Book excerpt: Excerpt from Atlas and Essentials of Gynecology This atlas was undertaken with the idea that, although there are many excellent text-books, compendiums, and atlases on gynecology, there is no book which gives the student and physician the material which personal clinical observation should supply. If such a book were purely schematic, it would perhaps meet the requirements of many readers, but the actual state of affairs is not thus properly represented; while, on the other hand, an exact reproduction of anatomical preparations makes the distinction between the essential and incidental conditions difficult. I determined, therefore, to combine the two methods, and in addition to drawings from nature to emphasize any variation from the normal. I have further sought to present the various subjects from as many points of view as possible; that is, to give the etiology, development, secondary complications, termination, or recovery; and have therefore added schematic and partly schematic drawings to the pictures of anatomical specimens. Olshausen secures the insertions of the round ligaments to the angles of the peritoneal wound and to the sheaths of the recti (Plate 42), while Czerny and Leopold pass the sutures through the fundus itself. The same sutures that are used to close the abdominal wall are passed through the fundus. Catgut may also be used. X. ALEXANDER-ADAMS OPERATION, PERFORMED AFTER EXPOSURE OF THE INGUINAL CANAL. Topographic and Surgical Anatomy of the Inguinal Region. The skin is divided immediately above Poupart's ligament and in a direction parallel with it, then the panniculus adiposus, and, underneath that, the superficial abdominal fascia, in which, midway between the symphysis and the anterior superior spine of the ilium, the epigastric vessels will be found. The superficial fascia is divided only by a thin layer of fat from the much thicker aponeurosis of the external oblique. The latter covers and forms the inguinal canal, the internal opening of which is situated at a point somewhat lateral to the epigastric vessels. The external inguinal ring is situated a short distance to the outer side of the pubic tubercle, which can be readily palpated. It is formed by the aponeurosis of the external oblique (intercrural fibers), which here undergo division and form the radiating crura ligamenii inguinalis superius et inferius. The opening is covered by the thin cremasteric fascia. Between the limbs of the inguinal ring the round ligament emerges with the ilioinguinal nerve. Book excerpt: Excerpt from Marin's Atlas of Obstetrics and Gynaecology The numerous illustrated works on Midwifery, deal, with few exceptions, besides illustrations of the pelvis and the female genital organs in the normal state, almost exclusively with the development of the ovum and the uterus during pregnancy, the positions of the foetus, the obstetric instruments, and the operative manoeuvres required in Obstetrics. Not only are the illustrations of the foetal positions, and of the obstetric operations most frequently diagrammatic, and obtained by placing the dummy in various positions in a phantom or pelvis, but the works themselves afford an insufficient insight into those conditions necessary to complete the knowledge of the gynoecologist. Midwifery must be considered in its natural relation to the conditions of the other female sexual functions, and to the various physiological and pathological changes which occur in the sexual organs, and consequently in its relation to gynoecology. Thus the scientific methods of study require it, and rebel more and more against the continued reign of the obstetric method. The practice which had long ago forced itself upon the obstetrician, also required a knowledge of the so-called diseases of women. On these grounds, then, that the pathological and physiological conditions of the sexual organs should be represented in the various phases of their functions, the demand of the publishers for an atlas of obstetrics was very pleasing to me, especially as I had often felt the want of an illustrated description of these subjects in my annual course of lectures since 1838.The exhibition of enlarged illustrations or of preparations during the lecture, is useful and even indispensable, but it is insufficient. A mere passing glance at a diagram during a lecture leaves behind no fixed impression. And so I had long been anxious to place in the hands of my hearers a collection of drawings to illustrate my lectures. But such a collection of illustrations is also of great value to the practising physician, who does not see gynoecological cases every day, and who seldom has the opportunity to study the organic changes seen at post-mortem examinations. One look at an exact and faithful drawing, presents the anatomical condition of the parts more dearly to his mind than far-fetched deductions for the elaboration of which he frequently has no time. It moreover starts fresh ideas for future diagnosis and treatment of doubtful individual cases. The execution, of the undertaking, to present to students and practitioners an illustrated guide to obstetrics and gynoecology, was necessarily restricted to enable the work to be placed within the reach of a sufficiently large circle of readers. Though all its problems are, in truth, not yet solved; though many differences of opinion still exist; we have, nevertheless, a firm foundation of essentials in fact; concepts which demand and obtain universal acceptance. Since the induction by Olshausen of curetment, or abrasio mucosae, as a gynecologic operation, new and, in fact, hitherto entirely unsuspected paths have opened up in diagnosis and therapy. Scraping away the mucous membrane of the uterus permits a simple, sure recognition, comjiaratively without danger, of the structural composition of the inner lining of this organ. Diagnosis of the benign or malignant character of mucosal alteration is determinable; a cancer is early cognized; hemorrhage, deviating from normal occurrences, is etiologically referred to an endometritis or established as an obstetric phenomenon and sequela. Still, interpretation of the microscopic data afforded by curetage was not got in a day; only after years of tireless striving, chiefly under the leadership of Carl Ruge, and sequent to the determination and establishment of certain typical forms, was it possible to define more sharply the often vague lines of differentiation among the individual conditions and gather together data of the greatest import.
