Book excerpt: Human Papillomavirus (HPV) infection has been a rising concern due to its association with cervical cancer, but many parents are still not getting their children vaccinated to prevent this type of cancer. The purpose of the proposed research is to increase the uptake of the HPV vaccine by identifying the barriers preventing adolescents from receiving this vaccination, and increasing the literacy of parents/guardians about HPV and the HPV vaccine. To identify the aforementioned barriers, a questionnaire will be given to participating parents/guardians of children/adolescents visiting the Department of Pediatrics at UNT Health Science Center. This questionnaire was designed to aid in the assessment and discovery of the parent's/guardians' knowledge regarding HPV and the HPV vaccine, potential barriers to accepting the HPV vaccine for their children, and their attitudes towards the HPV vaccine and vaccination in general. Materials for this clinical research study have been completed, and IRB approval has been obtained, so data collection can begin. After completion of this research study, we expect to not only increase parents/guardians literacy regarding HPV and the HPV vaccine, but also to identify the major barriers preventing them from getting their child vaccinated. Information gained will provide a basis for developing a new health literacy intervention within the Department of Pediatrics at UNT Health Science Center.

Book excerpt: Introduction: Human papilloma virus, HPV, is a sexually transmitted infection that can cause genital wards or even cancer. Though the HPV vaccine has been approved for many years, vaccine rates are extremely low, especially in the male population. This review of literature was conducted to examine the factors that influence parents' decision-making processes regarding Human Papilloma Virus (HPV) vaccination for their adolescent male children, in order to improve HPV vaccination rates. Methods: The ProQuest Nursing & Allied Health Database and CINAHL database were utilized within this review of literature. There were 21 studies included in the bib matrix. Results: Four main themes emerged: knowledge, beliefs, cost, and demographic influence. Many parents/guardians are unaware that the vaccine was intended for males. Beliefs surrounding the effect of the vaccine on sexual behavior, low perceived risk of HPV infection, and social interactions also pose a barrier. Cost is a potential barrier that was mentioned throughout many studies. Lastly, HPV vaccination decisions were influenced by the demographic grouping of participants Discussion: The four themes (knowledge, beliefs, cost, and demographic influence) were explored. The Health Belief was utilized in attempt to explain the parents' health behavior of deciding whether or not to have their child receive the HPV vaccine. Conclusion: Overall, parents continue to be reluctant to vaccinate their children against HPV. Most parents lack knowledge about the vaccine and its immense benefits to their child. The Health Belief Model was utilized in order to analyze the parents' decision process and suggest nursing interventions to improve HPV vaccination rates. - Abstract

Book excerpt: This dissertation examines vaccination rates for Human Papillomavirus (HPV) among college women 18-26 years of age who participated in the American College Health Association's National College Assessment (ACHA-NCHA). Utilizing secondary data, this research sought to report HPV vaccination rates among a racially diverse population and to identify potential barriers to vaccination. The ACHA-NCHA survey provided a large sample size (N=68,193) in which to perform a binary logistic regression analysis. Demographic characteristics were analyzed as potential barriers to HPV vaccination. Additionally, lack of certain health behaviors were explored as potential barriers to HPV vaccination. In this study, White/non-Hispanic women had a higher HPV vaccination rate when compared to minority women. The binary regression analysis demonstrated that minority women were less likely to receive the HPV vaccine. Furthermore, it was determined that as the age of the respondents increased, the likelihood of receiving the vaccine decreased. Health behaviors that were predictive of receiving the HPV vaccine included receiving the Hepatitis B and Influenza vaccine, number of sexual partners and receiving sexually transmitted disease information. Women who received a gynecological exam were almost twice as likely to receive the vaccine, as were women who had parental health insurance coverage. One aim of The Affordable Care Act (2010) is to decrease disparities in health care. Drawing attention to potential barriers to HPV vaccination allows policy makers to make informed decisions regarding future activities to reduce disparities. Health promotion activities should be targeted to specific populations in an effort to increase HPV vaccination rates.

