The first was carried out in 1986 by the Health and Safety Commission Health Services Advisory Committee (1987). They conducted a comprehensive survey of the inddence of violence to 5000 workers in five separate health districts. The results from the 3000 people who eventually replied made many in the caring professions worried. One in 200 workers had suffered a major injury following a violent attack during the previous year and a further one in ten needed first aid following an assault. Other surveys also showed high risks: of sodal service staff, 6% had suffered an attack in the past 5 years (Saunders, 1987), and sodal workers were at even high er risk. 29% had been assaulted in the last 3 years (Rowett, 1986). In addition, 4% of general practitioners had experienced an attack resulting in injury in the past year (D'Urso and Hobbs, 1989). Clinical psychologists were also at risk - 53% had been assaulted at least once during their professional career and 18% in the past year (Perkins, 1991). Media reports of extreme violence seem to be the tip of the iceberg. Many staff are attacked and some of these attacks have serious physical or psychological consequences that interfere with the victim' s ability to return to their full working capadty. This loss of highly trained staff should be recognized by employers and the community.

Currently, there does appear to be a trend of increasing violence in hospital settings, including both civilly and forensically committed populations. In fact, physical aggression is the primary reason for admission to many hospitals. Given that violence is now often both a reason for admission and a barrier to discharge, there is a pressing need for violence to be re-conceptualized as a primary medical condition, not as the by-product of one. Furthermore, treatment settings need to be enhanced to address the new types of violence exhibited in inpatient environments and this modification needs to be geared toward balancing safety with treatment. This book focuses on violence from assessment, through underlying neurobiology, to treatment and other recommendations for practice. This will be of interest to forensic psychiatrists, general adult psychiatrists, psychiatric residents, psychologists, psychiatric social workers and rehabilitation therapists.

Book excerpt: Many research studies have attempted to explore the apparent relationship between mental illness and violence. These studies have generally examined the rates of violence during one of three phases: before, after, or during psychiatric hospitalization. Violence that takes place during psychiatric hospitalization has unique implications for both patients, treatment facilities, and for research on violence. This area therefore deserves special study. This article focusses on the most extreme violence that can take place on the wards of a psychiatric facility, assaults resulting in death. This article reviews the literature in this area, reports on a study of the incidence of such occurrences in New York State, and reports in some detail on an additional case which occurred prior to the period under study. The study presented here found that amongst all psychiatric facilities in New York State in a six year period there were three cases of in-patient homicide. An average of approximately 90,000 people were served within these facilities during each of the years under study. Suggestions are made for further research in this area.

Participants from 15 countries attended. Since care for persons with mental illness (schizophrenia, major depression, bipolar disorder, delusional disorder, atypical psychoses) has been deinstitutionalized, some persons with these disorders are committing crimes and serious violence. Consequently, societies around the world are confronted with a new challenge: to provide mental health care and social services to mentally ill persons in a humane way that will prevent illegal behaviours. Research in this field has been dominated by investigations designed to improve clinicians' accuracy in predicting violent behaviours, with little attention focused on the organization and implemen tation of treatments. The premise of the Advanced Study Institute was that treatments must have em pirically proven efficacy. Both professional ethics and public accountability require empirical evidence that each treatment will alleviate the problem that it targets. However, despite the fact that Western industrial societies provide treatment for mentally ill persons who have offended, there is a very limited base of knowledge on what constitutes effective treatment and how such treatments should be organized and delivered. The Advanced Study Institute was an attempt to stimulate and encour age research that will extend this knowledge base. The goals were to review what is known about mentally ill offenders and about effective treatments for them, and to provide a framework for the orientation of future investigations designed to improve treatment efficacy.
