Book excerpt: L'augmentation des exigences esthético-fonctionnelles en implantologie orale ces dernières années, a mis en évidence les limites des fixtures de diamètre standard. En effet, des concepts biomécaniques de profil d'émergence et de stabilité prothétique ont justifié l'apparition de nouveaux diamètres implantaires. Ainsi sont apparus des implants " larges " (5 mm et plus) et des implants de petit diamètre (3,3 mm et moins). L'analyse de la littérature met en évidence: - pour les implants de petit diamètre, des taux de succès identiques et comparables à ceux obtenus avec les fixtures standards. - pour les implants larges, des taux de succès sensiblement inférieurs, pouvant s'expliquer par leur utilisation, quasi systématique, comme implants " secours ", c'est-à-dire, dans des situations cliniques défavorables (sites osseux quantitativement et/ou qualitativement à risques, patients ayant eu des antécédents d'échecs implantaires...). Cette approche thérapeutique de l'implant large est certes motivée par une aptitude à obtenir une plus grande stabilité primaire mais biaise son évaluation. D'ailleurs, une étude récente (Krennmair et Coll, 2004) excluant les sites à volume osseux réduit (hauteur implantable inférieure à 10mm) rapporte des taux de succès très élevés et comparables à ceux obtenus avec les implants d'un diamètre standard. D'autres études de ce type sont nécessaires afin de confirmer ces données.