Book excerpt: The purpose of this dissertation was to learn more about the communication barriers and strategies reported by parents of pediatric patients and healthcare providers regarding vaccination against Human Papillomavirus (HPV). Participants in this study were parents of pediatric patients and pediatric healthcare providers at two health facilities in a rural, southern, religious community. My research questions sought to understand the communication barriers and communication strategies that could impact vaccine decision-making, not the decision-making process. I held preliminary informal interviews with providers; conducted in-depth, semi-structured interviews with parents and providers; and took extensive field notes. The data presented five types of communication barriers: comfort with the discourse, education, perceived risk, religion, and vaccine acceptance. Three communication strategies were found: collaborating, framing, and relationship building. My recommendations include the following: the relationship between parent and provider should be enhanced; both parties should listen intently; providers should work to understand the communication barriers presented by parents; and providers should develop frames, specifically stories used to explain and elicit vaccination.

Book excerpt: Background: Asian Indians are considered the third largest Asian community in the United States numbering 3.2 million per records from the U.S. census bureau for the year 2010, but little is known about Asian Indian parents' acceptance of the HPV vaccine for their children. Human Papillomavirus (HPV) is the most common sexually transmitted disease in the United States. Even though HPV vaccination is highly effective in preventing HPV infection, many studies have proposed that the vaccination rates in general are low. According to the teen vaccination coverage report by Center for Disease Control in 2014, only 37.6% of girls and 13.9% of boys between the age group of 13 - 17 years received all three doses of HPV vaccination. The key determinant of HPV vaccination rates is parental acceptance. The major factors affecting parental acceptance of HPV vaccination include health beliefs, educational level, religion, child's gender, subjective norms, (peer, family, and social pressure), and personal experience of the disease. While there are several studies in the U.S that have examined parental acceptance of HPV vaccination in general, to date there were no studies specifically focusing on Asian Indian parents living in the U.S, nor any that had assessed the effects of acculturation (cultural identity) on HPV acceptance in the commonly available databases. Therefore, because the factors affecting HPV vaccination acceptance of Asian Indian parents are unknown, the purpose of this study was to determine the effects of health beliefs (perceived seriousness, perceived susceptibility, perceived barriers, perceived benefits) and acculturation (cultural identity) and to explore the effects of educational level, religion, child's gender, subjective norms, and personal experience of the disease on Asian Indian parental acceptance of the HPV vaccine. Methods: A comparative descriptive cross-sectional design based on a theoretical framework of Health Belief Model was used for the study. The sample, Asian Indian parents, who have children between the ages of 9-16 years, were recruited from various places of worship and community organizations from the Houston Metropolitan area and all over the U.S. Participants were recruited directly, with the help of formal and informal leaders of these organizations and through email. Participants were provided with a web-link for the research survey to assess the effect of health beliefs, acculturation, and demographic factors on HPV vaccine acceptance. As established by previous research on non-Asian Indian parents, the acceptance of at least one dose of HPV vaccination was expected to be 45% and the acceptance of Hepatitis B vaccination to be 90%. Using the above estimates, a sample size of 160 participants was deemed sufficient to achieve 81% power to detect a difference between group proportions of 0.45 with a significance level (alpha) of 0.05 using a two-sided two dependent group McNemar test. The same sample size is sufficient for hierarchical logistic regression analysis to achieve 81% power, 0.05 alpha, and a 1.65 Odds Ratio. The expected survey response rate was about 50%, based on previous research conducted to examine variations in response rates to email surveys. Data Analysis: The obtained data was directly entered into SPSS and then cleaned, coded, and checked for shape of distribution and outliers, and then analyzed using descriptive statistics. The rates of HPV vaccination acceptance was compared to Hepatitis B vaccination acceptance using a two-sided two dependent group McNemar Chi square test. The effect of health beliefs and acculturation on vaccination acceptance was carried out using hierarchical logistic regression models. Educational level, religion, and child's gender were added as covariates in the first step; subjective norms, and personal experience of the disease were added as co-variates in the second step; and health beliefs and acculturation were added in the third step of the logistic regression model. Abstract: Human papillomavirus (HPV), a family of viruses with more than 40 genotypes is known to infect genital tract of males and females. High-risk HPV including genotypes 