Book excerpt: This book represents the proceedings from a conference that took place in Dallas in the spring of 1999 which was entitled "Pediatric Gender Assignment - A Critical Reappraisal". Some participants rightfully argued that the conference really focused on the issue of pediatric gender assignment, and that reassignment was not applied in most cases. Their comments were reflected in the title of this monograph. This multidisciplinary meeting was sponsored by a conference grant from the National Institutes of Health, and a broad inquiry into this complex topic took place from many points of view. Basic scientists offered insight into mechanisms of sexual differentiation of the gonads, physical phenotype and imprinting of the central nervous system. Endocrinologists reviewed their experience in diagnosis and management, surgeons described traditional as well as innovative approaches, and there was strong representation from the ethical and behavioral sciences. In putting together such a panel, it was essential that we identify a cast of speakers who could address their viewpoints with strong convictions, and yet not let their passions render the meeting counter productive. We were not disappointed. While many differing points of view were firmly expressed by the panelists and audience, all viewpoints were accorded the respect they deserved. The concept behind the meeting and this book really originated in 1997 shortly after Diamond and Sigmundson published their long term follow up study of the John/Joan case.

Book excerpt: The principal goal of the dissertation is to explain the political nature and effect of cultural characterizations on the development of student assignment policy. Cultural characterizations are socially constructed portrayals that become influential when stakeholders mobilize to bring about change. In education, as the professional authority of school boards and superintendents diminishes, community stakeholders are increasingly prominent. They serve as critical producers and providers of cultural characterizations of public education and its beneficiaries. As such, the engagement of community stakeholders with public sector institutions, organizations, and individuals can significantly amplify, modify, or blunt education policy. The dissertation traces the history of community mobilization in San Francisco from 1971 to 2005, during which the federal district court supervised all aspects of the school district's student assignment policy. Cultural characterizations of student assignment were structured by three distinct logics of action: integration, choice, and neighborhood. These logics were stable but not fixed. Changes in the institutional environment coupled with how stakeholders framed, understood, and shaped these logics led to transformations in student assignment policy that ultimately altered the educational experience of multiple generations of public school students. Data are drawn primarily from archival documents from the federal district court, the school district, and community organizations; mainstream and community newspaper articles, letters to the editor, and editorials; and, retrospective interviews with key stakeholders.

SM presents a significant mental and public health problem due to impact on the social, emotional, and academic functioning of young children at a critical point in their development. SM is closely related to childhood social phobia, but it cannot be treated in the same way because of the young age of the children affected, their lack of speech in the treatment setting, and the need for significant school involvement in treatment. Treatment for Children with Selective Mutism outlines the sequence and essential elements to guide clinicians through a comprehensive, integrated program for young children who display symptoms of SM. This approach utilizes behavioral interventions targeting gradual increases in speaking across settings in which the child initially has difficulty. The integrated nature of the therapy refers to the goal of incorporating input from the clinician with that from the parents and teacher, as well as others impacted by the lack of speech. Exposure exercises are based on behavioral techniques such as stimulus fading, shaping, and systematic desensitization that also allow for a less intense or gradual exposure to the speaking situation. These techniques are combined and used flexibly with a behavioral reward system for participation in treatment. The approach was developed by Dr. R. Lindsey Bergman as part of the UCLA Childhood OCD, Anxiety, and Tic Disorders Program. The treatment protocol consists of 20 sessions, 60 minutes each, delivered over the course of 24 weeks.
