With three added chapters and a new preface summarizing our progress in the area of pain management, this is a must-hve for those in palliative medicine and hospice care. The obligation of physicians to relieve human suffering stretches back into antiquity. But what exactly, is suffering? One patient with metastic cancer of the stomach, from which he knew he would shortly die, said he was not suffering. Another, someone who had been operated on for a mior problem--in little pain and not seemingly distressed--said that even coming into the hospital had been a source of pain and not suffering. With such varied responses to the problem of suffering, inevitable questions arise. Is it the doctor's responsibility to treat the disease or the patient? And what is the relationship between suffering and the goals of medicine? According to Dr. Eric Cassell, these are crucial questions, but unfortunately, have remained only queries void of adequate solutions. It is time for the sick person, Cassell believes, to be not merely an important concern for physicians but the central focus of medicine. With this in mind, Cassell argues for an understanding of what changes should be made in order to successfully treat the sick while alleviating suffering, and how to actually go about making these changes with the methods and training techniques firmly rooted in the doctor's relationship with the patient. Dr. Cassell offers an incisive critique of the approach of modern medicine. Drawing on a number of evocative patient narratives, he writes that the goal of medicine must be to treat an individual's suffering, and not just the disease. Book excerpt: Citizens’ aspirations for the future are politically important; they are linked to welfare and whether citizens engage in forward-looking political and economic behavior. How do natural disasters affect aspirations, and can governments’ social protection policies successfully mitigate any damaging effects? If natural disasters threaten aspirations, there is strong policy interest in understanding these threats and what government can do to protect aspirations. This article uses Pakistan’s 2010 floods to identify the effects of a natural disaster on citizens’ aspirations. Aspirations were significantly reduced—especially among the poorest and most vulnerable. However, by exploiting exogenous variation in access to targeted government social protection, the authors show that social protection following natural disasters can significantly reduce their negative aspirational effects. This offers a new understanding of government social protection. It not only raises social welfare in the short term by restoring livelihoods and replacing damaged assets; it also has an enduring effect by raising citizens’ aspirations for the future. The authors show not only that the aspirations of citizens matter for citizens’ behaviors, but also that government policies can effectively protect and increase those aspirations. This implies that the value and efficacy of government disaster relief programs are underestimated when aspirations are not taken into account.

Research in these fields may in fact provide technical knowledge and tools to support decision makers and technicians in: (i) planning mitigation and adaptation actions to climate change, extreme weather, earthquakes, drought, flooding and other natural disasters; (ii) designing efficient and sustainable strategies for resources exploitation, minimizing the impact and the unequal distributions; (iii) increasing the safety of structures and infrastructures under exceptional loadings and against the deterioration due to their lifecycle; (iv) adopting a holistic risk management approach and appropriate technologies to reduce pollution and environment deterioration, which increase vulnerability; (v) providing a safe drinking water and sanitation system to protect human health.

This unique, timely, and carefully researched monograph examines one such important yet generally under-researched and poorly understood intergovernmental organization, the Organization of Islamic Cooperation ('OIC', formerly the Organization of the Islamic Conference). In particular, it analyses in depth its institutional counter-terrorism law-making practice, and the relationship between resultant OIC law and comparable UN norms in furtherance of UN Global Counter-Terrorism Stategy goals. Furthermore, it explores two common (mis)assumptions regarding the OIC, namely whether its internal institutional weaknesses mean that its law-making practice is inconsequential at the intergovernmental level; and whether its self-declared Islamic objectives and nature are irrelevant to its institutional practice or are instead reflected within OIC law.