16, 18 can cause cervical cancer while low-risk HPV including genotypes 6, 11 can cause benign or low grade cervical lesions and genital warts. Considering HPV infection is the most common sexually transmitted infection worldwide, as well as in Macao, with prevalence up to 8.1%, and given that cervical cancer is the top ten most frequent female cancer which causes high burden in our health care system, it is important to have a comprehensive intervention for control HPV and cervical cancer. Since the HPV vaccine for preventing cervical cancer has been available in Macao, it is worth to know the willingness to be vaccinated among target population. Base on findings from previous studies, the parental attitudes and acceptance is the key factor of vaccination for young girls. Other potential factors, such as the HPV knowledge, vaccine safety and efficacy, cost, the age of daughter and the physician's attitudes, may also affect parental acceptance. Given that no previous study on this topic has been conducted, this study is aimed to understand the knowledge, attitudes and acceptability of HPV vaccination, and to explore and identify the affecting factors for acceptance among school girls and their parents. A school-based simple cluster random sampling method was used. Data was collected by anonymous self-administrated questionnaires among 574 school girls aged at 12-22 and 702 parents aged at 30-65. About 47% of the school girls had heard of HPV and 88.3% had heard of HPV vaccine, while 57.1% of parents had heard of HPV and 83.2% had heard of HPV vaccine. Although the HPV-related knowledge was poor among young girls and their parents, 92% of girls indicated a positive intention to be vaccinated for herself and 88% parents would willing to consent daughter to be vaccinated if the HPV vaccine became in routine immunization. However, the parental vaccination acceptability for daughters decreased to 40% and only 30% of mothers would willing to accept for themselves, if they had to pay a full course for three doses of HPV vaccine. About 97% of school girls and 95% of parents supported that HPV vaccine should be included in government immunization programme. We also found that young girls and parents are more favor in later HPV vaccination rather than vaccination at recommended age. Cost is the dominant affecting factor in determining parental acceptability of HPV vaccination. In addition, daughter's age, vaccine safety concern, Pap smear attendance and HPV knowledge are significantly associated with HPV vaccine acceptance. It may be worthwhile to further investigate the reasons for the delay vaccination among the young adolescent girls and among their parents, to examine if any underlying factors were unexpressed. Book excerpt: Health literacy includes the ability to understand and process written and spoken health information, and numbers and calculations related to health information. Low health literacy is associated with negative health outcomes, and poor patient-provider communication. In order to advance health literacy research, there is a need to assess health literacy comprehensively and to develop an understanding of how health literacy impacts people at various stages of their lives. A sequential explanatory mixed methods study was conducted to assess college students' health literacy. During the quantitative phase, the Short Test of Functional Health Literacy in Adults (S-TOFHLA), the Newest Vital Sign (NVS), the Cancer Message Literacy Test-Listening (CMLT-Listening), and the e-Health Literacy Scale (eHEALS) were used to assess the students' print literacy, health-related numeracy, aural cancer literacy, and eHealth literacy, respectively. During the qualitative phase, in-depth interviews were conducted with a subset of the original participants to obtain further evidence supporting the results of the literacy assessments and to identify factors in their social cultural environment that influence their health-related decisions including uptake of the Human Papillomavirus (HPV) vaccine. During the quantitative phase, 160 students completed four health literacy assessments. The mean scores on the health literacy assessment were: S-TOFHLA: 35.2; CMLT-Listening: 82%; eHEALS: 80.9%; and NVS: 5.1. The scores of 3 of the 4 health literacy assessments were not correlated with their uptake of the HPV vaccine. Twenty students participated in in-depth interviews during the qualitative phase. The students' scores on the quantitative health literacy assessments indicated that they had adequate health literacy. However, the qualitative data showed that the students' cancer literacy, eHealth literacy, and numeracy skills were not as adequate as their health literacy test scores indicated. Furthermore, the qualitative data revealed that the majority of the students who had received the HPV vaccine had done so as the result of a recommendation from a healthcare provider or their parents. Therefore, it did not appear that they made an autonomous decision to get the HPV vaccine. The results of this study indicate that assessing health literacy using a one-dimensional approach may not accurately reflect college students' level of health literacy. In addition, students tend to rely on their aural literacy and eHealth literacy skills when seeking health information and making healthcare decisions. Therefore, there is a need for the development and implementation of intervention aimed at improving these skills among college students.

