Medical evacuation is the system which provides the vital linkage between the roles of care necessary to sustain the patient during transport. This is accomplished by providing en route medical care and emergency medical intervention, if required, and to enhance the individual's prognosis and to reduce long-term disability.

Book excerpt: The AHS (Army Health System) is a component of the Military Health System (MHS) that is responsible for operational management of the health service support (HSS) and force health protection (FHP) missions for training, predeployment, deployment, and postdeployment operations. The Army's MEDLOG system (including blood management) is an integral part of the AHS in that it provides intensive management of medical products and services that are used almost exclusively by the AHS and are critical to its success. Also key to this success is the delivery of a MEDLOG capability that anticipates the needs of the customer and is tailored to continuously provide end-to-end sustainment of the AHS mission throughout full spectrum operations. Providing timely and effective AHS support is a team effort which integrates the clinical and operational aspects of the mission. The provision of MEDLOG support requires collaboration between the medical logisticians, clinicians, and other health care providers within the operational environment and encompasses the following functions: Medical materiel procurement and distribution (acquisition, receiving, shipping, storage, and stock record/property accounting); Medical equipment maintenance and repair; Optical fabrication and repair; Management of patient movement items; Production of medical gases; Blood storage and distribution; Medical hazardous waste management; Management of medical facilities and infrastructure; Medical contracting support; Total product life-cycle management of medical materiel and equipment. This manual describes the capabilities of the MEDLOG system and its role in sustaining the AHS mission. Medical logistics support for deployed forces is the primary focus of this manual. However, generating force or national strategic-level MEDLOG support is also addressed to present a clear picture of the processes involved and resources expended to guarantee a Class VIII support infrastructure. This Class VIII infrastructure ensures the seamless delivery of health care from the point of injury through successive roles of care to the continental US (CONUS) support base. This publication opens with an overview of Army MEDLOG, followed by a description of each MEDLOG unit, the capabilities available, and role of care where each element may be employed. This manual also covers the information systems and enablers available to facilitate the flow of supplies and equipment throughout the area of operations (AO), as well as the current force (Medical Force 2000, Medical Reengineering Initiative, modular division, and brigade combat team [BCT] force designs) and emerging concepts scheduled to occur as part of current and future force fielding events.

Treatment direction, which is more appropriate to the patient’s condition than the guideline, should be provided by the physician as long as the CCFP scope of practice is not exceeded. OVERVIEW Clinical Practice Guidelines (CPGs) are the backbone of the system-wide JTS Performance Improvement program. Health data abstracted from patient records and after action reports is analyzed and distilled into globally relevant CPGs to remove medical practice variations and prevent needless deaths. The CPGs compiled from DoDTR data and used by healthcare providers worldwide are largely responsible for the decreased Case Fatality Rate for the wars in Iraq and Afghanistan. Examples are better transfusion practices; reduced burn morbidity and mortality; near elimination of extremity compartment syndrome; better patient care documentation; and improved communication across the spectrum of care between geographically dispersed facilities. CPGs are evidence-based and developed with experts in the military and civilian communities, deployed clinicians, Service trauma/surgical consultants, JTS leadership and formerly deployed Trauma Directors and Coordinators. JTS has a formalized process for developing, reviewing, updating, and approving CPGs. The guidelines are developed and implemented by clinical subject matter experts in response to needs identified in the military area of responsibility. CPGs were developed originally for U.S. Central Command. However, collaborative efforts are ongoing with the other Combatant Commands to customize CPGs to their COCOMs. INTRODUCTION TO THE JOINT TRAUMA SYSTEM (JTS) The Joint Trauma System (JTS) is the Department of Defense (DoD) authority for the military’s trauma care system. The vision of the Joint Trauma System is that every Soldier, Sailor, Marine and Airman injured on the battlefield will have the optimum chance for survival and maximum potential for functional recovery. To achieve this vision, in 2006, the JTS implemented programs for data -driven trauma system development and improvement in addition to the collection of trauma data. As part of its data collection efforts, the JTS maintains a registry of trauma patients who received care at medical treatment facilities (MTFs). Since 2007, this registry – known as the DoD Trauma Registry (DoDTR) – has documented demographic, injury, treatment, and outcomes data for all trauma patients admitted to any DoD MTF, regardless of whether the injury occurred during on-going military operations, and is the largest military trauma data source in the world. Development of the DoDTR began during the early years of the Global War on Terror (GWoT) when the need to systematically improve trauma care for combat wounded resulted in the impromptu creation of a demonstration registry, known then as the Combat Trauma Registry (CTR). The CTR was constructed by the Center for AMEDD Strategic Studies (CASS); trauma-related information was initially abstracted into it from paper medical records received from trauma nurse coordinators (TNCs) at Landstuhl Regional Medical Center (LRMC) in Germany. Shortly after the demonstration program started, the Army Surgeon General approved its transition to an operational mode, leading to the formation of the Joint Theater Trauma System (JTTS) and, eventually, the Joint Trauma System (JTS).

It also discusses the fundamental aspects used by medical planners to determine the best possible AHS to support Army operations. Chapter 2 describes the ten medical functions and how they are aligned with specific medical disciplines of health service support (HSS) or force health protection (FHP) or sustainment medical tasks. It also provides the primary purposes of the functions to give the medical planner a planning reference point to work from. Chapter 3 provides guidance for some of the unique complexity inherent to AHS planning. It also provides a brief review of and references the Army planning process and how it applies to AHS planning. Chapter 4 discusses some of the many different and unique factors, terms, and computation the medical planner can use to develop the AHS estimate. Appendix A provides a detailed example of the AHS estimate with planning considerations. Appendix B provides an explanation of rate calculations and provides some of the more commonly used rate formulas. Appendix C provides an example and guidance on the preparation of an AHS appendix to an operation order (OPORD) or operation plan (OPLAN). Appendix D provides a methodology to manually calculate hospital bed requirements. It includes current and historical information to perform the calculations to assist in preparing the AHS estimate.
