Book excerpt: It is only recently that the use of the endoscope as the sole visualizing tool has been introduced in transsphenoidal pituitary surgery with its favorable related implications and minimal operative trauma. Of course, microscopic and endoscopic anatomy are basically the same, but the optical distorsion of endoscopic images is quite substantial compared to microscopic depictions. An endoscope lens produces images with maximal magnification at its center and severe contraction at its periphery. Nearer images are disproportionally enlarged and remote images are falsely miniaturized. This optical illusion may disorientate a surgeon who is not familiar with this peculiar condition at the skull base. This atlas acts as a guide through the endoscopic anatomy and gives detailed descriptions of the preoperative management and the surgical procedures.

Because of this, medicine—grounded in the unforgiving realities of the scientific method and peer review, and of flesh, blood, and pain—can sometimes confuse “design” with mere “prettifying.” Design solves real problems, however. This collection of papers underwrites the importance of design for the MMVR community, within three different environments: in vivo, in vitro and in silico. in vivo: we design machines to explore our living bodies. Imaging devices, robots, and sensors move constantly inward, operating within smaller dimensions: system, organ, cell, DNA. in vitro: Using test tubes and Petri dishes, we isolate in vivo to better manipulate and measure biological conditions and reactions. in silico: We step out of the controlled in vitro environment and into a virtual reality. The silica mini-worlds of test tubes and Petri dishes are translated into mini-worlds contained within silicon chips. The future of medicine remains within all three environments: in vivo, in vitro, and in silico. Design is what makes these pieces fit together—the biological, the informational, the physical/material—into something new and more useful.

The scientific goal of our workshop, “Computer Vision for Biomedical Image Applications: Current Techniques and Future Trends” (CVBIA), is to examine the diverse applications of computer vision to biomedical image applications, considering both current methods and promising new trends. An additional goal is to provide the opportunity for direct interactions between (1) prominent senior researchers and young scientists, including students, postdoctoral associates and junior faculty; (2) local researchers and international leaders in biomedical image analysis; and (3) computer scientists and medical practitioners. Our CVBIA workshop had two novel characteristics: each contributed paper was authored primarily by a young scientist, and the workshop attracted an unusually large number of well-respected invited speakers (and their papers). We had the good fortune of having Dr. Ayache of INRIA, France to talk about “Computational Anatomy and Computational Physiology,” Prof. Grimson of MIT to discuss “Analyzing Anatomical Structures: Leveraging Multiple Sources of Knowledge,” Dr. Jiang of the Chinese Academy of Sciences to present their work on “Computational Neuroanatomy and Brain Connectivity,” Prof.