Book excerpt: Purpose: To identify barriers to implementing practice recommendations regarding HPV (human papillomavirus) vaccination for male adolescent patients among Family Nurse Practitioners (FNPs). Rationale/Background: HPV infection is a source of numerous cancers. More than one-quarter of the HPV-associated cancers in the United States occur in males. The quadrivalent vaccine is approved in young males and is effective in the prevention of genital warts and reducing HPV related cancers yet vaccination rates are low and expected to remain low. Barriers to vaccination exist even after the 2011 recommendation for routine use. Method: Quantitative, surveys. A 22-item instrument was administered to FNPs working in primary care settings. Participants were surveyed regarding financial, logistic, provider, and parental barriers to vaccination among adolescent males. Results: Descriptive analysis at both the item and scale level demonstrated that FNPs report financial barriers as the most significant barrier. The barriers of least concern were provider attitudes. Barriers regarding FNPs' perception of parental attitudes were seen as moderately concerning. Independent samples t-tests showed that FNPs who did not administer the HPV vaccine to male adolescent patients reported having significantly more financing barriers as compared to FNPs who did. Conclusion: Results suggested that financial issues may constrain FNPs' implementation of practice recommendations for the HPV vaccine and that FNPs who did not administer the HPV vaccination to adolescent male patients may be unable to do so due to financial reasons. Perceptions of parental attitudes are also seen as playing a role in preventing male adolescent patients from receiving the HPV vaccine. Efforts to reduce barriers to implementing recommended HPV vaccine practices should focus on lessening the expense of the vaccine for both providers and parents and increasing parental knowledge and understanding of the HPV vaccine for their sons. Definitions: HPV4 is used to reference the quadrivalent and Gardasil® vaccinations; permissive refers to the 2009 Advisory Committee on Immunization Practices (ACIP) support of allowing adolescent males aged 9-26 to decide, in collaboration with their health care providers, to vaccinate; recommended is the ACIP's modification from permissive to routine recommendation.

Study Design: A qualitative study utilizing the Health Belief model conducted through individual interviews of parents, and providers, of 11-17 year old males in FQHCs in the Seattle area. Methods: The study involved semi-structured individual interviews with 6 parents/guardians and 3 healthcare providers of 11-17 year old adolescent boys in a FQHC setting. The questions were aimed at determining the facilitators and barriers in the uptake of HPV vaccination in this particular population from the perspective of both the parents/guardians and the providers. The interviews were recorded and transcribed verbatim for coding. The coded data was analyzed to determine common themes among parents and providers separately. Results: Several emergent themes related to HPV vaccine uptake were identified from the analyzed data from parent and provider interviews. The resulting themes included both barriers and facilitators to HPV vaccine uptake among 11-17 years old boys in FQHCs in the Seattle area. The themes involved parents, providers, healthcare system and health policy. Recommendation In the absence of a mandate for HPV vaccine as a requirement for school entry, lack of parental awareness and blind faith in providers by parents in FQHCs in the Seattle area, we suggest a strong providers’ recommendation as a mean for improving HPV vaccination uptake in this particular population. We recommend a multifaceted intervention that involves providers/staff training to effectively deliver strong recommendation for HPV vaccination to parents and a reminder system to help providers/staff deliver that recommendation urgently and consistently to 11-12 years old boys.

Book excerpt: Purpose: Many studies have assessed the public’s perceptions and acceptability of the Human Papillomavirus (HPV) vaccine, while others have analyzed the media’s coverage of the vaccine, however few have specifically assessed the media influence on the perception HPV and vaccine compliance. College freshman in fall 2013 were the first group of adolescent females targeted by the proposed Gardasil vaccine guidelines in 2006 for 11-12 year old girls, and the first group of adolescent males targeted in 2009 for 15-16 year old boys. Thus, there is limited data evaluating how this population was educated about HPV and if the media has influenced their perception and acceptance of the vaccine. The aim of this study was to evaluate this population’s education about HPV/Gardasil and determine if the media had in increased role in their reception of HPV knowledge and if the media influenced their reception of the HPV vaccine, Gardasil. Methods: One thousand randomly selected 18-19 year old freshman students at Grand Valley State University were invited to participate in this cross-sectional quantitative pilot study using an