Nevertheless, the work Of the Society for the past year is worthy Of mention, for it has been both active and produc tive, SO much' SO that I make bold to suggest that we might with advantage add one more monthly meeting to our list, in order the better to give Vent to our somewhat pent up energies. Our best enter prise was Our excursion to Cape May to study the war cases Of Dr. Frazier and Dr. Ingham, before which we had had a special program devoted to the military experiences Of others Of our members. We have, in fact, had our minds much fixed on the neuroses Of war, until, I believe, the time has come for us to turn the page and to begin to think about the neuroses of peace. It is apparent that the dangers Of peace may be even greater than the dangers Of war, and the pano rama Of the world - as spread before us today - may make the timid exclaim with King Pyrrhus, Another such victory, and we are lost. These world phenomena appeal to us not as politicians but as physicians, not as partisans but as scientists; it is for us to discover the pathologic in them, for even in the mad Whirl Of pandemic hysterias in which SO many others are losing their heads, the neurologists may be trusted to keep theirs on their shoulders. Let us gather the fruits Of a disastrous Victory, for sometimes the gleaning Of Ephraim is better than the vintage Of Abiezer. The condition in which the world finds itself imposes an Obligation on such a society as ours. It should aim to take a lead in sane think ing; to Oppose with a persistent and trenchant criticism the vagaries Of thought in high and low places: to stem the turbid tide which bears on its bosom the flotsam and jetsam of a neurasthenic age. A recent writer in Harper's Magazine exults in the thought that this age and country is that Of the commonplace. Book excerpt: Excerpt from Archives of Neurology and Psychiatry, 1923, Vol. 9 There are few conditions the physician has to treat which cause So much distress as that of intense pain uncontrolled by drugs. A means of relief in selected cases is afforded by chordotomy. The operation Will not be entirely successful in every case in which it is employed some pain fibers occasionally will escape the knife - but the Operation has now been performed sufficiently often to justify a belief in its efficacy. It is a delicate procedure, requires a skilful technic, and is not to be undertaken lightly by one untrained in the surgery of the spinal Cord. It requires on the part of the Operator a visualization of the microscopic anatomy of the cord, and a realization that a Slight misplacement of the incision may cause motor paralysis of one or both lower limbs, depending on whether the division is unilateral or bilateral. We 1'have written. On this subject previously and therefore do not refer to the literature at the present time. We report in this paper eight cases in Which chordotomy has been performed. All but one have not been reported previously, but in this one case chordotomy for certain reasons mentioned in the notes has been performed three separate times on the same side of the cord. In one case (case the operation was performed to remove the intense hyperesthesia above the anesthetic area produced by transverse myelitis, and was entirely successful. I would like to Show how this probably occurs and how we may use the concept in our professional work. Such astute physiologists as Cannon and Carlson, after they had perfected the technique Of studying the gastric motor functions that cause an intragastric itching or crav ing, that is, hunger for food, had to depend upon an introspective study of themselves in order to correlate the type of feeling or craving which was aroused by a certain type of gastric contrae tion. This emphasizes decisively that it is Of the utmost impor tance for the physician to train himself to be able to introspectively study himself if he wishes to understand the influence Of physiolog ical processes upon the content of consciousness, i. E., if he wishes to understand the influence Of his own or the patient's cravings upon thoughts and behavior. The neurologist and physiologist can no Ionger afford to think accurately in neurological or physiological terms and loosely in psychological terms, neither can the psychiatrist or psychologist afford to think carelessly Of the physiological proc esses that determine the content of consciousness. P., a child of the feminine sex, was born on January 26, 1910. The first child of the mother, being in transverse presentation, was born by version and extraction; the second child was born by the same manipulation; the third and the fourth child were born spontaneously, but the fifth, our patient, being in transverse position, was again born by version and extraction. The doctor, when summoned, stated a first cross-presentation (dorso-anterior) and a prolapse of the umbilical cord and of one arm; the dilatation of the os externum was 7 cm. The child lived; version and extraction were performed, and by Muller's method, the after-coming shoulders were delivered. The head offered some difficulties, the child was asphyctic and was revived with some difficulty. The method used was not stated. Three days post partum, the mother noticed three ulcers on the child's rump, which grew rapidly larger and deeper. Soon afterwards she observed that the child moved its legs only feebly. According to the mother, the child had never been seen to move its lower legs, feet or toes, but she is -sure that the child could flex the thighs on both sides, though only weakly. She had not tested the sensibility, but the child, when 11/2 years old, had burned its lower leg seriously, without feeling any pain. Though it grew up prosperously (except for the lower extremities, of course) and appeared to be very intelligent, it continued to show a complete incontinentia alvi et urinae. July 2, 1912: On account of the paralysis of the legs and the decubitus, the child was brought to hospital on September 11, 1912. Of the status praesens, I have found the following notes: The head and the arms are quite normal. Intellect also normal. Motility. - The legs are spastic and paretic, both can be moved very weakly (unfortunately it was not noted what muscles still functioned). Walking and standing quite impossible. Reflexes. - The knee and ankle-jerks of both sides are greatly exaggerated. The phenomenon of Babinski is positive on both sides. Sensibility. - The tactile sense does not seem to have been examined (perhaps the child was too young to give correct answers). Pain sense not completely lost in the legs, but much decreased. Thermic sense lost in the legs and the gluteal region. Deep sensibility not examined. Large and deep bedsore on the sacrum.
