Part of this new recognition stems from the problem's prevalence: the number of homeless individuals, while hard to pin down exactly, is rising. In light of this, Congress asked the Institute of Medicine to find out whether existing health care programs were ignoring the homeless or delivering care to them inefficiently. This book is the report prepared by a committee of experts who examined these problems through visits to city slums and impoverished rural areas, and through an analysis of papers written by leading scholars in the field.

Book excerpt: The study investigated responses made by adults in the City of Rochester and Olmsted County, Minnesota to a Community Shelter Plan (CSP) and to OCD booklet H-14, 'In Time of Emergency.' A total of 493 interviews were completed during the study. This included independent control samples before and after CSP as well as a panel of 97 persons interviewed at both time periods. The CSP plan allocated public fallout shelters to residents based on their place of residence. Attention was adequate. Seventy-four percent were aware of receiving CSP materials and half read some of either the CSP maps or booklet H-14. Comprehension was less adequate, however. Only half the readers (24 percent of the total population) could correctly identify the shelter which the plan assigned them. Attitudes formed toward the CSP program were extremely favorable, but little change in behavior resulted. A slight increase was observed in attitudes toward shelters. No net change in adoption of either home or public shelters was found although a number of individuals switched their positions. (Author).

Book excerpt: The study investigated responses made by adults in Des Moines, Iowa, to the Community Shelter Plan (CSP). A total of 938 interviews were completed during the study. This included adequate control samples before and after publication of CSP as well as panels. One panel was initiated before CSP, other were continued from the researchers' earlier Home Fallout Protection Survey study. The CSP message included a plan allocating public fallout shelters to residents based on place of residence. It was published in the form of a 12-page Sunday newspaper advertising supplement. Attention given the message was about average with similar supplements (slightly less than half of the total audience), but comprehension of the plan was low. Only seven percent of our control sample could recall the name or location of a shelter allocated for his use and only four percent could recall how they were instructed to travel to the shelter. Acceptance responses were low. A slight increase was observed in level of knowledge about a local civil defense program and in the anticipatory decision to concurrently adopt public or home shelter protection alternatives. The dispositional model did not hold well in predicting attention to CSP, but did hold in predicting comprehension and acceptance responses. (Author).

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.