on-line, anonymous questionnaire. The 48 question survey evaluated participant demographics, basic STD knowledge and perceptions, HPV knowledge, personal sexual behaviors, acquisition of sexual health information, knowledge about Gardasil, vaccination barriers, and vaccination status. Pearson Chi-Square tests, score developments, Mann-Whitney, and Spearman’s rank correlation were used to analyze data. Results: One hundred forty six students completed the survey. The majority of participants were female (69.2%) and white, non-Hispanics (87.7%). The study revealed 68% of respondents reported obtaining HPV and Gardasil related information from the media, and 37% of respondents felt that the information presented in the media was not believable. Respondents reported receiving the most believable HPV –related information from health education classes and healthcare providers. An HPV knowledge score was developed and Mann-Whitney analysis revealed that neither gender (p-value=0.619), vaccination status (p-value=0.417), nor parent’s having a college degree (p-value=0.210) had a significant impact on HPV knowledge. Additionally, a health score analyzed the amount of interaction between healthcare providers and media exposure students received. A Mann-Whitney test revealed gender had a significant impact (p-value=0.001) on the health score, with females outscoring (M=3.27. SD=1.054) males (M=2.36, SD=1.125). Individuals who received the vaccine scored significantly higher (M=3.64, SD=0.586, p-value,0.001) than those who did not receive the vaccine ((M=2.45, SD=1.234). Additionally, 41% of the respondents had a health score of 100% with 25% of this subset reporting receiving the vaccine. Thus, despite receiving all the health information about Gardasil, 75% had not been vaccinated. Conclusion: The result of this study suggests that regardless of the amount of interaction students had with healthcare providers and /or the media, it did not affect their overall knowledge of HPV or Gardasil. It is imperative as providers, to impact accurate, honest information with patients and their families so they have information necessary to make informed decisions about their health, particularly in regards to HPV information and Gardasil vaccine.

Book excerpt: The importance of increasing human papillomavirus (HPV) vaccination coverage among adolescents is a public health imperative to prevent unnecessary morbidity and mortality associated with cervical, oropharyngeal and other HPV related cancers and conditions. Despite the availability of two vaccines that protect against 90% of genital warts and 70% of cervical cancers, national HPV vaccination rates remain below the Healthy People 2020 targets of 80% coverage for females and males. The purpose of this study is to investigate human papillomavirus vaccination behavior among rural adolescents in Georgia. Specifically, in an effort to increase HPV vaccination coverage among hard to reach populations, this study aims to understand barriers as well as facilitators to HPV initiation among adolescent males and females living in rural communities. This study used a cross-sectional survey implemented with parents of adolescents age 10-18 years old in rural communities to measure: 1) parental attitudes, 2) healthcare utilization, 3) subjective norms and 4) cues to action. A step-wise forward logistic regression analyses was conducted to better understand the correlates of HPV vaccination. A majority of the sample was African American (69%, n=131) and female (52%, n=100). The mean age was 14 years old (SD=2.1). Most adolescents were covered by Medicaid insurance (64%, n=123) and less than 2% were uninsured (1.6%, n=3). However, complex intersections of ideology, morality, and politics have made this new vaccine considerably contested, particularly as public debate has turned to the ethics of state-mandated HPV vaccination for 11-12 year old girls. Subsequently, the extent to which mandatory vaccinations are accepted by parents and implications regarding the infringement of these coercive measures on their rights to make health care decisions for their children has become powerfully positioned in public discourse. This research seeks to examine how mothers of girls conceptualize Gardasil and the potential mandates in order to illuminate the multi-faceted socio-cultural context of risk embedded within this immunization. Major themes that emerged from in-depth interviews include diverse perceptions of the risk of HPV for their daughter(s) specifically, children as actual or potential sexual beings, concerns about vaccine safety, mistrust of pharmaceutical companies and government collusion, and conceiving of vaccination against HPV as imbued with a either a moral or cancer prevention subtext. The need for collaboration and communication between the medical and governmental institutions who promote vaccines such as Gardasil and the public who politically and socially consumes them has been apparent throughout my research. Applied anthropologists have a unique role to play by situating diverse stakeholder perspectives across interdisciplinary fields in order to develop more appropriate and informed policies.

