers.

The project addressed young people and mentors and helped them understand the importance of non-formal and informal education, supported by volunteering, in preventing early school leaving. In this way, the project team put special attention on recognising the key competences that young people acquire through their voluntary work. The initial goal of the project was to test ideas and concepts at the local levels – to see which approaches are the most appropriate in a certain country, socio-cultural framework and even on a micro-level of schools and other institutions involved in the project. The last several decades show considerable growth in the numbers of school volunteers, with increasing participation by people from the business community, retired citizens, and college students, in addition to the traditionally active mothers of school children. Yet, we must do more to ensure the success of volunteer programs in schools. It is not enough to assume that any volunteer effort will produce results, nor is it wise to use volunteers simply because they are "free."