Evidence indicates that about 79 million people are affected in the United States. Every year, about 14 million Americans are infected with HPV (CDC, 2016c). It is associated with multiple diseases such as cervical cancer, vaginal and vulvar cancers, anal cancers, throat cancer and penile cancers. A safe and efficient vaccine has been approved for both boys and girls. The Center for Disease Control and Prevention announced that teens and young adult under age 26 are at a particular risk for HPV. It is essential to start the vaccination series between 9 and 17 years of age. Parental and caregivers acceptance of HPV vaccine for their children has been a problem. Despite, the HPV vaccine promotions and availability, the vaccination rates of HPV continue to low. The focus should be on parents and caregivers because parental attitudes, beliefs, knowledge, intention to vaccinate and behaviors appear to contribute to low parental acceptance of the vaccine (Gamble et al., 2010). Parental and caregiver education improved parental knowledge, intention to vaccinate and attitude towards HPV vaccination. The purpose of this project was to develop an evidence-based parental and caregiver educational intervention to improve knowledge, intention to vaccinate and attitude towards HPV vaccine, hoping to influence parents and caregivers of children ages 9 to 17 to consent to the HPV vaccine. Parental education as an intervention, focusing on HPV and HPV vaccine was found to have positive impact on parental knowledge, intention to vaccinate and parental attitude toward HPV vaccine. There was a 40%, 73%, and 33% improvement respectively on parental knowledge, attitude, and intention to vaccinate which was higher than the benchmark. Book excerpt: "Parental knowledge about Human Papillomavirus (HPV) and HPV vaccine is an important predictor factor for their children's HPV vaccine status. Studies suggest that there is the need for educational intervention to increase HPV vaccine acceptability. At a private pediatric practice in northeastern Maryland, many parents were observed to opt out of the vaccine for their children possibly due to lack of knowledge. The aims of this project were to assess parents' knowledge and to evaluate the effect of a short educational intervention on improving parents' knowledge of HPV and HPV vaccine. Changes in total knowledge score before and after the intervention were measured using a content validated HPV knowledge questionnaire, while changes in the vaccine acceptance were measured by comparing aggregate data from the same month one year prior to the start of the project. Fifty parents were recruited for this evidence based practice change project, all participants completed both pre- and post-intervention questionnaires. Parents' knowledge about HPV vaccine increased after the intervention compared to the baseline results. The pre-intervention mean score was 15.44 (30.88%) and post-intervention mean score was 19.74 (38.88%) indicating a 4.30 (8.60%) increase, and an overall increase of 48% of HPV vaccine accepted. The results of data analysis suggested that increasing parental knowledge on HPV and HPV vaccine led to an increase in the vaccine acceptance. Infection with HPV is a necessary cause of cervical. Hispanic women in the U.S. experience significantly higher rates of invasive disease than non-Hispanic Whites. In this population, HPV vaccines hold significant potential to eliminate further disparities in cervical cancer morbidity and mortality. The purpose of this study was to examine factors associated with Human Papillomavirus (HPV) vaccine acceptability among a national sample of Spanish speaking callers to the National Cancer Institute2s (NCI) Cancer Information Service (CIS). Specifically this research aimed to identify the sociodemographic, sociocultural and attitudinal determinants of HPV vaccine acceptability. This research involved a cross-sectional study with phone-based interviews conducted in Spanish (n = 836). All female Spanish callers to the CIS were asked to respond to a three-part questionnaire that included items relating to ethnic identity and acculturation, knowledge of cervical cancer and related risk factors, and HPV vaccine acceptability. Descriptive, univariate and multivariate logistic regression were used to characterize the study population and to determine the effect of each of the demographic/sociocultural variables on vaccine acceptance. Independent predictors of HPV vaccine acceptability were determined using multivariate linear regression models. Results showed that HPV vaccine acceptance was high among this group of Hispanic women (78%) and that attitudes about vaccines in general and the HPV vaccine specifically were positive. Factors associated with vaccine acceptance included physician recommendation, awareness and accurate knowledge about HPV, and speaking only or mostly Spanish. Other important predictors included influence of peers, positive attitudes about vaccines in general, higher education and being a mother of a female adolescent. The primary reason cited by those who did not favor vaccination was concern over vaccine safety. This research was the first study looking at vaccine acceptability in a large, national sample of Hispanic women. HPV vaccination can lead to important public health benefits for Hispanic women. Targeted educational interventions must take into account the important sociocultural and attitudinal influences on the decision to vaccinate, such as those identified in the present study. Future educational efforts must involve the physician and take into the account the cultural context of attitudes and beliefs regarding vaccine safety and disease susceptibility. Further studies elucidating the interplay between culture specific beliefs and practices regarding vaccination and the decision to participate in HPV vaccination are needed.
